Predicting high-risk factors for postoperative inadequate analgesia and adverse reactions in cesarean delivery surgery: a prospective study.

BACKGROUND: Early identification of high-risk factors for inadequate analgesia and adverse reactions in obstetric patients is critical for improving outcomes. This study developed a machine learning model to predict these factors and optimize anesthesia management in obstetric surgery. MATERIAL AND METHODS: This prospective study included 763 obstetric patients who underwent elective cesarean delivery between December 2023 and May 2024. A machine-learning model for postoperative analgesia and adverse reactions (MLPIAAR) was constructed using 42 variables categorized into preoperative (18), in

Factors influencing deviation from target temperature during targeted temperature management in postcardiac arrest patients.

BACKGROUND: Targeted temperature management (TTM) is a recommended therapy for postcardiac arrest patients. Hyperthermia worsened the patient outcome, and overcooling increased the incidence of complications; therefore, a high-quality TTM is required. The target temperature tended to be modified worldwide after the TTM trial in 2013. Our institute modified the target temperature to 35°C in 2017. This study aimed to compare the conventional and modified protocols, assess the relationship between target temperature deviation and patient outcomes, and identify the factors influencing temperature

Clinical presentation of 54 patients with endogenous hyperinsulinaemic hypoglycaemia: a neurological chameleon (observational study).

BACKGROUND Important causes of endogenous hyperinsulinaemic hypoglycaemia (EHH) in adult patients are insulinoma and adult nesidioblastosis. Data on main symptoms in EHH are scarce and controversial. We analysed main symptoms of patients with EHH in the framework of two prospective studies investigating glucagon-like peptide-1 receptor imaging. METHODS Patients were referred from secondary European endocrine centres and endocrinologists. Inclusion criteria were biochemically proven EHH (glucose <2.5 mmol/l in the presence of inadequate insulin and C-peptide levels) with neurological hypoglycae

Esophageal Heat Transfer for Patient Temperature Control and Targeted Temperature Management.

Controlling patient temperature is important for a wide variety of clinical conditions. Cooling to normal or below normal body temperature is often performed for neuroprotection after ischemic insult (e.g. hemorrhagic stroke, subarachnoid hemorrhage, cardiac arrest, or other hypoxic injury). Cooling from febrile states treats fever and reduces the negative effects of hyperthermia on injured neurons. Patients are warmed in the operating room to prevent inadvertent perioperative hypothermia, which is known to cause increased blood loss, wound infections, and myocardial injury, while also prolong

Emergence times are similar with sevoflurane and total intravenous anesthesia: results of a multicenter RCT of patients scheduled for elective supratentorial craniotomy.

BACKGROUND: Nearly every anesthetic agent has been used for craniotomy, yet the choice between intravenous or volatile agents has been considered an area of significant debate in neuroanesthesia. We designed a Randomized Clinical Trial to test the hypothesis that inhalation anesthesia (sevoflurane/remifentanil--group S) reduces emergence time by 5 minutes compared with intravenous anesthesia (propofol/remifentanil--group P) in patients undergoing neurosurgery for supratentorial neoplasms. METHODS: Adult ASA I-III elective patients were randomly assigned to group S or P. The primary outcome wa

Thermoregulatory effects of 3,4-methylenedioxymethamphetamine (MDMA) in humans.

RATIONALE: Although 3,4-methylenedioxymethamphetamine (MDMA; Ecstasy) has been reported to cause fatal hyperthermia, few studies of the effects of MDMA on core body temperature in humans have been conducted demonstrating increased body temperature. In rats, MDMA causes hyperthermia at warm ambient temperatures but hypothermia at cold ones. OBJECTIVES: In this study, the physiological and subjective effects of MDMA in humans were determined at cold (18 degrees C) and warm (30 degrees C) ambient temperatures in a temperature and humidity-controlled laboratory. METHODS: Ten healthy volunteers w

Immediate and delayed complications of dexamethasone cyclophosphamide pulse (DCP) therapy.

Dexamethasone-cyclophosphamide pulse (DCP) is the prefered mode of therapy in pemphigus in India because it is relatively free from the side effects seen with heavy doses of daily oral steroids. One hundred forty-six pemphigus patients treated with DCP were observed for side effects of this regimen. One hundred forty mg of dexamethasone was administered IV in 200 ml of 5% dextrose over a period of 60-90 minutes on 3 consecutive days. Five hundred mg of cyclophosphamide was added on first day of the pulse and 50 mg given orally daily in the intervening period. DCP was repeated every 4 weeks and

Thermoregulation during cold exposure after several days of exhaustive exercise.

This study examined the hypothesis that several days of exhaustive exercise would impair thermoregulatory effector responses to cold exposure, leading to an accentuated core temperature reduction compared with exposure of the same individual to cold in a rested condition. Thirteen men (10 experimental and 3 control) performed a cold-wet walk (CW) for up to 6 h (6 rest-work cycles, each 1 h in duration) in 5 degrees C air on three occasions. One cycle of CW consisted of 10 min of standing in the rain (5.4 cm/h) followed by 45 min of walking (1.34 m/s, 5.4 m/s wind). Clothing was water saturated

Feasibility and Safety of Intraoperative Abdominal Warming During Spinal Surgery in the Prone Position: A Pilot Study.

BACKGROUND: Inadvertent perioperative hypothermia caused by general anesthesia leads to several complications, including shivering, coagulopathy, surgical site infections, and delayed wound healing. Prone positioning for spine surgery severely restricts the available body surface area for warming, complicating intraoperative temperature management. This study investigated a novel approach utilizing the anatomical space beneath the chest and abdomen created by a standard four-point bolster support to maintain intraoperative core body temperature using standard pediatric underbody forced-air war

Operating-room temperature and warming protocols as predictors of postoperative hypothermia in ovarian cancer surgery: A retrospective cohort study.

Perioperative hypothermia is a common but preventable complication associated with increased morbidity and delayed recovery in surgical oncology patients. However, limited evidence exists regarding its prevalence, predictors, and consequences in women undergoing cytoreductive surgery for ovarian cancer. This retrospective cohort study included 245 patients who underwent primary or interval cytoreductive surgery for epithelial ovarian cancer at a tertiary care center between 2014 and 2022. Patients were stratified into normothermia (≥36.0°C) and hypothermia (<36.0°C) groups based on core temper
