Petrous apex cholesterol granuloma involving the sphenoid sinus.

Petrous apex cholesterol granulomas are slowly expanding lesions that clinically present with a variety of cranial nerve deficits. We present a case of a 40-year-old man with a right-sided, sudden-onset hearing loss. Apart from a 60-dB high-frequency sensorineural hearing loss in the right ear, all other neurotologic examinations were normal. Computed tomography revealed partial destruction of the right carotid canal, petrous apex, and clivus. Surgical treatment was performed via an endoscopic transnasal, trans-sphenoid approach using an image-guidance system. Postoperative magnetic resonance

Tension forces in relation to LeFort III osteotomies.

Complications associated with LeFort III osteotomies have led to modifications of the procedure. Attempts to decrease morbidity include performing incomplete osteotomies or eliminating pterygomaxillary dysjunction. The purpose of this study was to determine the amount of tension required for separation of the midface from the pterygoid plates and to observe the fracture patterns. An Instron machine (Instron Corporation, Canton, MA) was used to measure the tension forces in 16 cadaver skulls divided into five groups. The groups varied as to whether the osteotomies were complete and whether pter

Sphenoidal artery, ramus orbitalis persistens and pterygospinosus muscle--a unique cooccurrence of first branchial arch anomalies in man.

In a 76-year-old Caucasian male, the right middle meningeal was replaced by a branch of the ipsilateral ophthalmic artery (O), i.e. the sphenoidal artery (S); the right foramen spinosum was found to be absent. In the orbit, the right O bifurcated into a medial (merely orbital) and a lateral division, i.e. a meningolacrimal trunk. From that trunk, the S branched off and entered the middle cranial fossa at the superolateral angle of the superior orbital fissure. In addition, an anastomotic channel was formed by a remnant of the ramus orbitalis, connecting the right infraorbital with the deep orb

Clinoidal meningiomas.

Anterior clinoidal meningiomas are frequently grouped with suprasellar or sphenoid ridge meningiomas, masking their notorious association with a high mortality and morbidity rate, failure of total removal, and recurrence. To avoid injury to encased cerebral vessels, most surgeons are content with subtotal removal. Without total removal, however, recurrence is expected. Recent advances in cranial-base exposure and cavernous sinus surgery have facilitated radical total removal. The author reports 24 cases operated on with vigorous attempts at total removal of the tumor with involved dura and bon

[Primary intracranial yolk sac tumor developing in the frontal lobe from the inside of the sphenoidal ridge].

A case of primary intracranial yolk sac tumor, the first known case growing in the frontal lobe, is compared with similar cases of suprasellar region. The case, 18-year-old female, suffered from headache, vomiting and visual disturbance for one month prior to the hospitalization. Plain CT scan demonstrated suppressed left anterior horn and normal density area in front of it. After injection of contrast medium, the area was enhanced distinctly. The left carotid angiography displayed a hypervascular mass in the suprasellar region and tumor stain was also seen in the capillary phase. Bilateral fr

[Pneumatocephalus: a clinical contribution (author's transl)].

Treatment of traumatic brain oedema leads to a decrease in raised intracranial pressure, and this may facilitate formation or progression of pneumatocephalus. Lumbar puncture or lumbar CSF drainage for treatment of CSF-rhinorrhoea/otorrhoea is occasionally responsible for the occurrence or enhancement of pneumatocephalus. Pneumatocephalus with radiological absence of fracture of the skull and absence of CSF leakage signifies communication between rhinocranium and encephalocranium: this supplies evidence of an open head injury. Post-traumatic opacification of the sphenoid sinus and presence of

Incidence and risk factors of hyponatremia after transsphenoidal surgery: a systematic meta-analysis.

The transsphenoidal approach to sellar lesions is a mainstay technique in modern neurosurgical treatment of pituitary adenomas. One prominent complication following transsphenoidal surgery is the development of postoperative hyponatremia, frequently necessitating additional medical management and hospital readmission. However, the precise incidence and risk factors of postoperative hyponatremia remain unclear in the current literature. We conducted a systematic meta-analysis of 104 studies (31,676 patients) to clarify the incidence of postoperative hyponatremia and associated hospital readmiss

Optimizing the Endoscopic Transorbital Approach: MacCarty Keyhole for Enhanced Surgical Exposure in the Opticocarotid Region.

BACKGROUND: In recent years, the endoscopic transorbital (TO) approach has gained increasing interest for the treatment of middle cranial fossa lesions. We propose a technical refinement to the conventional superior eyelid TO approach, which improves the surgical exposure and augments the working angles when targeting the opticocarotid region. METHODS: Four embalmed adult cadaveric specimens (8 sides) were dissected at the Laboratory of Surgical Neuroanatomy of our institution. A TO approach was performed, with removal of the anterior clinoid process and the lateral orbital rim. Subsequently,

Different types of combined endoscopic and transcranial approaches for complex giant pituitary adenomas: how I do it.

BACKGROUND: Surgery for giant pituitary adenomas (GPAs) poses significant challenges, particularly when the tumor cannot be fully addressed using either the endoscopic endonasal approach (EEA) or the microscopic transcranial approach (MTCA) alone. METHOD: We utilized two combined approaches for complex GPAs: 1) expanded endoscopic endonasal approach (EEEA) combined with microscopic pterional approach (MPA), and 2) EEEA combined with endoscopic transventricular approach (ETVA). CONCLUSION: The combined approach is an effective surgical technique for complex GPAs. The EEEA-MPA approach is suit

Pulsatile Proptosis and Sphenoid Wing Dysplasia with no Evidence of Neurofibromatosis Type 1: A Case Report and Review of the Literature.

In this study, we aimed to present a rare case of pulsatile proptosis due to sphenoid wing dysplasia without the features of neurofibromatosis type 1 (NF1). A 17-year-old male patient presented with swelling in the superotemporal region of the right eye. Physical examination revealed facial asymmetry with a pulsatile, ill-defined, soft lesion with in the superotemporal region of the right orbit associated with pulsatile proptosis, downward dystopia, and hypotropia. Computer tomography imaging to establish a differential diagnosis showed temporal lobe herniation secondary to sphenoid wing dyspl

3D Printing as a Planning Tool to Optimize Sinonasal Irrigation.

BACKGROUND: Topical sinus irrigation plays a critical role in the management of sinonasal diseases. Yet, the penetration of irrigant to targeted sinuses may be highly variable and difficult to predict. Here, we investigate the use of 3D printing as a planning tool to optimize outcomes. METHODS: Eight post-operative models were 3D printed with a FormLabs Form3 printer based on individual CT scans. Irrigations were performed and video recorded with a squeeze bottle attached via silicon water-tight seal, in 4 head positions: 45° to-the-side, 90° to-the-side, 45° forward and 45° to-the-side, and

Delayed coil migration into oropharynx following endovascular coiling of a traumatic carotid cavernous fistula: management considerations.

Carotid-cavernous fistulas (CCFs) are abnormal vascular shunts between the carotid artery and the cavernous sinus. A 37-year-old male presented with a traumatic CCF and basal skull fracture extending through the medial wall of the cavernous sinus and sphenoid sinus. The CCF was treated with endovascular coiling. Three months after this procedure, he was found to have coil migration through the traumatic sphenoid defect into the pharynx. He underwent urgent endonasal endoscopic surgery to disconnect and remove the extruded coil. Post-operative coil migration is a rare but serious complication f

Clinically non-functioning pituitary adenomas.

Clinically non functioning pituitary adenomas (NFPAs) include all pituitary adenomas that are not hormonally active. They are not associated with clinical syndromes such as amenorrhea-galactorrhea (prolactinomas), acromegaly, Cushing's disease or hyperthyroidism (TSH-secreting adenomas) and are therefore usually diagnosed by signs and symptoms related to a mass effect (headache, visual impairment, sometimes pituitary apoplexy), but also incidentally. Biochemical work up often documents several pituitary insufficiencies. In histopathology, the majority of NFPAs are gonadotroph. In the absence o
