Performance of interleukin-27 as a sepsis diagnostic biomarker in critically ill adults.

PURPOSE: We recently identified interleukin-27 (IL-27) as a sepsis diagnostic biomarker in children. Here we assess IL-27 as a sepsis diagnostic biomarker in critically ill adults with systemic inflammatory response syndrome and sepsis. METHODS: IL-27 and procalcitonin (PCT) were measured from plasma samples in three groups: no sepsis (n = 78), pulmonary source of sepsis (n = 66), and non-pulmonary source of sepsis (n = 43). Receiver operating characteristic curves and classification and regression tree methodology were used to evaluate biomarker performance. RESULTS: IL-27 did not discrimin…

The stress response and the hypothalamic-pituitary-adrenal axis: from molecule to melancholia.

Organisms survive by maintaining equilibrium with their environment. The stress system is critical to this homeostasis. Glucocorticoids modulate the stress response at a molecular level by altering gene expression, transcription, and translation, among other pathways. The effect is the inhibition of the functions of inflammatory cells, predominantly mediated through inhibition of cytokines, such as IL-1, IL-6, and TNF-alpha. The central effectors of the stress response are the corticotrophin-releasing hormone (CRH) and locus coeruleus-norepinephrine (LC-NE)/sympathetic systems. The CRH system

[Risks of surgery and anesthesia and optimization of anesthesiological maintenance of abdominal delivery].

The paper deals with abdominal labor pain relief in pregnant women at high risk for impaired development of the general adaptation syndrome: significant sympathicotonia, placental ischemia-reperfusion syndrome, and systemic inflammatory response syndrome. All these syndromes are attended by either impaired oxygen transport or a high risk for the occurrence of such at surgery. The detection and correction of these impairments in development of the general adaptation syndrome in pregnancy, which are provided by the author's modified rating scale for risks of surgery and anesthesia, contribute to…

Prevention of bile peritonitis by laparoscopic evacuation and lavage after nonoperative treatment of liver injuries.

One of the major lessons learned in the World War II experience with liver injuries was that bile peritonitis was a major factor in morbidity and mortality; the nearly uniform drainage of liver injuries in the subsequent operative era prevented this problem. In the era of nonoperative management, patients who do not require operative treatment for hemodynamic instability may develop large bile and/or blood collections that are often ignored or inadequately drained by percutaneous methods. These inadequately treated bile collections may cause systemic inflammatory response syndrome and/or respi…

Novel therapeutic targets for acute pancreatitis and associated multiple organ dysfunction syndrome.

Acute pancreatitis is a common clinical condition. The exact mechanisms by which diverse etiological factors induce an attack are unclear but once the disease process is initiated, common inflammatory and repair pathways are invoked. Acinar cell injury early in acute pancreatitis leads to a local inflammatory reaction; if marked, this leads to a systemic inflammatory response syndrome (SIRS). An excessive SIRS leads to distant organ damage and multiple organ dysfunction syndrome (MODS). MODS associated with acute pancreatitis is the primary cause of morbidity and mortality in this condition. T…

Association of dietary inflammatory index with severity and mortality among adults with cardiovascular-kidney-metabolic syndrome.

BACKGROUND AND AIMS: Cardiovascular-kidney-metabolic (CKM) syndrome is driven by chronic inflammation. The impact of the energy-adjusted dietary inflammatory index (E-DII) on CKM severity and mortality remains unclear. We evaluated E-DII's association with advanced CKM and mortality, quantifying the mediating role of systemic inflammatory markers. METHODS AND RESULTS: Analyzing data from the National Health and Nutrition Examination Survey (NHANES,1999-2018),we employed weighted logistic regression and Cox regression to evaluate the associations of the E-DII with advanced CKM and mortality. Connective tissue disease with macrophage activation syndrome: A case report.

RATIONALE: Macrophage activation syndrome (MAS), or secondary hemophagocytic lymphocytosis (sHLH), is a rare systemic inflammatory response syndrome that is fatal. Adult patients lack clear criteria for diagnosis and treatment, primarily derived from guidelines and protocols for treating family hemophagocytic lymphocytosis and systemic juvenile idiopathic arthritis (sJIA)-related MAS in children or from retrospective case reports. As a subtype of sHLH, MAS has a clinical presentation like sHLH, but treatment varies. Herein, we report the case of a 40-year-old female with MAS caused by a connec…

[Current concepts on the physiopathology of adult-onset Still's disease].

PURPOSE: Adult-onset Still's disease (AOSD) is a rare systemic inflammatory disorder of unknown origin. It is characterized by hectic fever, evanescent rash, polyarthralgias or polyarthritis, sore throat, hepatosplenomegaly, lymphadenopathy, polynuclear leukocytosis, liver cytolysis, and high serum level of ferritin with low glycosylated fraction. CURRENT KNOWLEDGE AND KEY POINTS: An increased serum level of ferritin, IL-8, IL-6, IL-18 and TNF-alpha indicates that macrophages are highly activated in AOSD. Interleukin 18 (IL-18) seems to be a key cytokine in the pathogenesis of AOSD. Serum IL-

An updated review of anti-Ro52 (TRIM21) antibodies impact in connective tissue diseases clinical management.

Anti-Ro52 (or anti-TRIM21) antibodies are part of the family of anti-Ro/SSA antibodies, historically markers of Sjögren syndrome and systemic lupus erythematosus. Anti-Ro52 antibodies represent one the most frequently encountered autoantibodies in patients with connective tissue disease (primary Sjögren syndrome, systemic lupus erythematosus, systemic sclerosis and idiopathic inflammatory myopathies). Because of their lack of specificity and detection in patients with non-autoimmune disorders, the usefulness of anti-Ro52 testing in connective tissue diseases is still matter of debate among cli

Multiorgan Dysfunction in Severe Acute Pancreatitis: A Case of Septic Shock, Renal Cortical Necrosis, and Takotsubo Cardiomyopathy.

INTRODUCTION: Severe acute pancreatitis (AP) may evolve rapidly into systemic inflammatory response syndrome and multiorgan dysfunction syndrome, posing significant management challenges. CASE PRESENTATION: We present a rare and complex case of severe AP complicated by septic shock, bilateral renal cortical necrosis (RCN), and Takotsubo cardiomyopathy (TTC). Septic shock contributed to profound hemodynamic instability, while RCN - a rare and often fatal form of acute kidney injury - added to the severity of renal dysfunction. Concurrent TTC, likely precipitated by catecholamine surges during …

Lipid Mediators in Critically Ill Patients: A Step Towards Precision Medicine.

A dysregulated response to systemic inflammation is a common pathophysiological feature of most conditions encountered in the intensive care unit (ICU). Recent evidence indicates that a dysregulated inflammatory response is involved in the pathogenesis of various ICU-related disorders associated with high mortality, including sepsis, acute respiratory distress syndrome, cerebral and myocardial ischemia, and acute kidney injury. Moreover, persistent or non-resolving inflammation may lead to the syndrome of persistent critical illness, characterized by acquired immunosuppression, catabolism and

Peri-onset non-steroidal anti-inflammatory drugs use and organ failure in acute pancreatitis: A multicenter retrospective analysis.

BACKGROUND: Organ failure (OF) of acute pancreatitis (AP) significantly contributes to AP-related mortality. Non-steroidal anti-inflammatory drugs (NSAIDs) have been associated with reduced complications of AP. AIMS: We aimed to investigate whether NSAIDs ameliorates SIRS and OF in patients with AP. METHODS: Eligible patients with AP were retrospectively identified in 4 hospitals between January 2015 and December 2018. Associations between peri-onset NSAIDs use (day -3 to day 3) and OF, persistent OF (POF), and SIRS within the first week were analyzed. Propensity score-matched (PSM) analysis

Pathogenesis of systemic inflammatory diseases in childhood: "Lessons from clinical trials of anti-cytokine monoclonal antibodies for Kawasaki disease, systemic onset juvenile idiopathic arthritis, and cryopyrin-associated periodic fever syndrome".

Inflammation has often been considered to be a nonspecific response and to play a bridging role in the activation of adaptive immunity. However, it is now accepted that inflammation is the product of an independent innate immune system closely linked to the adaptive immune system. The key mediators of inflammation are inflammatory cytokines, as determined by multiple lines of evidence both in vitro and in vivo. Due to the crucial role of inflammatory cytokines in the pathogenesis of autoimmune disorders, anti-cytokine treatment has been developed as a therapy for rheumatoid arthritis, juvenile

The effect of gender on clinical characteristics and diagnostic process of obstructive sleep apnea syndrome.

BACKGROUND: Obstructive sleep apnea syndrome (OSAS) has long been considered a predominantly male disorder; however, increasing evidence suggests substantial sex-related differences in clinical presentation and disease progression. METHODS: This retrospective study included adult patients diagnosed with OSAS by polysomnography between January 2020 and January 2026. Demographic characteristics, polysomnographic findings, laboratory parameters, and echocardiographic measurements were analyzed. Systemic inflammation was assessed using the monocyte-to-HDL ratio, monocyte-to-LDL ratio, systemic im

Amniotic Fluid Embolism.

Amniotic fluid embolism remains one of the most devastating conditions in obstetric practice with an incidence of approximately 1 in 40,000 deliveries and a reported mortality rate ranging from 20% to 60%. The pathophysiology involves an abnormal maternal response to fetal tissue exposure associated with breaches of the maternal-fetal physiologic barrier during parturition. This response and its subsequent injury involve activation of proinflammatory mediators similar to that seen with the classic systemic inflammatory response syndrome. Maternal treatment is primarily supportive, whereas prom…
