The mechanical consequence of failure of ossified union in attempted posterior spinal fusion. A canine model.

The mechanical behavior of pseudarthrosis in posterior spinal fusion was investigated. A canine model was developed in which an incompletely ossified posterior fusion mass was consistently produced. The spines were excised, and the motion segments were mechanically tested using a specially developed loading apparatus. Tests were performed to evaluate stiffness of the segments to loading with compression, torsion, and anterioposterior and lateral bending shear stiffness. Changes in other modes of loading were less consistent. The motion characteristics of the pseudarthrosis could not be predict

Clinical, pathological and genetic features as well as follow-up of 68 patients with late-onset Pompe disease: a single-center retrospective study.

INTRODUCTION: Pompe disease is a muscular lysosomal storage disorder characterized by autosomal recessive inheritance and caused by deficiency of the acid alpha-glucosidase (GAA) enzyme. Late-onset Pompe disease (LOPD) exhibits heterogeneous clinical presentations, which are influenced by the type of GAA mutation and residual enzyme activity. METHODS: In this study, we conducted a retrospective analysis of 68 Chinese LOPD patients over a 17-year period at a single center to delineate the real-world disease status and survival outcomes. RESULTS: Among the 47 patients who received enzyme repla

Stand-alone lumbar cage subsidence: A biomechanical sensitivity study of cage design and placement.

BACKGROUND AND OBJECTIVE: Spinal degeneration and instability are commonly treated with interbody fusion cages either alone or supplemented with posterior instrumentation with the aim to immobilise the segment and restore intervertebral height. The purpose of this work is to establish a tool which may help to understand the effects of intervertebral cage design and placement on the biomechanical response of a patient-specific model to help reducing post-surgical complications such as subsidence and segment instability. METHODS: A 3D lumbar functional spinal unit (FSU) finite element model was

Shoulder-Level Asymmetry Pre- and Post-Posterior Spinal Fusion in Adolescent Patients with Idiopathic Scoliosis.

Background: Posterior spinal fusion is the mainstay of treatment for Cobb angle over 50 degrees with satisfactory long-term results. In the surgical management of scoliosis, surgeons usually focus on the amount of coronal curvature correction because it can determine the surgical outcome. Nevertheless, there are many factors that contribute to patients' satisfaction after surgery, and achieving shoulder balance is one of the most vital factors of a successful surgery. Our objective is to study the differences in managing idiopathic scoliosis with pedicle screws versus hybrid fixation with rega

Is Obstructive Sleep Apnea-Associated Adenoid Hypertrophy Linked to Scoliotic Attitudes in Children?

Introduction Scoliosis, a multifaceted spinal deformity commonly affecting pediatric and adolescent populations, has spurred extensive scientific inquiry to understand its origins and impacts. Early-onset scoliosis (EOS), characterized by spinal curvature exceeding 10° before the age of 10, presents a unique challenge necessitating a comprehensive understanding of its etiological factors. Within this context, the potential role of hypoxia-induced by adenoid hypertrophy in contributing to the pathogenesis of EOS has emerged as an intriguing avenue of investigation. Materials and methods This re

The Relation Between Sacral Angle and Vertical Angle of Sacral Curvature and Lumbar Disc Degeneration: A Case-Control Study.

The purpose of this study is to determine the reliability and validity of a goniometric measurement of the vertical angle of the sacrum and sacral angle (SA), and their relationships to lumbar degeneration.A herniated lumbar disc is one of the most frequent medical issues. Investigators in a number of studies have reported associated risk factors for prevalent disc degeneration. Atypical lumbosacral angles and curvature are thought to contribute to the degradation of the spine by many researchers. This study analyzed 360 patients referred to our clinic from 2013 to 2015 due to low back pain. A

Effect of leg length discrepancy on spinopelvic alignment and mobility in healthy volunteers using a shoe lift.

BACKGROUND: Leg length discrepancy (LLD) is a musculoskeletal condition in which leg length is asymmetric. LLD can adversely affect adjacent joints. A typical example is hip-spine syndrome. We aimed to investigate how LLD affects spinopelvic alignment and mobility, under conditions simulating LLD. METHODS: Fifty healthy adults (31 women, 19 men ; mean age 37.1 ± 8.9 years) participated in this study. A shoe lift was applied under the left foot to simulate LLD of 1, 2, 3, and 4 cm. Spinopelvic alignment while standing upright and mobility in flexion, extension, and lateral bending were measure

The Reissner Fiber in the Cerebrospinal Fluid Controls Morphogenesis of the Body Axis.

Organ development depends on the integration of coordinated long-range communication between cells. The cerebrospinal fluid composition and flow properties regulate several aspects of central nervous system development, including progenitor proliferation, neurogenesis, and migration [1-3]. One understudied component of the cerebrospinal fluid, described over a century ago in vertebrates, is the Reissner fiber. This extracellular thread forming early in development results from the assembly of the SCO-spondin protein in the third and fourth brain ventricles and central canal of the spinal cord

Case presentation: abdominal compartment syndrome complicating posterior spinal fusion.

Abdominal compartment syndrome (ACS) is a life-threatening entity that requires rapid recognition and treatment. This case report represents the first case report of ACS associated with the correction of a marked scoliosis. Of the many possible causes for respiratory compromise and cardiovascular collapse associated with major spine surgery, ACS should be considered, particularly in instances of profound spinal curvature correction. We present the case of an 8-year-old child with Duchenne muscular dystrophy (DMD) undergoing correction of a severe scoliotic curvature. Near the end of an otherwi

Comparison of post-operative lordosis with the PEEK cage and the cervical plate.

OBJECTIVE: The maintenance of post-operative lordosis has been shown to be a key factor in decreasing adjacent level disc stress. Previous studies of the PEEK (polyether ketone) cage have used intervertebral bony fusion as the primary measure of surgical success; however, little is known about its effects on spinal curvature. Our objective was to compare the PEEK cage to the cervical plate with respect to the maintenance of cervical lordosis at one year. Secondary outcomes included fusion and complication rates. METHODS: We performed a retrospective study of patients who underwent ACDF (anter

Supine Bridge Exercise for Low Back Pain: A Fascial Approach for Movement Impairment Syndromes (Part II).

The supine bridge exercise (SBE) is widely used in rehabilitation for individuals with low back pain (LBP) due to its ability to enhance spinal stability and neuromuscular control. This second installment of a three-part review builds on previously discussed biomechanical principles to offer a functional rationale for applying the SBE to LBP subgroups identified through the Movement Impairment Syndromes framework. A novel interpretative model is proposed in which the SBE is reconceptualized not solely as a muscular strengthening tool, but as a method to increase passive fascial stiffness and p

Magnetic Controlled Growth Rods in the Treatment of Scoliosis: Safety, Efficacy and Patient Selection.

Early-onset scoliosis is defined as a spinal curvature greater than 10° in children prior to 10 years of age. Untreated EOS may lead to progressively severe spinal deformity, impaired pulmonary development, restrictive lung disease, and both increased morbidity and mortality. Limitations of established conservative treatments include inability to correct severe deformity, as well as challenges with compliance when casting and bracing is applied. In addition, surgical treatment in the form of traditional growing rods requires regular surgical lengthenings and is associated with complications in

The Cross-Sectional Area of Paraspinal Muscles Predicts the Efficacy of Deep Drain Stimulation for Camptocormia.

BACKGROUND: Camptocormia, a disturbance of posture, is a well-described clinical feature of PD and other parkinsonian syndromes. Previous reports have shown that DBS of the subthalamic nucleus (STN) or globus pallidus internus is effective in treating camptocormia. However, the efficacy of DBS for camptocormia varies. OBJECTIVE: To determine a clinical marker for selecting an appropriate therapy for camptocormia, a disabling manifestation of Parkinson's disease (PD) that has a variable response to systemic and local therapies. METHODS: We obtained pre-operative lumbar magnetic resonance imag
