Effects of surgical correction of nasal obstruction in the treatment of obstructive sleep apnea.

Negative upper airway pressure is thought to play a key role in the pathophysiology of obstructive sleep apnea. Because nasal resistance contributes to the increase of the transpharyngeal pressure gradient, we evaluated the effects of nasal surgery on sleep-related breathing abnormalities in 20 adults with obstructive sleep apnea. Polysomnographic studies were done before (baseline), and 2 to 3 mo after surgery (septoplasty, turbinectomy, and/or polypectomy). Nasal resistances were measured at these visits in 14 patients. Cephalometric measurements were obtained before surgery. Cephalometric a

Site of upper airway obstruction in patients with idiopathic obstructive sleep apnea.

We developed a technique to determine the site of upper airway obstruction in patients with idiopathic obstructive sleep apnea (OSA). This technique is based on the analysis of inspiratory airflow pressures at various levels of the pharyngeal airway during sleep. Pharyngeal pressure was measured by a moveable Millar catheter pressure transducer. The catheter's position in the airway was localized radiographically. Ten patients with OSA were tested: five patients were found to have upper airway obstruction at the level of the soft palate, and five had upper airway obstruction at the base of the

Multiple Drug-Induced Sleep Endoscopy in Pediatric Obstructive Sleep Apnea: Obstruction Patterns and Outcomes.

OBJECTIVES: Residual obstructive sleep apnea (OSA) following adenotonsillectomy (AT) is common in children with obesity. Drug-induced sleep endoscopy (DISE) identifies upper airway obstruction and guides surgery. A gap remains in evaluating multiple DISE-directed treatments. This study evaluates multiple DISE-directed procedure outcomes, obstruction patterns, and BMI in pediatric patients. METHODS: This single-institution retrospective cohort study included pediatric patients who underwent DISE-directed interventions from 2009 to 2020. Variables included patient characteristics, DISE timing,

Using sleep heart rate variability to investigate the sleep quality in children with obstructive sleep apnea.

BACKGROUND: Obstructive sleep apnea (OSA) is associated with impaired sleep quality and autonomic dysfunction. Adenotonsillectomy significantly improves subjective and objective sleep quality in children with OSA. However, the postoperative changes in heart rate variability (HRV) indices (indicators of cardiac autonomic function) and their importance remain inconclusive in childhood OSA. This retrospective case series aimed to investigate the association of sleep HRV indices, total OSA-18 questionnaire score (a subjective indicator of sleep quality) and polysomnographic parameters (objective i

Chronic Obstructive Pulmonary Disease and Obstructive Sleep Apnea Coexistence: From Pathophysiology to Clinical Complications.

BACKGROUND: Obstructive sleep apnea (OSA) and chronic obstructive pulmonary disease (COPD) are two common chronic respiratory diseases. Their co-existence forms overlap syndrome (OVS), a distinct clinical entity that imposes a dual burden on patients due to its unique pathophysiological mechanisms and significant clinical implications. PURPOSE: This narrative review describes the consequences of OVS by tracing its pathophysiological from molecular mechanisms to clinical phenotypes, including non-cardiovascular complications. The primary aim is to synthesize this knowledge into a conceptual fr

Obstructive hypopnea and gastroesophageal reflux as factors associated with residual obstructive sleep apnea syndrome.

OBJECTIVE: The mechanism of persisting obstructive sleep apnea (OSA) after adenotonsillectomy is not fully explained. The purpose of this study was to evaluate factors associated with residual OSA. The primary outcome measures were metabolic tests and polysomnographic respiratory indices in children with residual disease compared with children who were diagnosed with OSA but were untreated. Secondary outcome measures were acid gastroesophageal reflux indices recorded parallel to the sleep study. METHODS: In the one-year study consecutive series of patients with sleep disordered breathing hosp

Obstructive sleep apnea in children with Down syndrome: is it possible to predict severe apnea?

The objectives are to explore the demographic and polysomnographic features of children with Down syndrome and to determine the predictive factors associated with severe sleep apnea. A total of 81 children with Down syndrome referred for full-night polysomnography were analyzed. In addition, parental interviews were performed for each child. Data were available for 81 children, with a mean age of 4.8 years. Severe obstructive sleep apnea was determined in 53.1%. Age, sex, exposure to second-hand smoke, clinical findings, anthropometric features, and the presence of comorbidities were not predi

Relationship between CO2 drive and characteristics of apneas in obstructive and central sleep apnea.

Previously we showed that CO2 drive is increased in patients with obstructive sleep apnea (OSA). In the present study we would like to evaluate a possible relationship between CO2 drive and characteristics of apneas in obstructive and central sleep apnea (CSA). We compared the hypercapnic ventilatory response (HCVR) between patients with OSA and CSA. HCVR was correlated with total event time and mean event duration in both groups. 17 normocapnic patients in each group and 14 controls were studied. The apnea patients were matched for apnea-hypopnea index, age, sex, and BMI. SHCVR (slope) tended

[Electrophysiological recording of nocturnal sleep and the multiple sleep latency test in obstructive sleep apnea syndrome].

Polysomnographic studies were performed in 6 patients with obstructive sleep apnoea syndrome (OSA). The sleep study consisted of: electroencephalography, electromyography, electrooculography, electrocardiography, pulse oximetry and observation of respiration. During day multiple sleep latency tests were performed. In all patients fragmentation of sleep with prevalent stages 1. and 2. of NREM and occasionally deep sleep and REM phase were observed. Concomitantly with the appearance of electrophysiologic sleep stages the muscle tone decreased and episodes of obstructive apnoea occurred. The peri

Upper airway stimulation for obstructive sleep apnea: Treatment outcomes by sleep position and sex.

Obstructive sleep apnea (OSA) severity varies by sleep position and sex; however, it is unknown whether these factors affect hypoglossal nerve stimulation (HGNS) treatment effectiveness. Thus, this study evaluates HGNS control of OSA by sleep position and sex. To enhance real-world clinical applicability of results and address concerns regarding appropriate postoperative sleep study criteria to evaluate HGNS outcomes, our study also proposes and utilizes a standardized HGNS titration study scoring protocol to assess apnea-hypopnea index (AHI). A retrospective cohort of adult patients underwent

Deep learning-based automatic scoring of drug-induced sleep endoscopy in obstructive sleep apnea.

Polysomnography is the standard tool for assessing obstructive sleep apnea (OSA) severity; however, it does not provide information regarding the anatomical site or extent of upper airway obstruction. Drug-induced sleep endoscopy (DISE) serves as a dynamic method to evaluate airway collapse under sleep-like conditions, thereby helping to bridge this gap. However, its clinical utility is limited by inter-observer variability and subjectivity in interpretation. We developed internally and externally validated deep learning models utilizing convolutional neural networks based on EfficientNet-B2 a

Precision in Pediatric Obstructive Sleep Apnea: Preventing Unnecessary Surgery Using Drug-Induced Sleep Endoscopy.

Pediatric obstructive sleep apnea (OSA) is often multifactorial, and awake airway assessment may fail to accurately identify the sites of upper airway obstruction. Drug-induced sleep endoscopy (DISE) enables dynamic evaluation of the upper airway under sleep-like conditions and may be particularly valuable in children at an increased risk of persistent OSA after adenotonsillectomy. We report the case of a 10-year-old girl with severe OSA, obesity (body mass index: 31.04 kg/m², above the 99th percentile), and neurocognitive impairment, in whom awake assessment suggested adenotonsillar hypertrop
