Trochanteric Femur Fractures: Application of Skeletal Traction during Surgery Does Not Alter Soft-Tissue Microcirculation.

Background and Objectives: Wound infections provoked by alterations in microcirculation are major complications in the treatment of trochanteric femur fractures. Surgical fracture fixation on a traction table is the gold standard for treatment, but the effect on tissue microcirculation is unknown. Microcirculation could be impaired by the pull on the soft-tissue or by a release of vasoactive factors. We hypothesized that intraoperative traction impairs soft-tissue microcirculation. Materials and Methods: In 22 patients (14 women, eight men), average age 78 years (range 36-96 ± 14), with trocha…

Circumferential shoulder laceration after posterior axilla sling traction: a case report of severe shoulder dystocia.

BACKGROUND: Shoulder dystocia is an unpredictable and potentially catastrophic complication of vertex vaginal delivery. Posterior axilla sling traction (PAST) has recently been proposed as a method to resolve severe shoulder dystocia when commonly used techniques have failed. CASE PRESENTATION: A 33-year-old woman (gravida 5, para 0) at 35 weeks, 1 day gestation underwent induction of labor for poorly controlled type 2 diabetes mellitus. Delivery of the large-for-gestational-age infant (4,060 g) was complicated by intractable shoulder dystocia, relieved at 3 minutes with PAST, resulting in a …

Neurologic Deficit During Halo-Gravity Traction in the Treatment of Severe Thoracic Kyphoscoliotic Spinal Deformity.

Correction of severe spinal deformity is a significant challenge for spinal surgeons. Although halo-gravity traction (HGT) has been shown to be well-tolerated and safe, we report here a case of neurologic decline during treatment. A 24-year-old male presents with severe thoracic kyphoscoliosis with > 180° of 3-dimensional deformity. Magnetic resonance imaging showed his thoracic spinal cord draped across his T7-9 apex. His neurologic exam showed lower extremity myelopathy. During week 7 at a goal traction weight of 18.1 kg, his distal lower extremity exam declined from 4+/5 to 2/5. His tractio…

Ultrastructural analyses of internal limiting membrane excised from highly myopic eyes with myopic traction maculopathy.

PURPOSE: To evaluate the ultrastructure of the internal limiting membranes (ILMs) excised during vitrectomy from highly myopic eyes with myopic traction maculopathy (MTM). The clinical findings before and after the vitrectomy were compared. METHODS: Seven eyes of 7 patients with macular retinoschisis were studied. Four of these eyes also had a foveal detachment but without a retinal break. All the eyes underwent vitrectomy with the creation of a posterior vitreous detachment and ILM peeling. The excised ILMs were examined by transmission electron microscopy (TEM). RESULTS: The retinas were r…

Dural Traction a Possible Cause of Hemodynamic Changes During Single-Level Transforaminal Lumbar Interbody Fusion.

BACKGROUND: Lumbar spinal surgery may be associated with electrophysiologic and hemodynamic abnormalities during the procedure. CASE DESCRIPTION: A 58-year-old man with grade II L4-5 spondylolisthesis and degenerative changes underwent single-level transforaminal lumbar interbody fusion. During decompression of the L4 foramina, distraction of the disc space, and placement of the interbody cage and pedicle screws, episodes of extreme bradycardia with up to 5 seconds of asystole were detected on electrocardiogram and invasive hemodynamic monitoring. The events correlated with and possibly could…

Management of the open abdomen using vacuum-assisted wound closure and mesh-mediated fascial traction.

BACKGROUND: The open abdomen has become an accepted treatment option of critically ill patients with severe intra-abdominal conditions. Fascial closure is a particular challenge in patients with peritonitis. This study investigates whether fascial closure rates can be increased in peritonitis patients by using an algorithm that combines vacuum-assisted wound closure and mesh-mediated fascial traction. Moreover, fascial closure rates for patients with peritonitis, trauma or abdominal compartment system (ACS) are compared. METHODS: Data were collected prospectively from all patients who underwe…

Comparison between Langerhans cell concentration in lichen planopilaris and traction alopecia with possible immunologic implications.

Lichen planopilaris and long-standing traction alopecia are both traditionally classified as scarring alopecias. The etiology of lichen planopilaris has not been fully elucidated, although an autoimmune mechanism is generally accepted with Langerhans cell involvement implicated in previous studies. The etiology of traction alopecia is generally considered to be the result of mechanical force with subsequent inflammation without an autoimmune component. Langerhans cells in pure traction alopecia have not been previously evaluated nor have Langerhans cell concentrations been compared among the s…

Traumatic vertical atlantoaxial instability: the risk associated with skull traction. Case report and literature review.

Traumatic overdistraction between C1 and C2 may occur when all the ligaments connecting C2 to the skull are ruptured, and may be manifested when an attempt to reduce C1-C2 subluxation is made by means of traction. We describe here the case of a patient with traumatic anterior atlantoaxial dislocation, who developed atlantoaxial vertical dissociation after skull traction using a Gardner-Halo with lb 4.02 (1.5 kg) of weight. The identification of patients who are susceptible to this complication is difficult. In this case, it might have been prevented by avoiding spinal traction. The aim of this…

Dynamic digital traction for unstable comminuted intra-articular fracture-dislocations of the proximal interphalangeal joint.

Unstable intra-articular fracture-dislocations about the proximal interphalangeal joint present a formidable management dilemma. Fourteen patients with comminuted injuries of the proximal interphalangeal joint were treated with dynamic digital traction and followed for an average of 24 months. Dorsal fracture-dislocation and pilon-type injuries made up the majority of cases. Average proximal interphalangeal joint articular surface involvement was 80%. Immediate motion was instituted after traction application. The device was applied in the office with readily available materials. At final foll…

External compression with elastic bandages: its effect on the peripheral blood circulation during skin traction.

Experiments were performed in rabbits to evaluate the effects of varying degrees of external pressure on the venous circulation in a limb. F … effects of external application of varying degrees of pressures with an elastic bandage during skin traction on the distal portion of the leg were evaluated by plethysmographic and thermometric techniques. Arterial pulsations obtained by photoelectric plethysmography at external limb pressures of 30mmHg were not significantly different in peak amplitude fr …

Diagnosis of brachial plexus traction lesions by sensory nerve action potentials and somatosensory evoked potentials.

Forty-two patients with unilateral brachial plexus traction lesions were investigated by recording sensory nerve action potentials (SNAPs) from the lower arm and somatosensory evoked potentials (SEPs) from the clavicle, the cervical spine and the scalp overlying the contralateral somatosensory cortex, in response to electrical stimulation of peripheral nerves. The median and radial nerves were assumed to derive principally from the C6 and C7 roots, and the ulnar nerve from the C8 and T1 roots. Combination of SEP and SNAP findings suggested a location for the lesion (preganglionic, postganglion…

Ultrastructure of traction retinal detachment in rhesus monkey eyes after a posterior penetrating ocular injury.

We studied the ultrastructural characteristics of an experimental model of traction retinal detachment secondary to a penetrating eye injury in the rhesus monkey. Cells with the characteristics of myofibroblasts were present in the intravitreal fibrous tissue and in epiretinal membranes. These cells were probably derived from many sources, and were capable of contraction through the mechanism of intracytoplasmic contractile proteins such as actin or myosin. The morphologic observations supported our hypothesis that traction retinal detachment is a cell-mediated event analogous to the processes…
