We performed a retrospective study in 42 consecutive tracheotomy patients hospitalized in an intensive care unit. Endoscopy was used systematically to assess all patients at decanulation and whenever symptoms suggested possible tracheal granuloma. Nine tracheal granulomas were evidenced. There was no significant association between presence of a granuloma and patient status: age, underlying chronic respiratory failure, or tracheobronchial infection. We report our experience with the treatment of patients with bilateral vocal cord paralysis. The sample was small because the disease is rare. The therapeutic techniques first used were arytenoidopexy using King's technique and Graff-Woodman's arytenoidectomy with cordopexy. Cordectomy was used when other techniques failed. Between 1980 and 1993, 10 infants underwent endoscopic treatment and/or open surgical excision of a subglottic haemangioma. The occurrence of scar laryngostenosis due to traumas and removal of laryngeal tumors is now on the increase. This negative trend may be corrected by rejection of long-term intubation, immediate conversion of crico-, conico- and thyrotomy into tracheostomy, employment of preventive laryngostomy in initial treatment of patients with serious trauma of the larynx. It should be taken into consideration that in restoration of respiration through natural airways bronchopulmonary complications, dysphonia, dysphagia, hair growth into the laryngeal lumen are possible.

A tracheo-innominate artery erosion in a 10-year-old boy 10 months after 80% burns in a gasoline explosion is reported. Tracheostomy was performed on the day of the burn to maintain an airway. Fatal hemorrhage from the erosion was the cause of death. Prevention can be achieved by: correct placement (second or third ring); aseptic tracheal toilet; positive nitrogen balance; and earliest possible tracheostomy removal.
