Modulation of Hepatic Cholesterol-Related Pathways by Ophiocordyceps sinensis and Coenzyme Q10 in a Sequential Gut-Liver In Vitro Model.

Dysregulated hepatic cholesterol handling and oxidative stress represent key targets for nutritional interventions. This study investigated a combination of Ophiocordyceps sinensis (formerly Cordyceps sinensis) extract and coenzyme Q10 (CoQ10) using a sequential gut-liver in vitro model. Differentiated Caco-2/HT29-MTX co-cultures were used to assess intestinal transport, transepithelial electrical resistance (TEER), and junctional organisation. The resulting basolateral intestinal-conditioned medium was applied to high-glucose-stressed HepG2 cells. The combined treatment-maintained epithelial

Mechanisms of action and therapeutic potential of PCSK9-regulating drugs.

CONTEXT: Proprotein convertase subtilisin/kexin type 9 (PCSK9) regulates plasma low-density lipoprotein cholesterol (LDL-C) metabolism and is a key target for cardiovascular therapies. It also plays roles in inflammation, cancer, and metabolic disorders, prompting interest in repurposing PCSK9-targeting drugs for non-lipid conditions. OBJECTIVE: This review comprehensively summarizes PCSK9-regulating medications, delves into their mechanisms of action, and explores their increasingly expanding therapeutic potential across multiple organ systems, such as the liver, immune system, small intesti

Effect of Non-Statin Lipid-Lowering Therapy on Hepatic Outcomes in Metabolic Dysfunction-Associated Steatotic Liver Disease and Steatohepatitis: A Systematic Review.

Metabolic dysfunction-associated steatotic liver disease (MASLD) and metabolic dysfunction-associated steatohepatitis (MASH) are significant health concerns affecting a large segment of the population and are known to be associated with adverse cardiovascular outcomes. As a result, various lipid-lowering medications are commonly employed in clinical practice to address the elevated cardiovascular risk in these patients. This systematic review summarizes the current evidence on emerging non-statin lipid-lowering therapies-ezetimibe, proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibito

PCSK9 in peripheral arterial disease: Mechanisms and therapeutic perspectives.

This narrative review comprehensively examines the multifaceted roles of proprotein convertase subtilisin/kexin type 9 (PCSK9) in peripheral arterial disease (PAD). PCSK9, a key enzymatic regulator of lipid metabolism, binds to low-density lipoprotein receptors (LDLRs) and promotes their degradation, thereby increasing plasma low-density lipoprotein cholesterol (LDL-C) levels. This mechanism establishes PCSK9 as a critical driver of atherosclerosis and PAD. Emerging evidence indicates that the role of PCSK9 in PAD extended beyond its traditional lipid-modulating functions. Specifically, PCSK9

Current Perspectives of Diabetic Dyslipidemia and Treatment Modalities.

INTRODUCTION: Diabetes mellitus is associated with an increased risk of atherosclerosis related to dyslipidemia. Although the terms hyperlipidemia and Diabetes Mellitus (DM) or diabetic dyslipidemia are interrelated to each other, these two conditions have some differences. AIM: This study aimed to highlight possible mechanisms of hyperlipidemia and/or dyslipidemia in diabetic patients, which can be treated with available and newer hypolipidemic drugs. We also re-checked current specific guidelines and their recommendations on the management of patients with diabetic dyslipidemia. METHODS: C

[Mechanism of the Molecular Pathophysiology for Familial Hypercholesterolemia].

Familial hypercholesterolemia (FH) is characterized by high serum low-density lipoprotein cholesterol (LDL-C) levels from birth, tendon/skin xanthomas, and premature coronary artery disease. The prevalence of FH is 1 per 300 individuals in the general population. FH is caused by a pathogenic (rare) variant in the LDL receptor (LDLR), apolipoprotein B (APOB), and proprotein convertase subtilisin/kexin type 9 (PCSK9) genes. In Japan, there has been only one reported case of a family with FH caused by the known APOB p.(Arg3527Gln) variant. Those without pathogenic variants in the LDLR or PCSK9 ge

Population Pharmacokinetics and Pharmacodynamics Modeling for the Use of Recaticimab in Healthy Volunteers and Patients with Hypercholesterolemia.

INTRODUCTION: Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a protein secreted by the liver that binds to low-density lipoprotein (LDL) receptors, which leads to a decreased ability of the liver to clear LDL-C from circulation. By inhibiting PCSK9, it is possible to provide early intervention to achieve clinical benefits. METHODS: The database was built using data derived from seven clinical studies, population pharmacokinetic/pharmacodynamic (PopPK/PD) and PK/PD analyses were conducted via nonlinear mixed effects analysis with NONMEM software. The parameter estimation of the model

Soluble endoglin reflects endothelial dysfunction in myocardial infarction patients: a retrospective observational study.

Acute manifestations of ischemic heart disease are among the most serious and fatal consequences of atherosclerotic processes. In this study, we hypothesized that a soluble proprotein convertase subtilisin/kexin type 9 (PCSK9), soluble bone morphogenetic protein 4 (BMP-4), soluble E-selectin (sE-selectin), soluble endoglin (sENG) and soluble endocan (Endocan) would differ from healthy controls in myocardial infarction (MI) patients admitted to the hospital without any previous history of cardiovascular disease and with no cardioprotective drugs taken before admission. The study was conducted u

Rationale and Design of the EPISODE Trial: A Randomized Controlled Trial on the Effect of PCSK9 Inhibitors in Calcific Aortic Valve Stenosis.

BACKGROUND: Calcific aortic valve stenosis (CAVS) can lead to cardiac adverse outcomes; however, currently, no effective pharmacological interventions are available to prevent or delay disease progression. Emerging evidence has identified significant associations between CAVS and key biomarkers, including Lp(a) (lipoprotein [a]), low-density lipoprotein cholesterol, and PCSK9 (proprotein convertase subtilisin/kexin type 9). However, robust evidence from randomized controlled trials is still lacking to substantiate these associations. METHODS: The EPISODE (Effect of PCSK9 Inhibitors on Calcifi

Ovotransferrin Fibrils Formed by Peptides Self-Assembly Improve the Stability and Antioxidation of Quercetin and Their Application in Quercetin Fortified Soybean Milk.

Quercetin (Que) is a natural flavonoid compound with multiple biological functions. However, it is easily dissociated by external conditions and difficult to apply in the food industry, so it needs to be encapsulated with food-derived nanocarriers to improve its stability. Here, ovotransferrin (OVT) is hydrolyzed into peptides by subtilisin A, and then the peptides self-assemble into hollow nanofibrils under Ca2+ ion induction at 50°C and pH 7.5 for 1 h. The increase in Thioflavin T (ThT) fluorescence intensity confirms the continuous growth of OVT fibrils by β-sheet stacking. The mature OVT f

Drug intervention as an emerging concept for secondary prevention in patients with coronary disease.

Non-culprit lesion-related coronary events are a significant concern in patients with coronary artery disease (CAD) undergoing coronary intervention. Since several studies using intra-coronary imaging modalities have reported a high prevalence of vulnerable plaques in non-culprit lesions at the initial coronary event, the immediate stabilization of these plaques by intensive pharmacological regimens may contribute to the reduction in the adverse events. Although current treatment guidelines recommend the titration of statin and other drugs to attain the treatment goal of low-density lipoprotei

Vitamin D3 Regulates Energy Homeostasis under Short-Term Fasting Condition in Zebrafish (Danio Rerio).

Vitamin D3 (VD3) is a steroid hormone that plays pivotal roles in pathophysiology, and 1,25(OH)2D3 is the most active form of VD3. In the current study, the crucial role of VD3 in maintaining energy homeostasis under short-term fasting conditions was investigated. Our results confirmed that glucose-depriving pathways were inhibited while glucose-producing pathways were strengthened in zebrafish after fasting for 24 or 48 h. Moreover, VD3 anabolism in zebrafish was significantly suppressed in a time-dependent manner under short-fasting conditions. After fasting for 24 or 48 h, zebrafish fed wit

Role of CETP, PCSK-9, and CYP7-alpha in cholesterol metabolism: Potential targets for natural products in managing hypercholesterolemia.

Cardiovascular diseases (CVDs) are a leading cause of mortality worldwide, primarily affecting the heart and blood vessels, with atherosclerosis being a major contributing factor to their onset. Epidemiological and clinical studies have linked high levels of low-density lipoprotein (LDL) emanating from distorted cholesterol homeostasis as its major predisposing factor. Cholesterol homeostasis, which involves maintaining the balance in body cholesterol level, is mediated by several proteins or receptors, transcription factors, and even genes, regulating cholesterol influx (through dietary intak

[A Case of Successful Treatment of Severe Hyperlipidemia After Heart Transplantation With Inclisiran].

The prognosis after heart transplantation continues to improve. Therefore, the prevention of chronic post-transplant sequelae, such as chronic kidney disease, allograft vasculopathy, and malignancies is becoming increasingly important. Everolimus, an inhibitor of the mammalian target of rapamycin (mTOR), is increasingly used for immunosuppression after heart transplantation. However, everolimus may cause a characteristic complex of adverse effects, including dyslipidemia. Currently there are no guidelines for the long-term screening and treatment of dyslipidemia in heart transplant recipients
