The computer-assisted surgery (CAS) has significantly improved the accuracy, reliability and outcomes of traumatic, spinal, nerve surgery and many other operations with a less invasive way. The application of CAS for scaphoid fractures remains experimental. The related studies are scanty and most of them are cadaver researches. Some intrinsic defects from the registration procedure, scan and immobilization of limbs may inevitably result in deviations. Some deviations become more obvious with operations of small bones (such as scaphoid) although they are acceptable for spine and other orthopedi

Scapho-capitate fracture is a rare lesion. Its mechanism is controversial. Forced extension seems to be the most frequent etiology, as this movement of the wrist induces scaphoid fracture by compression. Hyperextension of the wrist, as a result of the scaphoid lesion, allows contact between the posterior margin of the radius and the neck of the capitatum, inducing capitatum fracture. Continuation of the movement allows a two-stroke rotation of the capitate proximal pole. Analysis of a personal case and review of the literature demonstrated: the frequency of extension movement for etiology, del

PURPOSE: The primary aim of our study was to investigate use of long axis computed tomography (CT) in predicting avascular necrosis of the proximal pole of the scaphoid and subsequent fracture nonunion after internal fixation. In addition, we describe a new technique of measuring the position of a scaphoid fracture and provide data on its reproducibility.
METHODS: Thirty-one patients operated on by the senior author for delayed union or nonunion of scaphoid fracture were included. Preoperative CT scans were independently assessed for increased radiodensity of the proximal pole, converging tra

The scaphoid is the most commonly fractured carpal bone; despite its frequent injury, the diagnosis of fracture can be complicated by the presence of normal radiographs at the time of presentation. Clinical intuition can be increased by physical examination and immediately available modalities such as ultrasound within the emergency department. Definitive diagnosis should be made with computed tomography and magnetic resonance to verify the presence of displacement. This article provides an overview of the incidence and presentation of acute scaphoid fractures with a surgical focus on percutan

PURPOSE: Scaphoid fractures (SF) in adolescents differ from adult fractures in fracture pattern, healing capacity, and skeletal maturity. Limited data exist on factors affecting union and healing time in this population. This study evaluated clinical, radiological, and treatment-related factors associated with union and healing time in adolescent SF.
METHODS: This study included 77 adolescents (aged 10-18 years) treated for SF. Chronological age, bone age, fracture characteristics, treatment modality, union status, and healing time were recorded. Fractures were classified by anatomical locati

PURPOSE: Scaphoid non-union treatment remains nonconsensual and is based on vascularized or non-vascularized bone grafting. This study aimed to evaluate with a long follow-up the functional, clinical, and radiological outcomes, reported complications and reoperations and studied non-union treatment prognostic factors.
METHODS: Patients who had undergone bone graft surgery for scaphoid non-union were retrospectively reviewed. The evaluated outcomes were pain, qDASH, PRWE and MWS scores, active range of motion, grip strength, union rate, scapholunate angle, carpal height, and presence of arthro

INTRODUCTION: The scaphoid bone is the most commonly fractured carpal bone. MRI is now recognised to be more sensitive and specific than serial plain films in the diagnosis of occult scaphoid fracture. What is not known is whether it is safe to discharge, without follow-up, those with a normal limited extremity MRI. The primary objective of this study was to determine whether a normal limited scaphoid MRI safely excludes clinically significant injury. The secondary objectives were to determine morbidity and patient experiences of those without fracture on MRI.
METHODS: This is a cohort study …

PURPOSE: This study aims to introduce the "tripod" technique using cannulated compression screw and two anti-rotational K-wires for treatment of unstable scaphoid waist fracture nonunion, and to compare it with the single cannulated screw fixation technique in term of scaphoid union and surgical outcomes.
METHODS: It was a retrospective study. From January 2014 to March 2020, 103 consecutive patients with scaphoid waist fracture nonunion treated with the tripod fixation and bone grafting (n = 45) or with single cannulated compression screw and bone grafting (n = 58) were included. All the pro

PURPOSE: Although the consequences of scaphoid nonunion have been well-established, the implications of malunions are not well-known. To date, malunions have mainly been studied with 2-dimensional imaging. The objective of this study was to employ 3-dimensional computed tomography (CT) imaging and inter-bone distance mapping to determine the implications of a scaphoid malunion on 3-dimensional joint surface area (JSA) (a measure of joint contact area) at an average of 7 ± 2 years (range, 4-12 years) after fracture.
METHODS: In 14 subjects with previous unilateral, malunited scaphoid fractures

Scaphoid fractures are the most common carpal fracture, accounting for 70 a 80% of all fractures of the wrist. The diagnosis of acute fracture of the scaphoid still continues to be missed despite an increasing improvement of imaging techniques. In fact, these fractures are mainly neglected because of minimal early symptoms or quick resolution of initial symptoms. X-ray analysis of acute fractures is rather difficult because of the irregular shape of the scaphoid and its overlapping with other structures. In case of doubtfull or negative initial radiographic views, a technetium 99 bone scan or

The results of treatment using the Herbert screw in 50 consecutive patients with delayed or established non-union of the scaphoid are reported. 14% of these fractures failed to unite after operation and a further 20% required prolonged protection, uniting within six months. There were significant technical problems in 28% of operations. A bone graft was only used when there was significant collapse of the scaphoid (52%) and the cases without graft did just as well. Overall, the results do not support the view that this method of treatment is a significant advance over Russe grafting in terms o

BACKGROUND: A scaphoid fracture is a common injury affecting the wrist joint. A fracture of the carpus scaphoid can heal uneventfully or be complicated by non-union. Scaphoid non-union can result in persistent wrist pain, and with functional difficulties affecting all activities of daily living of the patients, this disability is expected to be significant since most of these patients are young active adults.
HYPOTHESIS: Extensive removal of the bone from the scaphoid, with the application of a large amount of cancellous bone graft and fixation with two to three wires, could lead to a high un

UNLABELLED: A case of palmar dislocation of the scaphoid and lunate which where dissociated from each other is reported with a 3.5 years follow up. A 32 years old man fell while reading his motorcycle. He landed on his left hand dorsiflexed. There was no skin dilaceration and neuro-vascular status of his hand was intact. X-rays showed a palmar dislocation of the scaphoid and lunate with a large gap between the two bones. The lunate was also completely dissociated from the triquetrum and the capitate. Distal pole of the scaphoid remained in contact with the trapezium. The patient was taken to t

This prospective study reports on the use of the Herbert screw for fixation of fractures and nonunions of the scaphoid through a dorsal approach. In cases of delayed union and nonunion, screw fixation was combined with bone grafting. Twenty-five patients were treated in this manner, and union rates of 100% for acute fractures and 87% for delayed union and nonunion were achieved. We believe that the dorsal approach provides ready access to the scaphoid, enabling placement of the screw in the best possible position to provide fracture fixation.

The scaphoid is the most commonly fractured carpal bone. Headless compression screws became the gold standard for fixation, but the ideal screw diameter remains debated. This study investigates the relative benefit of using a larger screw diameter to improve stability in typical scaphoid fractures. It also examines the effects of preload and screw length on mechanical behaviour. A finite element (FE) model of a mid-waist scaphoid fracture was created. Screws from Medartis (1.7 mm, 2.2 mm, and 3.0 mm diameter; 23 mm length) were placed along the longitudinal axis. Boundary and loading condition

BACKGROUND: Scapholunate dissociation is usually the result of failure of multiple wrist ligaments. With continued use other structures attenuate, which results in change in position of the carpal bones. It is presumed that load characteristics in the wrist joint change with changes in carpal bone position. This is thought to result in localized pressure overload and arthritic change.
QUESTIONS/PURPOSES: The purpose of this study was to evaluate radioscaphoid joint pressures and carpal kinematics after sectioning specific wrist ligaments. Our hypotheses are that there would be increased scaph

Background: Current techniques for fixation of proximal pole scaphoid fractures utilize antegrade fixation via a dorsal approach endangering the delicate vascular supply of the dorsal scaphoid. Volar and dorsal approaches demonstrate equivalent clinical outcomes in scaphoid wrist fractures, but no study has evaluated the biomechanical strength for fractures of the proximal pole. This study compares biomechanical strength of antegrade and retrograde fixation for fractures of the proximal pole of the scaphoid. Methods: A simulated proximal pole scaphoid fracture was produced in 22 matched cadave

BACKGROUND: Scaphoid fractures are a prevalent and complex issue in orthopedic practice, particularly when they lead to complications, such as nonunion and avascular necrosis (AVN), especially in proximal pole fractures. Preoperative assessment of scaphoid vascularity using magnetic resonance imaging (MRI) has become increasingly important for guiding surgical interventions, including deciding on using avascular bone grafting or vascular grafting procedures. However, there is ongoing debate regarding the diagnostic accuracy of various MRI techniques in evaluating scaphoid vascularity. This stu
