Native valve staphylococcal endocarditis: etiology, risk factors and outcome in 53 cases.

Fifty-three cases of staphylococcal endocarditis from a national endocarditis survey were analyzed for risk factors and outcome. Thirty of 53 patients had predisposing heart disease (39.6% rheumatic fever) but only 3 were on dialysis, only 2 had central venous catheter, only 2 intravenous drug abuse but 7 had prior cardiosurgery. Mortality was 39.6%. In analyzing risk factors for death, attributable mortality was significantly associated with skin infections (P < 0.05), embolization (P < 0.02), inappropriate therapy (P < 0.005) either because of too short therapy (P < 0.003) or wrong antibioti

Protective role of sialophorin (CD43)-expressing cells in experimental Staphylococcus aureus infection.

Sialophorin (CD43) is a major surface mucin on most hematopoietic cell lineages, including phagocytes. Defects of CD43 expression occur in Wiskott-Aldrich syndrome, a disease characterized by susceptibility to pyogenic infections. In a newly established rat model of septic Staphylococcus aureus arthritis, we have investigated the role of CD43-expressing cells in the progression of the disease. A single injection of a monoclonal antibody specific for CD43 induced a highly erosive course of arthritis and increased mortality in animals exposed to a suboptimal dose of bacteria. Our results demonst

[Plasmid-encoded antibiotic resistance in Staphylococcus hyicus].

A total of 33 Staphylococcus hyicus-cultures from piglets with exudative epidermatitis were analyzed for the presence of antibiotic resistance plasmids. Four small plasmids encoding resistances to chloramphenicol, macrolide-lincosamide-antibiotics, streptomycin or tetracyclines could be identified in plasmid-curing and plasmid-transformation experiments. For further characterization these plasmids were digested with restriction endonucleases. This led to the construction of a specific restriction map for each of the 4 plasmids. On the basis of their restriction maps, these 4 antibiotic resista

Characterization of Staphylococcus aureus infections in children with Down syndrome.

Staphylococcus aureus infections in the Down syndrome (DS) population have not been well characterized. This study determined clinical and molecular characteristics of S. aureus infections in children with DS followed at Texas Children's Hospital (TCH), from 2001 to 2011. Patients were retrospectively identified from an ongoing S. aureus surveillance study. Medical records were reviewed. Isolates were characterized by antimicrobial susceptibility, pulsed-field gel electrophoresis patterns, and detection of PVL genes (pvl), mupA (high-level mupirocin resistance gene), smr (chlorhexidine resista

Metabolic responses to Staphylococcus aureus airway infection.

Respiratory tract infections caused by antibiotic-resistant Staphylococcus aureus are often fatal and largely influenced by the pathogen's ability to overcome host immune defenses. Despite the importance of immune evasion, staphylococci, along with their exoproducts, are also known to modulate specific metabolic pathways within the respiratory epithelium and infection hot spot-infiltrating phagocytic cells, thereby impacting inflammatory and antimicrobial responses. Here, we briefly discuss how Staphylococcus aureus induces disease-related metabolic alterations in professional and nonprofessio

Role of old antibiotics in multidrug resistant bacterial infections.

Multidrug resistant bacteria infections are associated with an increase in attributable mortality and morbidity in ICU patients. Unfortunately, an emerging resistance to novel antibiotics used in the therapy of gram negative and gram positive bacteria infections is often reported in literature. Old antibiotics have been reintroduced in clinical practice. In this review we report the efficacy and safety use of older antimicrobial agents in critically ill patients. Polymyxins are used for nosocomial infection caused by Pseudomonas aeruginosa and Acinetobacter baumannii resistant strains. Patient

The clinical use of macrolides.

Macrolides are active against Streptococcus pneumoniae, Legionella spp. and Mycoplasma pneumoniae, the main causes of community-acquired pne … after dental treatment. The cure rate with macrolides of streptococcal skin infections and of minor staphylococcal infections is equal to that achieved with penicillins. In diarrhoea due to Campylobacter jejuni, the administration of macrolides shortens the duration of the faecal excretion of organisms and may give clinical improvement in severe disease. Macrolides are the …

Single-dose intranasal 10% povidone-iodine for perioperative nasal decolonization of Staphylococcus spp.: a randomized, open label, placebo-controlled trial.

BACKGROUND: Preoperative nasal colonization with Staphylococcus aureus (including MRSA) increases surgical site infection (SSI) risk. Mupirocin requires a 5-day regimen and faces resistance concerns. We evaluated whether a single preoperative intranasal application of 10% povidone-iodine (PI) suppresses nasal staphylococcal burden. METHODS: In a prospective, open label randomized, placebo-controlled trial at a single academic hospital, adults undergoing outpatient surgery were randomized to intranasal 10% PI or sterile saline. Nasal swabs were obtained at baseline and at 1, 2, and 3 hours aft

Immunization Against Staphylococcus aureus Infections.

Background: Infections caused by Staphylococcus aureus continue to plague surgical patients, whether as surgical site infections or other nosocomial infections that complicate surgical care. The only meaningful methods available to decrease the risk of developing such infections are topical skin antisepsis (pre-operative skin preparation) and peri-operative antibiotic prophylaxis, neither of which offer a panacea. Alternatives to the latter are sought so as to minimize antibiotic selection pressure as a factor in the increasing problem of antimicrobial drug resistance. This review considers th

Pulmonary manifestations of Staphylococcus aureus septicemia.

This study includes 140 episodes (138 cases) of Staphylococcus aureus septicemia, made up mostly of community-acquired, nonintravenous drug abuse (nonIVDA) cases. Unlike other series, injury wounds and skin or soft tissue infections were the most common sites of primary infection. In spite of a different patient population and lack of cases with tricuspid valvular endocarditis, the lungs were still the most common site of secondary infectious foci and most developed within two weeks of onset of the septicemia.

Recurrent chancriform pyoderma: report of a case with tongue lesions. Is Staphylococcus aureus implicated?

We report a case of recurrent chancriform pyoderma. Staphylococcus aureus was isolated from only one of seven lesions. Immunological responses to S. aureus, as measured by peripheral neutrophil function, were normal. We question the notion that chancriform pyoderma represents an abnormal immune response to a primary staphylococcal infection. To our knowledge, this is only the third reported patient with lesions affecting the tongue.

Staphylococcical scalded skin syndrome: a case series description of a rare and critical disease in a tertiary pediatric center.

BACKGROUND: Staphylococcal-scalded skin syndrome (SSSS) is a potentially life-threatening disorder characterized by superficial skin blistering caused by exfoliative toxins produced by Staphylococcus aureus. This study aimed to investigate SSSS in a cohort of children admitted at a tertiary pediatric hospital in Italy. METHODS: Patients discharged with the diagnosis of staphylococcal infection and of SSSS between January 2010 and March 2023 were retrospectively identified using ICD-9-CM codes (695.81 and 041.1, respectively). Medical records were reviewed to extract epidemiological, clinical,

Late onset periprosthetic joint infection of the knee caused by Streptococcus anginosus. Case presentation and literature review.

Periprosthetic joint infection (PJI) is one of the most dramatic complications of joint arthroplasty. Although streptococcal bone and joint infections are less common than staphylococcal cases, their role as causative agents of bone and joint remains significant accounting for at least 10% of PJIs. Streptococcus anginosus group (SAG) bacteria are usually found in the normal flora of the urogenital tract, intestinal tract and oropharynx and could cause pyogenic infections to affect brain, lungs and liver. SAG bacteria are uncommonly reported as a cause of osteomyelitis and the involvement of a

Ceftazidime in neonatal infections.

Ninety one neonates received 108 courses of intravenous ceftazidime (25 mg/kg, 12 hourly) over a study period of 15 months. Fourteen had clinically and bacteriologically proved infections. Only one of these had resistant organisms. Four (two with group B beta haemolytic streptococcal infections, one with Escherichia coli meningitis, and one with Staphylococcal aureus septicaemia) failed to respond despite adequate treatment. Bacteriological eradication or clinical improvement, or both, were obtained in the remaining nine. Routine biochemical and haematological values were monitored and there w

Netilmicin: chemical development and overview of clinical research.

Netilmicin is a new semisynthetic aminoglycoside which was developed by Schering Corporation, USA, for the treatment of serious gram-negative and staphylococcal infections. Nephrotoxicity and ototoxicity in animal studies have indicated that netilmicin is both quantitatively and qualitatively safer than other aminoglycosides. Also, netilmicin has a broader spectrum of activity than either gentamicin or tobramycin. 37 clinical studies were conducted by 29 investigators in 10 countries. 840 courses of treatment in 960 infection sites were analyzed for effectiveness. Of the 724 courses in which a…
