The manifestation of cocaine-induced midline destructive lesion in bone tissue and its identification in human skeletal remains.

Cocaine-induced midline destructive lesion (CIMDL) is a condition that may arise in response to chronic insufflation ("snorting") of cocaine. It is clinically diagnosed when the nasal septum, lateral nasal walls, and/or hard palate show signs of destruction in association with cocaine use. Although its true incidence is unknown, CIMDL is not an uncommon clinical finding amongst intranasal cocaine abusers and is likely to be encountered by forensic anthropologists and medical examiners working worldwide. Given the preponderance of drug abusers amongst the subjects of forensic casework, the abil

A revised stylized model of the adult extrathoracic and thoracic airways for use with the ICRP-66 human respiratory tract model.

The extrathoracic airways and lymph nodes have not yet been represented explicitly in mathematical or stylized models of the human body utilized in the transport of photons internally between source and target organs. Currently, the ICRP assumes that the extrathoracic airways are reasonably approximated by using the thyroid or brain as the surrogate source and target region within the ICRP 66 respiratory tract model. In the present study, a new mathematical model was created to explicitly consider the extrathoracic airways, as well as other respiratory structures in the thorax of the adult. Th

Inverted papilloma of the nasal cavity and paranasal sinuses: clinical data, surgical strategy and recurrence rates.

BACKGROUND: In the last decade inverted papillomas of the nasal cavity and paranasal sinuses have been observed in increasing numbers, and treatment modalities have ranged from extensive open radical procedures to microinvasive endonasal surgical excision. OBJECTIVE: To establish criteria for selecting patients for open osteoplastic or endonasal surgery according to clearly defined pathological and clinical data. MATERIAL AND METHOD: In a retrospective study, clinical data of 55 patients treated surgically in the University ENT Clinic Giessen from 1991 to 1998 were analysed. In 33 patients (

Challenge of identifying a case of chronic invasive rhinosinusitis due to Rasamsonia aegroticola.

Fungal rhinosinusitis are frequently described in humans and its prevalence appears to be increasing. They mainly involve species of the genus Aspergillus. To date, conventional mycological diagnosis remains challenging, as only 30% and 50% of cultures are positive in cases of fungal balls and invasive rhinosinusitis respectively. Over the past decade, human infections due to the Rasamsonia argillacea species complex are increasingly frequent and mainly concern patients with cystic fibrosis or chronic granulomatous disease. We report here the first case of pseudotumoral R. aegroticola sinusiti

Subperiosteal abscess of the orbit with multiple abscesses in the brain and body due to Chromobacterium violaceum sepsis in an infant: a case report and review of the literature.

Chromobacterium violaceum is a rare pathogen known to cause severe, life-threatening infections in humans. Its typical clinical presentations include skin and soft tissue infections, fulminant septicemia, multiple visceral abscesses, and diarrhea. We report the case of a 2-month-old male infant who initially presented with acute fever and subsequently developed acute proptosis of the right eye, accompanied by a palpable mass in the right mastoid area. Computed tomography (CT) scans of the orbits, paranasal sinuses and brain revealed a subperiosteal abscess at the right sphenoid wing, right oto

Rhizopus microsporus and Mucor racemosus coinfection following COVID-19 detected by metagenomics next-generation sequencing: A case of disseminated mucormycosis.

Mucormycosis is an invasive opportunistic fungal infection, which may be lethal and mostly affects patients with immunodeficiency or diabetes mellitus. Among Mucorales fungi, Rhizopus spp. is the most common cause of mucormycosis, followed by genera such as Mucor and Lichtheimia. Here we report a patient with severe COVID-19 infection who developed nasal pain, facial swelling, prominent black eschar on the nasal root. CT scan revealed pansinusitis along the maxillary, ethmoidal, and sphenoid sinuses. Mixed mold infection with Rhizopus microsporus and Mucor racemosus was detected by blood metag

Review of olfactory cleft roof anatomy.

Progress in skull-base endoscopic surgery has improved our knowledge of olfactory cleft (OC) anatomy. This article presents a review of current knowledge concerning the OC roof: descriptive anatomy, radiological exploration, and endoscopic observation. The OC is a narrow area in the most superior part of the nasal cavity. Its roof is the thinnest structure separating the nasal cavities from the brain; it comprises three superimposed tissues: nasal mucosa, ethmoid cribriform plate (ECP), and dura mater. The ECP comprises the anterior ECP containing the ethmoidal slit (ES) medially and the cribr

Anatomy of the Orbit.

The orbit is a paired, transversely oval, and cone-shaped osseous cavity bounded and formed by the anterior and middle cranial base as well as the viscerocranium. Its main contents are the anterior part of the visual system, globe and optic nerve, and the associated neural, vascular, muscular, glandular, and ligamentous structures required for oculomotion, lacrimation, accommodation, and sensation. A complex stream of afferent and efferent information passes through the orbit, which necessitates a direct communication with the anterior and middle cranial fossae, the pterygopalatine and infrate

Sellar chondrosarcoma presenting with amenorrhea: A case report.

RATIONALE: Intracranial chondrosarcomas are rare entities and most of which arise off the midline. Chondrosarcomas that occur in the sellar region are extremely rare, and to the best of our knowledge, there is no reporting about sellar chondrosarcoma with amenorrhea as the onset symptom. PATIENT CONCERNS: A 45-year-old woman presented with a 7-month history of amenorrhea and a 3-month history of progressive visual loss in the left eye. DIAGNOSIS: The patient was diagnosed with recurrent sellar chondrosarcoma arising from intrasellar with extensive tumor invasion into bilateral sphenoid sinus

Acute Ischaemic Stroke as a Manifestation of Pituitary Apoplexy in a Young Lady.

Pituitary Apoplexy (PA) is defined as a clinical syndrome comprising headache, visual deficits and altered sensorium, which can result from haemorrhage or infarction of the pituitary gland. Acute ischaemic stroke following PA is very rare. We are presenting a 35-year-old young otherwise healthy lady who presented with neuro ophthalmological and vascular symptoms on a background of PA. Imaging revealed a pituitary macro adenoma with parasellar extension with internal bleed. Cerebral angiography revealed that the mass compressed the bilateral cavernous sinuses (left more than right), resulting i

Computed tomographic anatomy of the nasal cavity, paranasal sinuses and tympanic cavity of the koala.

OBJECTIVE: To use cross-sectional imaging (helical computed tomography (CT)) combined with conventional anatomical dissection to define the normal anatomy of the nasal cavity and bony cavitations of the koala skull. METHODS: Helical CT scans of the heads of nine adult animals were obtained using a multislice scanner acquiring thin slices reconstructed in the transverse, sagittal and dorsal planes. Subsequent anatomical dissection permitted confirmation of correct identification and further delineation of bony and air-filled structures visible in axial and multiplanar reformatted CT images. R

Facial reconstruction of a pathological case.

In this paper, we describe a forensic paleopathology case using findings obtained from images of a skull with leontiasis ossea. This unique specimen is on display in the National Museum of Anthropology and History in Mexico. The skull shows tissue overgrowth, periosteal bone proliferation, which produced a cortical and diploid thickening involving the entire bone matrix. The study of images, X-rays, and helical computed tomography revealed generalized hyperostosis obliterating the maxillary and sphenoid sinuses, and two exuberant bony masses arising from the maxilla with encroachment of the an

[Epidemiological analysis of the incidence of invasive and non-invasive pneumococcal infections in different population groups].

Despite modern achievements in diagnostics and treatment, invasive and non-invasive pneumococcal infections remain a topical public health problem. To a large extent, it is connected with the absence or inconsistence of evidence-based information on this kind of infection. In this paper, retrospective analysis of the incidence of pneumococcal infections was performed on the basis of medical records available today in every health institution; the peculiarities of their nosologic structure were revealed. Among invasive forms, pneumococcal pneumonias prevailed (50.06%); apparent hypodiagnostics
