Static Suspension of the Paralyzed Face Utilizing the Midfacial Corridor: Anatomic Evaluation and Surgical Technique.

BACKGROUND: Fascia lata and tendon grafts are frequently utilized to support the paralyzed midface and to extend muscular reach in McLaughin style, orthodromic temporalis transfers. The grafts are frequently placed in a deep subcutaneous positioning that can lead to the development of a, bowstring deformity in the cheek. This paper describes insertion of tendon grafts into the midfacial corridor collectively formed by the buccal, submasseteric and superficial temporal spaces. METHODS: Over a seven-year period, all patients that underwent insertion of facia lata and tendon grafts in the midfac

Anti-Claw Procedure with Modified Dynamic Flexor Digitorum Superficialis Transfer for Ulnar Nerve Palsy: Our Experience.

INTRODUCTION: The ulnar nerve palsy is a distressing injury, resulting in clawing of hand. The cause of clawing is due to paralysis of the interosseous muscles in the presence of functioning long extensor and long flexors of fingers. Various methods have been proposed to correct this deformity which include both static and dynamic procedures. In this study, we share our experience with flexor digitorum superficialis tendon transfer using Zancolli's modification for anti-claw correction. MATERIALS AND METHODS: It was a retrospective case series study. A record of 53 patients was included in th…

Minimally Invasive and Endoscopic Treatment of Haglund Syndrome.

Haglund syndrome is a triad of posterosuperior calcaneal prominence (Haglund deformity), retrocalcaneal bursitis, and insertional Achilles tendinopathy. The sources of pain include the posterior calcaneal wall cartilage, retrocalcaneal and subcutaneous adventitial bursa, and the Achilles tendon. Resection of the posterosuperior calcaneal tubercle, bursectomy, excision of the Achilles tendon pathology, reattachment of the Achilles tendon, gastrocnemius aponeurotic recession, and flexor hallucis longus transfer have been proposed as surgical treatment options. All of them can be performed endosc

Unilateral Adolescent Pes Planus After a Bimalleolar Ankle Fracture: A Case Report.

We present the case of a 14-year-old female who presented with unilateral pes planus 30 months after a bimalleolar ankle fracture dislocation. At surgery, the tibialis posterior tendon was encased in fracture callus within the syndesmosis and required reconstruction using flexor digitorum longus transfer. Dislocation of the tibialis posterior tendon and entrapment within the tibiofibular syndesmosis has been previously reported. To our knowledge, this is the first case report tibialis posterior tendon syndesmotic entrapment presenting with unilateral pes planus.

Rupture of the flexor pollicis longus tendon after 30 years due to migration of a retained foreign body.

Delayed rupture of a flexor tendon in the hand due to the presence of a retained foreign body is rare. We present the case of a late flexor pollicis longus rupture 30 years after traumatic implantation of a glass fragment. The foreign body had migrated distally a distance of 4.5 cm from the site of the original injury and eroded into the flexor tendon sheath. Thumb function was restored with a flexor digitorum superficialis tendon transfer from the ring finger with excellent results.

All arthroscopic augmented Vargas procedure: An option after failed acromioclavicular joint dislocation reconstruction. A technical note.

UNLABELLED: Few salvage procedures have been described after a failed Weaver-Dunn procedure. We hypothesized that it was possible to perform an all-arthroscopic revision with a reflected vascularized conjoint tendon into the distal resected clavicle (Vargas procedure) augmented by an artificial coraco-clavicular ligament. Two patients were enrolled. A minimum of 3-cm proximal conjoint tendon was dissected under arthroscopic control. The half-longitudinal conjoint tendon split, leaving its proximal end attached to the coracoid process, was made and incised transversely near the muscles fibers.

Bridge Plate Failure with Extensor Tendon Injury: A Case Report and Literature Review.

BACKGROUND: Dorsal spanning plate fixation is an increasingly popular method of fixation for certain distal radius fractures. Published outcomes are encouraging, but complications are still reported. METHODS: We present a case report of a 26-year-old woman with spanning plate breakage and extensor pollicis longus (EPL) metallosis, fraying, and near-complete rupture. The patient's unsuccessful follow-up led to this complication. Metallosis and damage to the extensor indices (EI) and distal extensor carpi radialis longus (ECRL) were intraoperative obstacles for tendon transfer to restore EPL fu…

Late reconstruction of sagittal band laceration.

Injuries to the sagittal band with subsequent instability of the extensor digitorum communis tendon are uncommon. In an unusual presentation, a laceration to the radial sagittal band of the little finger was manifest as an abduction deformity of the digit. This injury was treated by repair of the sagittal band and extensor digiti minimi tendon transfer.

[Recurrent defects of the rotary cuff : Causes and therapeutic strategies].

Recurrent defects of the rotator cuff still represent the most common complication after rotator cuff repair. The etiology is multifactorial … f repairs appears to be a promising procedure. Irreparable tears can be managed using extraanatomic tendon transfer (latissimus dorsi or pectoralis major transfer) or shoulder replacement procedures (reverse prosthesis) depending on the functional symptoms of the patient. The results after re-reconstruction or open revision using tendon transfers are inferior com …

A Blauth IIIB hypoplastic thumb reconstructed with a vascularised metatarso-phalangeal joint transfer: a case report with 28 years of follow up.

We reconstructed the Blauth-IIIB hypoplastic right thumb of a 16-year-old girl with a vascularised metatarso-phalangeal (MTP) joint from her second toe combined with a dorsalis-pedis flap. Abduction was provided with an abductor policis long tendon advancement, and adduction, with an extensor indicis proprius tendon transfer. Opponoplasty was performed one year later using the flexor sublimis (IV) tendon. The transferred joint remained functional and non-osteoarthritic 28 years later. Radial and palmar abduction of the reconstructed thumb was 45 degrees and 75 degrees respectively. The index, …

A systematic approach to the surgical treatment of chronic shoulder pain.

A systematic surgical approach for dealing with chronic shoulder pain includes a carefully designed arthrectomy of the acromioclavicular joint, removal of the coracoacromial ligament, cuff repair, biceps tendon transfer, and removal of calcific deposits as indicated. Of 38 patients who have been followed for at least 24 months, 29 were rated as excellent on the basis of achieving a full range of pain-free movement and normal strength. Degenerative arthritis of the acromioclavicular joint was found in all cases, whereas bicipital tendinitis was noted in less than half. Inspection or transfer of…

Serratus fascia "sandwich" free-tissue transfer for complex dorsal hand and wrist avulsion injuries.

The serratus anterior fascia was used as a free-tissue transfer in four patients for the reconstruction of dorsal hand defects. All patients had multiple open metacarpal fractures with extensor tendon injuries. The fascia was used to "sandwich" the extensor tendons in a bed of areolar gliding tissue to avoid adhesions. The mean follow-up was 2 years. There were no complications and all flaps survived completely. All flaps were grafted with meshed split-thickness skin at the time of transfer with a 100 percent take in all cases. A good functional result was noted in all patients. This free-tiss

Outcome of flexor digitorum superficialis opponensplasty for Type II and IIIA thumb hypoplasia.

UNLABELLED: This study reports on outcomes of the flexor digitorum superficialis tendon transfer from the ring finger in Type II and IIIA hypoplastic thumbs. We included 22 patients with 27 involved hands: 16 Type II thumbs and five Type IIIA treated by transfer and six non-operated Type II thumbs. The outcomes were assessed by range of motion, strength, sensibility, joint stability and patient/parent satisfaction. Compared with normative data, the range of motion was diminished in all patient groups. Opposition strength in operated Type II thumbs was significantly better than in non-operated …

Factors Impacting the Success of Free Functioning Gracilis Muscle Transfer for Elbow Flexion in Brachial Plexus Reconstruction.

BACKGROUND: Free functioning muscle transfer is a reconstructive option to restore elbow flexion in brachial plexus injuries. The authors determined the impact of body mass index, age, and location of distal tendon attachment on elbow flexion strength after free functioning muscle transfer in traumatic brachial plexus injury patients. METHODS: A retrospective review of patients who underwent free functioning muscle transfer for elbow flexion as part of their brachial plexus injury reconstruction with a minimum 2-year follow-up were evaluated. Outcomes assessed included elbow flexion strength

Modified extensor carpi radialis brevis adductorplasty for ulnar nerve palsy.

Residual paralysis or irreparable injury to the ulnar nerve results in considerable impairment of hand function and posture that directly affect daily living. Usually, claw hand deformity and loss of key pinch become problematic, but tendon transfer can be used to restore the lost functions. Adductorplasty using a wrist extensor tendon can restore thumb adduction strength without a significant loss of wrist extension strength, but the short length of the wrist extensor necessitates a free tendon graft to reach the thumb. Here, the authors present a modified extensor carpi radialis brevis adduc…

Classical Surgical Alternatives for the Treatment of Lesser Toe Deformities.

Lesser toe deformities are the second most common deformities around the foot and ankle. Early reports of procedures for lesser toe correction date back to the 1880s. Lesser toe deformities have a high variability of appearance. Thorough assessment of the joints involved in the deformity is necessary. Numerous techniques of soft tissue and osseous interventions are available and need to be included for individual correction. Osseous corrections are usually accompanied by soft tissue procedures. Kirschner-wire fixation is still an effective method of fixation, but internal fixation is probably

The Camitz transfer and its modifications: a review.

UNLABELLED: The Camitz procedure involves transfer of the insertion of the palmaris longus tendon with a strip of the palmar aponeurosis to the insertion of abductor pollicis brevis in order to improve thumb opposition, and is usually done when severe carpal tunnel syndrome results in complete wasting of the thenar muscles. We carried out a systematic review of the published reports of this procedure. Analysis of available outcome data showed improvement in overall hand function in 86-100% of patients undergoing the original Camitz procedure. Several modifications of the original Camitz transf

The crossover toe and valgus toe deformity.

Second toe problems are among the most common of all forefoot complaints. Its proximity to the hallux combined with limited motion at the second tarsometatarsal joint likely contributes to the second MTP joint being the most common to experience both pain and deformity. Many causes have been linked to this problem, which has lead to many surgical techniques to correct this deformity. Although many techniques have been described, a systematic approach relying first on soft tissue releases and plication followed by osteotomies as necessary has lead to satisfactory outcomes in the treatment of th
