Spine surgery exposes to neurological complications. There were 170 immediate complications of spine surgery at Pitiè-Salpétrière Hospital out of 2,855 reviewed (5.95%) during 9 years. The nature of the complication (radicular or medullar), severity and evolution were quite different but less than 2.76% where permanent. Among them, 1.43% were major neurological complications. The "high risk" etiologies were cervical stenosis and primitive malignant tumors. The major cause for these complications was due to surgical procedure in 60% of the cases.

We present two cases of iliac artery damage arising from surgery to correct a lumbar disc hernia due to ventral perforation. The first was a pseudoaneurysm of the left iliac artery with a retroperitoneal hematoma diagnosed by ultrasound and TAC on the third day after surgery. The second was a retroperitoneal hematoma diagnosed by intraoperative ultrasound after the patient was shifted to a new position. Emergency laparotomy was performed on both patients to repair the damage. This is an unusual but serious complication, and in most cases requires intuitive diagnosis and lifesaving surgery.

Most lumbar disk displacements occur through the lateral fibers of the posterior longitudinal ligament, whereas only a small percentage occur through the strong midline fibers. The conventional way of appreciating working capacity is no longer satisfying considering the improved diagnostic tools with increasing frequency of various deformities, instability and ankylosis and their combinations. As in the evaluation of peripheral joint function a concept is proposed, which is based on the structural changes in the motion segments. The concept is practicable in all parts of the vertebral column and allows a differentiated judgement within the usual percentages.

More than 20,000 lumbar spine fusions are performed annually in the United States. Results of surgery unfortunately are inconsistent and may reflect unsatisfactory patient selection. Indications for lumbar arthrodesis may arise in degenerative disk disease, deformity, distal extension of previous arthrodesis, trauma, spondylolisthesis, and spinal stenosis (in association with diskectomy or decompression). There are several techniques in the assessment of potential candidates for low lumbar arthrodesis.
