OBJECTIVES: Ewing sarcoma radiotherapy requires precise target volume delineation. As part of the implementation of the SIOP Inter-Ewing-1 trial in France, we conducted a national benchmark and educational workshop to assess and improve delineation quality among pediatric radiation oncology centers.
METHODS: French pediatric radiotherapy centers were invited to participate in a benchmark case of postoperative clavicular Ewing sarcoma. Twelve radiation oncologists submitted initial delineations, and 8 participated both before and after an educational workshop. Delineations were evaluated quali

The World Health Organization introduced substantial revisions in the 2020 fifth edition of the classification system for bone and soft-tissue tumors, reorganizing what were previously called the Ewing sarcoma family of tumors or Ewing-like sarcomas into a new category of "undifferentiated small round cell sarcomas" based on molecular genetic characteristics. This reclassification established four distinct entities: Ewing sarcoma (ES), CIC-rearranged sarcoma, sarcoma with BCOR genetic alterations, and sarcoma with EWSR1-non-ETS fusion genes. Each subtype may demonstrate specific clinical, path

Primary sarcomas of bone are rare malignant mesenchymal tumors. The most common bone sarcomas are osteosarcoma, Ewing's sarcoma, and chondrosarcoma. The prognosis has improved over the years, but bone sarcomas are still life-threatening tumors that need a multidisciplinary approach for diagnosis and treatment. Bone sarcomas arising in the pelvis present a unique challenge to orthopedic oncologists due to the absence of natural anatomical barriers, the close proximity of vital neurovascular structures, and the high mechanical demands placed on any pelvic reconstruction following the excision of…

CASE: We describe 2 case studies, involving a 10-year-old girl with an aneurysmal bone cyst and a 12-year-old adolescent boy with Ewing sarcoma. The patient with Ewing sarcoma was previously managed with wide surgical excision and fibular graft reconstruction and subsequently experienced significant graft resorption, hardware failure, and fracture 24 months after operation. A revision limb salvage attempt was undertaken. In both cases, fibular strut grafts were harvested and fixed with intramedullary k-wires to recreate the medial and lateral columns of the distal humeral triangle.

This case highlights that gastric CIC-DUX4 rearranged sarcoma is highly aggressive and diagnostically challenging. Early comprehensive genomic profiling was essential for confirming this rare subtype. Despite transient responses to Ewing sarcoma regimens, rapid chemoresistance was observed, emphasizing the need for precise molecular diagnosis and novel therapeutic approaches.

Primary sarcoma of the lung and mediastinum is rare. The diagnosis requires careful exclusion of sarcomatoid carcinoma, sarcomatoid mesothelioma, and metastases from extra-thoracic sites. This review summarizes the key morphologic, immunohistochemical, and molecular characteristics of sarcomas that are encountered in the lung and mediastinum. The tumor types discussed are synovial sarcoma, well-differentiated/dedifferentiated liposarcoma, myxoid pleomorphic liposarcoma, intimal sarcoma of the pulmonary artery, inflammatory myofibroblastic tumor, epithelioid hemangioendothelioma, primary pulmon

MicroRNA molecules have a variety of roles in cellular development and proliferation processes, including normal osteogenesis. These effects are exerted through post-translational inhibition of target genes. Altered miRNA expression has been demonstrated in several cancers, both in the tumor tissue and in the peripheral circulation. This may influence carcinogenesis if the specific miRNA targets are encoded by tumor suppressor genes or oncogenes. To date, most research investigating the role of microRNAs and primary bone tumors has focused on osteosarcoma and Ewing sarcoma. Several microRNAs i

Subsets of primitive round-cell sarcomas remain difficult to diagnose and classify. Among these is a rare round-cell sarcoma that harbors a CIC gene rearrangement known as CIC-rearranged undifferentiated round-cell sarcoma, which is most commonly fused to the DUX4 gene. Owing to its aggressive clinical behavior and potential therapeutic implications, accurate identification of this novel soft tissue sarcoma is necessary. Definitive diagnosis requires molecular confirmation, but only a few centers are as yet able to perform this test. Several studies have shown that PEA3 subfamily genes, notabl

INTRODUCTION: Ewing sarcoma (ES) is a highly aggressive malignant neoplasm characterized by small, round, undifferentiated cells presumed neural crest origin, accounting for approximately 10-15% of all primary bone tumors. While it predominantly involves the skeletal system, ES can also arise in extraosseous soft tissues, including the retroperitoneum, thoracic wall, paravertebral region, and extremities. Primary pulmonary involvement by Ewing sarcoma is exceedingly uncommon, with fewer than 60 cases documented in the literature over the past decade.
AIM: The objective of this study was to an

The Ewing family tumors (EFT) of soft tissue represent a spectrum of neoplasms of uncertain histogenesis, arising in soft tissue without bone involvement. They include Extraskeletal Ewing Sarcoma and Primitive Neuroectodermal Tumor. These tumors are morphologically indistinguishable from Ewing sarcoma of the skeletal system. Known to be translocation-associated neoplasms, they share a common non-random translocation leading to the fusion of the EWSR1 gene on the 22q12 region, with one of the many members of the ETS family of transcription factors. Occurring mainly in adolescents and young men

BACKGROUND: Mesenchymal chondrosarcoma (MCS) is an ultra-rare sarcoma, molecularly defined by the HEY1::NCOA2 fusion, and characterized by a poor prognosis in the advanced phase. Data on the activity of systemic agents in MCS are only retrospective and limited to case reports or heterogeneous series. We report the results of an Italian, multicentric, retrospective study on the activity of cytotoxic chemotherapy in patients with MCS.
PATIENTS AND METHODS: This retrospective, multicentre Italian Sarcoma Group study included patients with molecularly confirmed MCS, treated with anthracycline-bas

UNLABELLED: Gene fusions generated by chromosomal rearrangements function as oncogenic drivers in human cancers. We previously showed that EWSR1-ETS oncofusions of Ewing sarcoma directly induce surface expression of IL1 receptor accessory protein (IL1RAP), which along with limited expression in healthy tissues except in the placenta nominate IL1RAP as a promising Ewing sarcoma immunotherapy target. We therefore engineered antibody-drug conjugates (ADC) with different cytotoxic payloads to target IL1RAP. ADCs potently blocked tumor growth and induced durable regression of Ewing sarcoma xenograf

A mouse model that recapitulates the human Ewing's sarcoma-specific chromosomal translocation was generated utilizing the Cre/loxP-mediated recombination technique. A cross between Ewsr1-loxP and Fli1-loxP mice and expression of ubiquitous Cre recombinase induced a specific translocation between Ewsr1 and Fli1 loci in systemic organs of both adult mice and embryos. As a result Ewsr1-Fli1 fusion transcripts were expressed, suggesting a functional Ews-Fli1 protein might be synthesized in vivo. However, by two years of age, none of the Ewsr1-loxP/Fli1-loxP/CAG-Cre (EFCC) mice developed any malign

Ewing sarcoma (ES) is an undifferentiated round cell sarcoma affecting children and young adults. It is characterized by gene fusions involving one of the gene members of FET family ( EWSR1 ) and ETS transcription family members. Recent studies have observed certain undifferentiated round cell sarcomas with EWSR1 -non-ETS fusions, particularly EWSR1::NFATC2 fusion. We identified 5 cases of EWSR1::NFATC2 fusion sarcomas. There was male preponderance and all the patients had a history of trivial trauma. It affected the distal femur and proximal tibia with destructive osteolytic lesions. There wa

Sarcomas represent a diverse group of mesenchymal tumors with high rates of recurrence after resection. While recent technical advances have enabled the detection of rare circulating tumor DNA (ctDNA) in other malignancies, the complexity and heterogeneity of sarcoma genomics have historically limited ctDNA in these cancers. This narrative review highlights the rapidly evolving evidence supporting potential clinical applications of ctDNA in common sarcoma subtypes including gastrointestinal stromal tumor, leiomyosarcoma, rhabdomyosarcoma, osteosarcoma, and Ewing sarcoma.

One hundred ten patients with primary chest wall neoplasms were analyzed for long-term results. The diagnosis of 59 malignant and 51 benign tumors was confirmed by the Armed Forces Institute of Pathology. No deaths were associated with primary definitive therapy. Among the five most frequently encountered malignant tumor types, five-year survivals were obtained in 9 of 17 (53%) patients with fibrosarcoma, 8 of 9 (89%) patients with chondrosarcoma, 2 of 8 (25%) patients with solitary chest wall plasmacytoma (multiple myeloma), 1 of 6 (17%) patients with Ewing's sarcoma, and 2 of 4 (50%) of pati

BACKGROUND: Epidemiological data for sarcoma in adolescents and young adults (AYAs) and across age groups are limited. We aim to: 1) update … S, respectively. Compared to children, AYAs had poorer survival for Ewing sarcoma of bone, synovial sarcoma, Ewing sarcoma of soft tissue and rhabdomyosarcoma. Compared to mature adults, AYAs had higher 5-year RS for all BS and for most of the STS subtypes. In AYAs, incidence increased for a few bone and soft tissue subtypes. Survival increased mainly for BS.

EWSR1::FLI1 is an established oncogenic driver of Ewing sarcoma, a malignant round cell sarcoma, while EWSR1::PBX3 fusions have been associated with myoepithelioma, in particular the syncytial type, which is characterized by cutaneous predilection, syncytial growth and co-expression of S100 and EMA in the absence of cytokeratin expression. We recently reported two cases of basaloid neoplasms with adnexal differentiation harboring EWSR1::FLI1 and EWSR1::PBX3 fusions. Herein, we report four additional basaloid epithelial tumors with similar morphologic features and EWSR1 fusions. We discuss the

FUS and EWSR1 are RNA-binding proteins with prion-like domains (PrLDs) that aggregate in amyotrophic lateral sclerosis (ALS) and frontotemporal dementia (FTD). The FUS and EWSR1 genes are also prone to chromosomal translocation events, which result in aberrant fusions between portions of the PrLDs of FUS and EWSR1 and the transcription factors CHOP and FLI. The resulting fusion proteins, FUS-CHOP and EWS-FLI, drive aberrant transcriptional programs that underpin liposarcoma and Ewing's sarcoma, respectively. The translocated PrLDs alter the expression profiles of these proteins and promote the

OBJECTIVES: EWS/FLI-1 fusion mainly appears in Ewing's sarcoma or the primitive neuroectodermal tumors and represents a genomic marker for these tumors. However, it can appear with lower frequency in other soft tissue tumors. The paper investigates the presence of EWS/FLI-1 fusion in clinically diagnosed sarcoma belonging to different non-Ewing connective tissue tumors in order to search for a possible new biomarker valuable for investigators.
METHODOLOGY: 20 patients with soft tissue tumors, who underwent surgery, were tested. Intra-operative samples of normal and tumor tissue were collected
