Direct Repair of Chronic Achilles Tendon Ruptures Using Scar Tissue Located Between the Tendon Stumps.

BACKGROUND: Several surgical procedures for chronically ruptured Achilles tendons have been reported. Resection of the interposed scar tissue located between the tendon stumps and reconstruction using normal autologous tissue have been well described. We developed a direct repair procedure that uses scar tissue, which obviates the need to use normal autologous tissue. METHODS: Thirty consecutive patients with Achilles tendon ruptures with a delay in diagnosis of >4 weeks underwent removal of a section of scar and healing tissue with direct primary suture of the ends of the tendon without the …

Conservative management of partial extensor tendon lacerations greater than half the width of the tendon in manual workers.

Conservative management (without suturing or splints) of partial extensor tendon lacerations greater than half the width of the tendon has not been previously investigated. In this prospective study, a total of 45 injured tendons (with lacerations involving 55%-90% of the width of the tendon) in 39 patients were treated conservatively. Injury zones I, III, and V of the fingers; and zones I and III of the thumb were excluded. Immediate non-resistive active mobilization was initiated and continued for 4 weeks, followed by resistive exercises. Patients were allowed to go back to work after 6 week…

The interfascicular matrix enables fascicle sliding and recovery in tendon, and behaves more elastically in energy storing tendons.

While the predominant function of all tendons is to transfer force from muscle to bone and position the limbs, some tendons additionally function as energy stores, reducing the cost of locomotion. Energy storing tendons experience extremely high strains and need to be able to recoil efficiently for maximum energy storage and return. In the equine forelimb, the energy storing superficial digital flexor tendon (SDFT) has much higher failure strains than the positional common digital extensor tendon (CDET). However, we have previously shown that this is not due to differences in the properties of…

Extensor tendon injury due to repetitive wrist dorsiflexion: morphological study of extensor retinaculum and extensor tendon.

Most etiological studies of extensor tendon injury were based on the normal anatomy of extensor tendon and extensor retinaculum of the wrist. Further understanding of the morphological changes of the extensor tendon and extensor retinaculum during wrist dorsiflexion might contribute to improved and more accurate understanding of the etiology. The morphology of the extensor tendon of the mid-finger and the fourth compartment of the wrist extensor retinaculum was studied by sonography, and the anatomy was studied in 15 extremities from 11 young male cadavers. Compared with anatomical images, ult

Single-stage flexor tendon transfer for the treatment of severe concomitant peroneus longus and brevis tendon tears.

BACKGROUND: Although peroneal tendon injuries are a common cause of lateral ankle pain, there is a paucity of literature specifically addressing the treatment of severe concomitant peroneus longus and brevis tears. The purpose of this study was to evaluate patient outcomes following a single-stage flexor tendon transfer for the treatment of severe concomitant tears of both peroneal tendons. METHODS: Eight patients were treated with a single-stage flexor tendon transfer for severe concomitant peroneus longus and brevis tears over a 15-year period. Mean age at the time of surgery was 54 years (…

[Subpectoral tenodesis of the long head of the biceps tendon for pathologies of the long head of the biceps tendon and the biceps pulley].

OBJECTIVE: The presented surgical technique aims at fixation of the long head of the biceps tendon at the proximal humerus, distal to the bicipital groove, after arthroscopic tenotomy. This preserves the length and shape of the biceps muscle belly and avoids groove tenderness. INDICATIONS: All forms of degenerative changes of the long head of the biceps tendon, biceps instability due to pulley lesions, irreparable SLAP tears. The technique offers a cosmetically favorable outcome compared to a tenotomy. CONTRAINDICATIONS: Old complete tears of the long head of the biceps, significant loss of

Longitudinal tendon tear concurrent with bilateral medial luxation of the superficial digital flexor muscle tendon in a dog.

Luxation of the superficial digital flexor muscle tendon (SDFMT) is a rarely reported condition in dogs, and tendon tear concurrent with SDFMT luxation had never been reported. Abnormal conformation of both calcanei and strenuous activity were suspected as contributing factors in this case. Bilateral medial SDFMT luxation was surgically treated with bursa medial incision release, redundant lateral bursa excision and apposition to the edge of the SDFMT with nonabsorbable sutures. The unilateral longitudinal tendon tear of the SDFMT associated with medial SDFMT luxation was successfully treated

The "Cascade" membrane: a new PRP device for tendon ruptures. Description and case report on rotator cuff tendon.

Rotator cuff tears are common soft-tissue injuries that often require surgical treatment. Initial efforts to better tendon healing centered on improving the strength of repair. More recent studies have focused on abiologic enhancement of the healing process. Platelet rich plasma (PRP) is a fraction of plasma that has been isolated and used to enhance regeneration in bone and soft tissues. The healing potential of PRP has been attributed to the release of multiple growth factors from the highly concentrated platelets. The "Cascade" membrane is a thin layer of autologous fibrine that is very ric

Localization of the semitendinosus-gracilis tendon bifurcation point relative to the tibial tuberosity: an aid to hamstring tendon harvest.

PURPOSE: Autologous hamstring grafts are commonly used for knee reconstruction. The proximity of the saphenous nerve and its branches, the superficial medial collateral ligament, and the inherent variability of fascial attachments necessitates effective surgical incision placement to minimize tissue morbidity during graft harvest. This study describes a method of locating the semitendinosus and gracilis tendon bifurcation point relative to the tibial tuberosity. It uses a novel diagonal surgical incision placed between a proximal point placed 3 cm medial to the tibial tuberosity and at 3 cm di

[Surgical shortening of the Achilles tendon for correction of elongation following healed conservatively treated Achilles tendon rupture].

INTRODUCTION: Is operative shortening of the achilles tendon an adequate therapy in cases of elongation following conservative treatment of achilles tendon rupture? METHODS: From 11/89 to 12/97, 12 patients underwent achilles tendon shortening (9 male, 3 female, average age 43 years). All patients had an elongated tendon following conservative treatment of achilles tendon rupture. We were able to examine 8 patients (67%) after the operation. The average follow-up period was 35 (7-103) months with an average span between the primary trauma and surgery of 22 (8-45) months. RESULTS: Prior to su

[Transfer of the pronator teres tendon to the extensor carp radialis tendon in wrist flexion deformity in cerebral palsy].

The flexion deformity of the wrist in cerebral palsy is quite common, carrying disability to the effective use of the hands. From 1980 to 1990, 36 transferences of the pronator teres to the extensor carpi radialis were reviewed. The authors describe the operative technique by a unique incision, that may be used in cerebral palsy, and conclude that it is a good method for correction of deformity.

Deformity of the foot following anterior transfer of the posterior tibial tendon and lengthening of the Achilles tendon for spastic equinovarus.

Twenty-four children with spastic equinovarus deformity due to cerebral palsy were treated by anterior transfer of the posterior tibial tendon and Achilles tendon lengthening. In five patients, the operation was performed on both sides, making a total of 29 feet available for evaluation after an average follow-up of five years. Only 38 per cent of the results were graded "good" or "satisfactory." Sixty-two per cent were rated as "poor" because of valgus, calcaneus or equinus deformity severe enough to require re-operation. The post-operative deformity was generally evident one or more years af

Time to treat the tendon rupture induced by surgery: early hypertrophy of the patellar tendon graft site predicts strong quadriceps after ACLR with bone-patellar tendon-bone autograft.

PURPOSE: Quadriceps dysfunction is ubiquitous after anterior cruciate ligament reconstruction, especially when using bone-patellar tendon-bone (BPTB) autografts. The role of patellar tendon hypertrophy after graft harvest on knee extensor strength is unknown. The purpose of this study was to determine the predictive ability of patellar tendon (PT) and quadriceps muscle (Quad) cross-sectional area (CSA) on knee extensor strength 1-2 months after ACLR using BPTB autografts. METHODS: This is a cross-sectional analysis of a cohort 1-2 months after ACLR using BPTB autograft. Peak knee extensor tor

Methods of analyzing the long head of the biceps tendon in the management of distal supraspinatus tendon ruptures. Part 2: The role and validation of an arthroscopic exploration protocol of the long head of the biceps tendon.

INTRODUCTION: Inter-observer arthroscopic assessments of the Long head of the Biceps tendon (LHB) injuries, in the absence of predefined instructions, are poorly reproducible. There are several types of LHB injuries, of varying severity, which can make its intraoperative analysis subjective. HYPOTHESIS: The application of a precise arthroscopic exploration protocol, particularly dynamic, associated with an equally precise analysis of the possible tendon lesions, intrinsic or extrinsic, makes it possible to obtain a reproducible analysis of the lesions of the LHB and aid decision-making around

Staged patellar tendon reconstruction using doubled bone-patellar tendon-bone allograft for infected patellar tendon rupture: a rare case report of three years follow-up.

BACKGROUND: Patellar tendon rupture is a relatively rare injury that usually requires surgical treatment. The optimal therapeutic strategy is still controversial, especially when either concomitant patellar tendon infection or soft tissue infection surrounds the patellar tendon. Until recently, most reported reconstruction methods are extensive and difficult to apply because of the poor condition of the soft tissue surrounding the patellar tendon. CASE PRESENTATION: A 19-year-old male patient presented to our clinic three weeks following a motorcycle accident. There was a 5 x 4 cm sized skin

Allograft Tendon Pretensioning Technique Guide.

In foot and ankle surgery, allograft tendons are frequently used in both primary and revision cases to enhance stability and function. However, postoperative complications, such as creep, can compromise the intended correction and stability over time. This paper presents a practical guide on pretensioning techniques prior to implantation, aiming to reduce creep and improve long-term outcomes. Using two common allograft specimens, we demonstrate pretensioning methods with specific focus on tendon stitching, techniques for securing the allograft to the graft board, optimal tensioning and time pa…

Meniscal Substitution With a Semitendinosus Tendon Autograft.

The menisci play a critical role in load transmission and shock absorption and serve as important secondary stabilizers of the knee. Unfortunately, meniscal repair is not always feasible, and resection may be required. Meniscal allograft transplantation is an established method of meniscal substitution; however, its use remains limited by graft sizing challenges, high costs, and restricted availability. The semitendinosus tendon has the capacity to remodel and revascularize within the intra-articular environment. The purpose of this technical note is to describe the surgical technique for meni

[Progress on the treatment of Achilles tendon rupture].

Achilles tendon rupture is an important research topic in the field of sports medicine. Its imaging diagnosis, treatment methods, rehabilitation strategies, and prognosis assessment have always been the focus of academic attention. MRI examination is widely used in diagnosing Achilles tendon rupture, but it has disadvantages such as high cost, long examination time, and metal contraindications. Ultrasound examination has the advantages of being non-invasive, low cost, comfortable, and portable, and it also has high specificity and accuracy, which has significant application value. The treatmen
