Ankylosing spondylitis (AS) has a striking disease marker, i.e., HLA-B27, indicating the major genetic predisposition; however, expression of disease is also strongly influenced by age- and sex-related factors. Sex steroids studies suggest greater androgenicity in AS than normal control persons. Therapeutic interventions that normalize such sex steroid status have shown clinical improvements in males and females. Three patients, all exhibiting symptoms before 15 years of age, were diagnosed as juvenile ankylosing spondylitis (JAS) by stigma of JAS. The families of these three patients--a total of fifteen first-degree relatives--had clinical, radiologic and laboratory examinations. We describe a 60-year-old man with ankylosing spondylitis (AS), hospitalized for leg ulcers. Laboratory investigations revealed the presence of the lupus anticoagulant. We found no such an association by a computer assisted search of the literature published from 1975 to 1989. The relationship between the AS, the lupus anticoagulant and leg ulcers is discussed.

To the best of our knowledge, only two patients with concurrent diffuse idiopathic skeletal hyperostosis (DISH) and ankylosing spondylitis (AS) have been reported so far. Here we present 3 patients in whom clinical and radiological findings indicative of DISH and AS coexisted. Two of these cases exhibited HLA B27. Although the presence of sacroiliitis would appear to exclude DISH, calcification and ossification of the anterior common vertebral ligament (ACVL) confirmed diagnosis of the latter disease.

Eighteen patients with spondylitis underwent 21 examinations by computed tomography (CT). Bone destruction was found in all, paravertebral inflammatory mass in 18, and epidural extension in 12 examinations. Conventional techniques revealed a paravertebral mass in only two cases. A review mainly based on Norwegian cases of ankylosing spondylitis and allied disorders is given. This concludes that it is a question of one or more syndromes which may frequently overlap. It is suggested that a clinical descriptive name like ankylosing spondylitis for the whole group may be misleading since the symptoms are so variable. The conditions are due to genetically determined patterns of reactions caused by different trigger mechanisms.

It is the intention to direct the attention from the old by orthopedists' influenced association of the stiff back to the polymorphism of the disease especially to the early stages which still frequently are recognized too late. The paper deals with the nosology, the various states of the clinical course with the numerous radiological criteria including therapeutical approaches.

The authors report a case of ankylosing spondylitis affecting a woman with low-back pains. Although initial X-rays did not show any lesions of the spine, one year later, radiographs prove the existence of multiple lesions mimicking spondylodiscitides and characterized by erosions diffusely involving end-plates of cervical, dorsal and lumbar vertebrae. Surgical biopsy at the lumbar site provides an histological evidence of non specific inflammatory lesions.

Fifty-five patients with ankylosing spondylitis and 16 control patients matched for sex and age were examined for evidence of occult inflammatory bowel disease. In all patients evaluation included history and physical examination, barium enema, sigmoidoscopy, and rectal biopsy. The results of this study suggest that there is no increased prevalence of occult inflammatory bowel disease in patients with ankylosing spondylitis.

Using a standard microdroplet lymphocyte cytotoxicity test for tissue typing, the distribution of the HLA antigens was determined in 37 female patients; 25 with osteitis condensans ilii (OCI) and 12 with ankylosing spondylitis (AS). We have experienced six patients of juvenile ankylosing spondylitis. Their chief complaints were polyarthralgia or coxalgia, however, any pain or stiffness in lumbar regions was absent. These clinical features were atypical for those of ankylosing spondylitis, and much more like those of a peripheral arthropathy such as juvenile rheumatoid arthritis.
