Quantitative and qualitative 5-aminolevulinic acid-induced protoporphyrin IX fluorescence in skull base meningiomas.

OBJECT: Complete resection of skull base meningiomas provides patients with the best chance for a cure; however, surgery is frequently difficult given the proximity of lesions to vital structures, such as cranial nerves, major vessels, and venous sinuses. Accurate discrimination between tumor and normal tissue is crucial for optimal tumor resection. Qualitative assessment of protoporphyrin IX (PpIX) fluorescence following the exogenous administration of 5-aminolevulinic acid (ALA) has demonstrated utility in malignant glioma resection but limited use in meningiomas. Here the authors demonstrat

Current concepts in stereotactic radiosurgery - a neurosurgical and radiooncological point of view.

Stereotactic radiosurgery is related to the history of "radiotherapy" and "stereotactic neurosurgery". The concepts for neurosurgeons and radiooncologists have been changed during the last decade and have also transformed neurosurgery. The gamma knife and the stereotactically modified linear accelerator (LINAC) are radiosurgical equipments to treat predetermined intracranial targets through the intact skull without damaging the surrounding normal brain tissue. These technical developments allow a more precise intracranial lesion control and offer even more conformal dose plans for irregularly

A Large Solitary Fibrous Tumor of the Nasal Cavity and Paranasal Sinuses Involving the Anterior Frontal Fossa: A Case Report and Review of the Literature.

The Solitary Fibrous Tumor (SFT) is a rare mesenchymal neoplasm that arises mainly from the pleura. The sinonasal tract is generally not affected by SFT, and less than 100 cases have been reported in the English literature to date. We report an extremely rare SFT of the nasal cavity and paranasal sinuses extending into the anterior frontal fossa through the floor of the anterior skull base. To our knowledge, this case is the fourth SFT of the sinonasal tract involving the anterior frontal fossa in the world. Meanwhile, the tumor, measuring 13 × 6 cm in images, is the largest SFT of the sinonas

Invasive aspergillosis of the skull base in an immunocompetent patient: a diagnostic challenge.

We describe the case of a 32-year-old man from Cape Verde having headache and increasing visual loss. Clinical and radiological investigations disclosed a massive destructive lesion involving the anterior and central skull base, orbit and nasoethmoid region initially interpreted as a malignant small round cell tumour. Surgical biopsies were negative for neoplasm, showing an intense inflammatory infiltrate together with fungus, later characterised as Aspergillus flavus spp. The patient was immunocompetent with no evidence of congenital or acquired immunodeficiencies. Invasive fungal infections

The 4 F (Fat, Fascia, Fibrin, and Fat) Technique for Skull Base Reconstruction in Endoscopic Transorbital Surgery.

BACKGROUND: Superior eyelid endoscopic transorbital approach (SETOA) has demonstrated broad versatility in addressing heterogeneous lesions involving the paramedian anterior and middle skull base in carefully selected patients. Although various skull base reconstruction techniques have shown promising results in reducing cerebrospinal fluid (CSF) leaks, no standardized method has yet been established that consistently ensures optimal outcomes in the presence of an intraoperative CSF leak to achieve a watertight seal and minimize the risk of potentially life-threatening complications. METHODS:

Assessment of Factors Associated With Internal Carotid Injury in Expanded Endoscopic Endonasal Skull Base Surgery.

IMPORTANCE: Injury to the internal carotid artery (ICA) during endoscopic endonasal skull base surgery does not typically occur as an isolated circumstance but often is the result of multiple factors. OBJECTIVE: To assess the factors associated with ICA injury in an effort to reduce its occurrence. DESIGN, SETTING, AND PARTICIPANTS: This quality improvement study used a multicenter root cause analysis of ICA injuries sustained during endoscopic endonasal skull base surgery performed at 11 tertiary care centers across 4 continents (North America, South America, Europe, and Asia) from January

Endoscopic endonasal approach for resection of cranial base chordomas: outcomes and learning curve.

BACKGROUND: Gross total resection (GTR) of cranial base chordomas represents a surgical challenge because of the location, invasiveness, and tumor extension. In the past decade, the endoscopic endonasal approach (EEA) has been used with notable outcomes. OBJECTIVE: To present the endoscopic endonasal experience in the treatment of cranial base chordomas at our institution. METHODS: From April 2003 to March 2011, 60 patients underwent an EEA for primary (n = 35) or previously treated (n = 25) cranial base chordomas. We evaluated the degree of GTR and complications. We studied the factors that

Anatomic reconstruction of the posterolateral cranial base with titanium micromesh for combined transpetrosal approach: technical note.

OBJECTIVE: We describe a new technique for the anatomic closure of the posterior petrosectomy defect after the combined supra-infratentorial transpetrosal approach for petroclival lesions. METHODS: With this technique, a titanium micromesh, tailored according to the defect of the drilled petrous bone and contoured to the outer appearance of the posterolateral cranial base, is used to cover the petrosectomy area. The mesh is fixed to the cranium and to the repositioned bone flap with titanium microscrews. RESULTS AND CONCLUSION: To date, this technique has proven to be simple and rapid and ha

How I do it: endoscopic endonasal transclival resection of ventral foramen magnum meningioma.

BACKGROUND: Surgical management of ventral foramen magnum meningiomas (FMMs) is challenging due to intervening neurovascular structures. The endoscopic endonasal transclival approach provides a direct access, offering a potential route to achieve Simpson Grade 1 resection while minimizing brainstem retraction. METHOD: We detail the technical nuances of a binostril, four-handed EEA for a ventral FMM. We explain the key anatomical landmarks required for safe decompression, including clival drilling limits, the identification of pre-medullary neurovascular structures, and the execution of a mult

Sinonasal and skull base phosphaturic mesenchymal tumours: a case series and narrative review.

Phosphaturic mesenchymal tumours (PMTs), which are common causes of tumour-induced osteomalacia (TIO), are exceptionally uncommon in the sinonasal tract and skull base. PMTs are characterized by their nonspecific location, small size, and indolent growth. When they occur in the sinonasal region and skull base, they often lack typical sinonasal symptoms, which poses significant diagnostic challenges and contributes to high rates of missed and misdiagnosis. A retrospective analysis was conducted on 5 patients with pathologically confirmed sinonasal/skull base PMTs treated at our institution from

Revisiting traditional transcranial approaches for anterior skull base meningiomas: a systematic review and meta-analysis of transcranial approaches against minimally invasive surgical techniques.

Advancements in surgical techniques and technology have led to a proliferation of less-invasive approaches for managing Anterior Skull Base Meningiomas (ASBMs), creating new challenges in selecting the optimal surgical approach. We reviewed the current literature comparing outcomes between Traditional Transcranial Approaches (TCA) and Minimally Invasive Surgical (MIS) techniques. PubMed and Embase databases were searched for studies published up to 18th November 2025. The primary outcome was extent of tumor resection. Secondary outcomes included visual improvement and post-operative complicati

Identification of BAP1 Germline Mutation in a Pediatric Patient With Skull Base Meningioma.

Genetic testing based on family history and tumor type may miss numerous germline mutations. This case report describes an adolescent girl with a rare skull-base meningioma. Initial imaging suggested schwannoma but germline genetic testing with schwannomatosis panel (NF2, LZTR1, and SMARCB1) was negative. A stepwise surgical approach prioritized facial nerve preservation, followed by tumor removal. Histopathology identified the tumor as a meningothelial meningioma, WHO grade 1, rather than the initially suspected schwannoma. Comprehensive tumor-normal sequencing revealed a germline splice site

Myeloid sarcoma of the skull base: a brief review of the literature and a rare case confused radiologically with end plaque meningioma.

BACKGROUND: Myeloid sarcoma (MS) involving the skull base is an exceptionally rare clinical entity, with limited cases described in the literature. Due to its radiological similarity to meningiomas and other extraaxial lesions, MS often leads to misdiagnosis and delayed treatment. Insights from reported cases suggest that early histopathological confirmation and systemic therapy are critical for improving prognosis. OBJECTIVE: To present a rare case of skull base MS mimicking en plaque meningioma and to contextualize its clinical, radiological, and pathological features with respect to existi
