Uterine inversion is a rare complication of the postpartum period which can occur in various degrees. The three key symptoms for diagnosis are pain, cardiovascular shock and vaginal bleeding combined with corresponding vaginal and uterine signs of an inversion. Early diagnosis is one of the most important factors determining the outcome of disease. METHODS/RESULTS: We describe a case of spontaneous rupture of considerable size in the renal pelvis arising from a calculus in the pelvic ureter. A significant urinary extravasation was produced which became infected. Percutaneous drainage was performed. Repair of the reptured renal pelvis and stone removal were achieved by open surgery.

BACKGROUND: Postinfarction rupture of the interventricular septum is usually fatal without surgical intervention. The optimal timing and the most appropriate technique of surgical repair remain unsettled.

Splenic rupture is an uncommon complication of colonoscopy. A high index of suspicion is a crucial factor in the prompt diagnosis of this rare but potentially fatal complication. HISTORY AND CLINICAL FINDINGS: Case 1: a 20-year-old previously healthy man sustained multiple nonvascular injuries without visible chest trauma in a car accident. Four days later a loud systolic murmur was heard over the heart. We report a case of spontaneous splenic rupture due to myelomatous infiltration of the spleen without any evidence of amyloidosis or any other complicating factors. A computer search of Medline since 1966 did not reveal any previously reported case of spontaneous splenic rupture in uncomplicated myeloma.

Two cases of obstetrical uterine rupture after operative hysteroscopy without perforation are described. One was treated for uterus septus, the other one for synechia. However, the uterus could have been fragilized in both cases: one had been perforated by a uterine device, the other case had a past history of repeated curetage. After a severe ankle sprain the incidence of residual complaints, particularly on the medial side of the joint, is high. We studied a consecutive series of 30 patients who had operative repair of acute ruptures of lateral ligaments. A 45-year-old male patient presented with rupture of the left renal pelvis after blunt abdominal trauma. The patient had preexisting bilateral megaureters with secondary obstruction of the pyeloureteral junction. The underlying disease was a persisting type III posterior urethral valve. Diagnosis of renal pelvis rupture and the underlying malformation was achieved sonographically and by abdominal CT scan. The left kidney was exposed at the time of admission and a pyeloplasty according to Anderson-Hynes was carried out. Secondarily the urethral valve was resected.
