Long-term survivorship and outcomes after surgical repair of full-thickness rotator cuff tears.

INTRODUCTION: Rotator cuff injuries are commonly diagnosed orthopaedic conditions. The purpose of this study was to determine survivorship of primary open cuff repairs, with survivorship defined as a shoulder not requiring additional surgery. MATERIALS AND METHODS: Between 1993 and 2004, open cuff repairs were performed in 254 patients (263 shoulders). All had a complete tear of the supraspinatus. In addition, 86 patients had tears of the infraspinatus and 50 had subscapularis tears. Survivors did not require further surgery. Survivorship was analyzed by use of the Kaplan-Meier method, and th…

Surgical Versus Nonsurgical Management of Rotator Cuff Tears: A Matched-Pair Analysis.

BACKGROUND: Rotator cuff disease is a major medical and economic burden due to a growing aging population, but management of rotator cuff tears remains controversial. We hypothesized that there is no difference in outcomes between patients who undergo rotator cuff repair and matched patients treated nonoperatively. METHODS: After institutional review board approval, a prospective cohort of patients over 18 years of age who had a full-thickness rotator cuff tear seen on magnetic resonance imaging (MRI) were retrospectively evaluated. After clinical evaluation, each patient elected to undergo e

Identification and treatment of existing copathology in anterior shoulder instability repair.

Recurrent anterior instability is a common finding after traumatic glenohumeral dislocation in the young, athletic patient population. A variety of concomitant pathologies may be present in addition to the classic Bankart lesion, including glenoid bone loss; humeral head bone loss; rotator interval pathology; complex/large capsular injuries including humeral avulsions of the glenohumeral ligaments (HAGL lesions), SLAP tears, near circumferential labral tears, and anterior labral periosteal sleeve avulsions (ALPSA lesions); and rotator cuff tears. Normal anatomic variations masquerading as path

Asymptomatic Elite Adolescent Tennis Players' Signs of Tendinosis in Their Dominant Shoulder Compared With Their Nondominant Shoulder.

CONTEXT: Tennis is an asymmetric overhead sport with specific muscle-activation patterns, especially eccentrically in the rotator cuff. Magnetic resonance imaging (MRI) findings in asymptomatic adolescent elite tennis players have not previously been reported. OBJECTIVE: The first aim of the study was to describe MRI findings regarding adaptations or abnormalities, as well as muscle cross-sectional area (CSA), of the rotator cuff. The second aim of the study was to investigate the rotator cuff based on the interpretation of the MRI scans as normal versus abnormal, with the subdivision based o

Glenohumeral joint: internal and external rotation range of motion in javelin throwers.

OBJECTIVE: To assess differences in glenohumeral joint rotatory range of movement in javelin throwers between the throwing and non-throwing arm. METHOD: A universal 360 degrees goniometer was used to assess glenohumeral joint external and internal rotation range in 90 degrees of shoulder abduction in a group of ten senior international javelin throwers. RESULTS: Both arms had significantly greater degrees of external than internal rotation (p < 0.01), and the throwing arm had significantly greater range of external rotation than the non-throwing arm (p < 0.01). CONCLUSIONS: The presence of

[Diagnostic imaging for atraumatic shoulder pain].

Shoulder pain is a common cause of consultation in primary care medicine. A detailed history and a carefully carried out physical exam allow to orientate the diagnosis towards an intrinsic or extrinsic cause and to the differential diagnosis. Rotator cuff injury is the most frequent affection. Plain radiography will often be done in first intention since it identifies direct or indirect signs associated with certain pathologies. Ultrasonography is an excellent way of evaluating soft tissue injury and allows a diagnosis in most cases (rotator cuff injury, tendinopathy of the long head of the bi

Isolated Goutallier Grade 4 supraspinatus atrophy: a rare case without rotator cuff tear or nerve compression.

The supraspinatus muscle is susceptible to atrophy, with fatty infiltration most commonly attributed to chronic full-thickness tendon tears. Chronic denervation due to suprascapular nerve entrapment may also be responsible for supraspinatus atrophy. However, such cases typically show concurrent supraspinatus and infraspinatus involvement due to the muscles' shared innervation by the suprascapular nerve. We report a rare case of isolated supraspinatus atrophy in a 66-year-old male with intact rotator cuff tendons, preserved infraspinatus morphology, and no evidence of tendon tear or nerve compr

Rotator cuff tear arthropathy and reverse total shoulder arthroplasty: biomechanics and contemporary designs.

Rotator cuff tear arthropathy (RCTA) is a complex condition characterized by rotator cuff insufficiency, altered shoulder kinematics, and progressive joint degeneration. In advanced stages, it results in pain, loss of active elevation, and significant functional impairment. Reverse shoulder arthroplasty (RSA) is now the best surgical option for advanced RCTA because it restores shoulder elevation by changing how the joint works and making the rotator cuff less important. The original RSA concept focused on medialization and distalization of the center of rotation to increase the deltoid moment

Orthobiologic Augmentation to Improve Rotator Cuff Repair Outcomes: Current and Future Strategies.

Rotator cuff tear (RCT) management continues to represent a notable area of research given its widespread prevalence. Although current strategies of care focus on pain reduction and functional restoration, there has been an increasing amount of focus on the orthobiologics of RCTs in hopes of delivering more efficient, targeted therapies. Understanding these cellular mechanisms of rotator cuff injury outcomes is thus crucial in advancing biologic treatments; as such, this article first aims to summarize the orthobiologic mechanisms that underpin post-RCT tendon-bone healing and muscle degenerat

Biceps lasso loop and self-locking tenodesis alongside repair of rotator cuff tears: randomised controlled trial study protocol (BLAST 1).

INTRODUCTION: Pathology of the long head of the biceps tendon (LHBT) frequently accompanies rotator cuff tears, with tenotomy and tenodesis often being used to address this pathology. While meta-analyses report comparable functional outcomes between these techniques, tenotomy is linked to higher rates of Popeye deformity, whereas tenodesis is more technically demanding and might involve extra material. A novel self-locking tenodesis technique aims to reduce deformity risk while being a simpler alternative to the conventional tenodesis procedure; however, comparative evidence is currently limit

Editorial Commentary: Lower Trapezius Transfer May Be Indicated for Surgical Management of Massive Irreparable Rotator Cuff Tears in Younger, Non-Arthritic Patients.

Although shoulder rotator cuff repair fixation constructs and suture anchor design have evolved, repair of massive or functionally irreparable tears historically has relied on tendon mobilization and compression, optimizing footprint biology, and attempting a tension-free repair. However, despite these efforts, rates of failure of complete healing may be high, ranging from 20% to 94%. This has led to a search for alternative approaches, including bridging grafts, subacromial balloons, superior capsular reconstructions, biologic tuberoplasties, bursal acromial grafts, and, ultimately, renewed i

The influence of rotator cuff tear type and weight bearing on shoulder biomechanics in an ex vivo simulator experiment.

Glenohumeral biomechanics after rotator cuff (RC) tears have not been fully elucidated. This study aimed to investigate the muscle compensatory mechanism in weight-bearing shoulders with RC tears and asses the induced pathomechanics (i.e., glenohumeral translation, joint instability, center of force (CoF), joint reaction force). An experimental, glenohumeral simulator with muscle-mimicking cable system was used to simulate 30° scaption motion. Eight fresh-frozen shoulders were prepared and mounted in the simulator. Specimen-specific scapular anthropometry was used to test six RC tear types, wi

Biomechanical properties of various rat rotator cuff repair techniques.

While rat models are frequently used to study tendon healing, there is a lack of research comparing various rotator cuff repair methods in this animal model. Determining the most effective method to begin with is pivotal for biological studies focused on healing augmentation. No study to date has shown the superiority of one repair over the other for rotator cuff repair in a rat model. We performed a biomechanic study using a rat model to study the strength of four common grasping techniques. We assessed if the bone tunnel trajectory influenced the early biomechanics of the repair at postopera

Shoulder brace has no detrimental effect on basic spatio-temporal gait parameters and functional mobility after arthroscopic rotator cuff repair.

BACKGROUND: The risk of falls in patients undergoing orthopedic procedures is significant in terms of health and socioeconomic effects. RESEARCH QUESTION: Is there an influence of the shoulder abduction brace (SAB) on gait parameters in patients undergoing arthroscopic rotator cuff repair (ARCR)? METHODS: Thirty-five patients undergoing ARCR, who used a 15° SAB in the postoperative period, were included in a prospective study. Participants underwent gait analysis preoperatively (T0), 24 h after surgery (T1), 1 week (T2), and 4 weeks after surgery (1 week after SAB removal) (T3) by using a we

Rotator Cuff Repair and Overlay Augmentation by Direct Interlocking of a Nonwoven Polyethylene Terephthalate Patch Into the Tendon: Evaluation in an Ovine Model.

BACKGROUND: Arthroscopic repair of large rotator cuff tendon tears is associated with high rates of retear. Construct failure often occurs at the suture-tendon interface. Patch augmentation can improve mechanical strength and healing at this interface. PURPOSE: To introduce a novel technique for suture-free attachment of an overlaid patch and evaluate its biomechanical strength and biological performance. STUDY DESIGN: Descriptive and controlled laboratory studies. METHODS: An established ovine model of partial infraspinatus tendon resection and immediate repair was used. After a nonwoven p

Work-related outcomes of revision rotator cuff repair for patients receiving workers' compensation.

BACKGROUND: Workers' compensation (WC) patients are susceptible to poorer outcomes following primary rotator cuff repair (RCR). Failure of structural healing can explain some poor results, and outcomes of revision RCR in this population are unknown. METHODS: A retrospective review was performed of individuals receiving WC who underwent arthroscopic revision RCR with or without dermal allograft augmentation at a single institution between January 2010 and April 2021. Preoperative magnetic resonance imaging (MRI) scans were assessed for rotator cuff tear characteristics, Sugaya classification,
