OBJECTIVE: This study provides prevalence data of mental disorders (4-week, 1-year, lifetime) and psychological distress in Adolescent and Y … ce rates. The lowest prevalence rates were reported for alcohol dependence (0.8%; 95%-CI:0%-2%) and somatoform disorders (3.7%; 95%-CI:1%-7%). The 1-year-prevalence was 55.4% (CI:47.36-62.64), and the lifetime-prevalence was 69.5% (CI: 62.29-77.06).
CONCLUSION: Our findings may sensitize clinicians to the possible presence of mental disorders in AYA. The results indi …

BACKGROUND: Mood disorders are often associated with somatic symptoms. The role of somatic symptoms on disease progression in unipolar depression is substantially better characterized than that role in bipolar disorder. Moreover, the contribution of comorbid anxiety disorders and medical illness is not well understood.
METHOD: We investigated 527 patients with bipolar I disorder clustered within 102 families using a latent class approach. Predictors were added stepwise into the model. Anxiety and commonly associated medical illnesses were added as covariates.
RESULTS: The rate of somatic sym

Anxiety disorders tend to become chronic if untreated and often have considerable social consequences. Therefore, early recognition of anxiety disorders at the primary health care level is of great importance. This article gives a short overview of important aspects of the diagnosis of anxiety disorders in general medical practice. Panic disorder, generalised anxiety disorder and phobic disorders are describer and phobic disorders are described based on ICD-10 criteria. Anxiety disorders have important differential diagnostic distinctions from somatic disorders and other psychiatric disorders.

Relations among measures of trait anxiety, depression, panic, somatization, alcohol use, drug use, and treatment for depression were investigated because, typically, studies (a) addressed relations among subsets of only 2 or 3 of the measures and (b) dealt almost exclusively with narrow samples of the population representing extremes on 1 or 2 of the measures. In this study, relations among all 7 measures were assessed with participants representing a wide range of scores on all the measures. The 369 participants (155 men, 214 women) were sampled from the general population. Three replications

BACKGROUND: Somatic Symptom Disorder (SSD) is defined as persistent and clinically significant somatic symptoms accompanied by excessive and inappropriate health-related thoughts, feelings and behaviours. The causes of SSD are not fully understood, but the biopsychosocial model posits that they result from vulnerability to both precipitating and maintaining factors that contribute to the progression of symptoms from short-term/acute distress presentation to a more persistent and even disabling problem.
CASE PRESENTATION: We report the case of a patient who developed a SSD after a prolonged pe

OBJECTIVE: To identify risk factors for persistence of functional somatic symptoms (FSS; ie, somatic symptoms that cannot be sufficiently explained by underlying organic pathology).
STUDY DESIGN: The first (N = 2230, mean age = 11.1 years [SD 0.6], 50.8% girls), second (N = 2149, mean age = 13.7 years [SD 0.5], 51.0% girls), and third (N = 1816, mean age = 16.3 years [SD 0.7], 52.3% girls) assessment waves of the general population study TRacking Adolescents' Individual Lives Survey were used. FSS were assessed with the Youth Self-Report and the Child Behavior Checklist. Growth mixture models

This special issue is devoted to the topic of medically unexplained symptoms (MUS), a heterogeneous group of conditions characterized by persistent physical symptoms that cannot be explained by medical illness or injury. Although psychological factors have long been regarded as central to these problems, patients with MUS have typically been managed within medical settings and referrals to mental health services have been relatively rare. In recent years, however, interest in the psychological nature and treatment of MUS has expanded, culminating in the development of tailored psychological in

Retrobulbar neuritis is often very complicated clinical entity. The most common cause of retrobulbar neuritis is demyelinating disease of CNS. This report is to express some other uncommon causes of it. Three children, age 8 to 12 with sudden and severe visual loss are presented. The diagnosis of retrobulbar neuritis is made by complete ophtalmological examination in consultation with neuropediatrics and neuroradiologist. Different ethiological causes of retrobulbar neuritis are found: pranasal sinusitis, functional visual loss and pseudotumor cerebri. In first two children complete therapeuti

The authors revealed numerous somatic dysfunctions in vibration disease. The dysfunctions appeared to correlate with the vibration type and to play a role in pathogenesis of pain, tunnel and sensory polyneuropathy syndromes. Manual therapy proved effective in correction of somatic dysfunctions in vibration disease.

Polydipsia, unrelated to organic polyuria is present in 20% of the patients hospitalised in psychiatry on a long term basis. It seems that about a third of them have presented at some point an episode of water intoxication. Most patients with psychogenic polydipsia excrete all the water they absorb. In a subgroup of them, at risk for water intoxication, the capacity to excrete water is deficient. This is usually due to an inappropriate secretion of antidiuretic hormone. The first two steps in the treatment of patients with psychogenic polydipsia consist in making the diagnosis and in evaluatin

The Freiburg Complaint List (Freiburger Beschwerdenliste FBL-R) is a self-report questionnaire which consists of 80 somatic complaints. A survey was conducted in 1993 and a sample of N = 2070, representative for the German adult population of 16 years and above, was obtained. Factor analysis, cluster analysis and item analysis were employed to develop nine scales: General Condition, Tiredness, Cardiovascular, Gastrointestinal, Head-Throat, Tenseness, Emotional Reactivity, Pain, Sensory and Total Score. Regression analyses revealed substantial effects of gender, age group, and, to a lesser exte

The development of somatization disorder is supported by the way physicians focus on somatic complaints but fail to effectively diagnose and relieve them if a specific anatomic or biochemical lesion is not found. This inadequate treatment of somatizing patients may related to the focus of medical teaching as well as male skills, styles and vulnerabilities.

Patients with reflex sympathetic dystrophy are often considered by physicians and allied health personnel as having a peculiar personality. In medical literature they are frequently described as anxious and depressive, emotional, nervous and irritable patients with neurovegetative instability. A review of the literature on psychological research in this field is not always illuminating. Hypochondria and hysteria, whether or not accompanied by depression, are frequently reported to be typical traits, whereas other findings point more in the direction of psychosis. Increased anxiety, emotional l

The aim of this study was to examine the prevalence of 'functional' somatic symptoms in general practice and the factors associated with reporting these symptoms. During a one month period, all attenders aged 16 years and over at a general practice near Leeds were screened for functional somatic symptoms using the Bradford somatic inventory. The general practitioner recorded the patients' personal data and diagnostic assessment. Data were analysed from 670 Europid patients who completed the Bradford somatic inventory at their first attendance during the month. Higher mean numbers of functional

One hundred adults with a chief complaint of chronic fatigue were evaluated in an outpatient setting to determine a possible association with somatization disorder. Somatization disorder was diagnosed in 15 patients. Eight functional somatic symptoms were reported more frequently by these patients: pain in extremities, joint paint, chest pain, other pain, shortness of breath, blurred vision, muscle weakness, and sexual indifference (p less than 0.001). Current mood disorders, anxiety disorders, and psychoactive substance use disorders were less common in patients with somatization disorders th

The current definition of Somatization Disorder requires the assessment of each of 35 symptoms, 13 of which have to be scored as meeting special criteria (e.g., not due to medical reasons) in order to make the diagnosis. The detailed questioning and probing about each symptom can be quite elaborate--hence the interest in developing briefer screening indexes. This study evaluated two sets of screening indexes for Somatization Disorder (those of Othmer and DeSouza and Swartz and colleagues) that had been previously developed in psychiatric and community samples. One hundred medical outpatients w

In addition to being a risk factor for the course of chronic pain, the personality characteristics of the individual attachment style are also predictors for the success of medical and psychosocial interventions and aspects of the physician-patient relationship. Insecurely attached patients seem to be less able to sustain the positive effects of pain therapy. These results are especially relevant as insecure attachment patterns are overrepresented among chronic pain patients. As a result the attachment style can be seen as a psychosocial vulnerability factor for the chronification of acute pai
