BACKGROUND: Individuals with sickle cell disease (SCD) experience poor clinical outcomes while transitioning from paediatric to adult care. Standards for SCD transition are needed. We established a Quality Improvement (QI) Collaborative that aimed to improve the quality of care for all young adults with SCD by establishing a standardised SCD transition process. This study evaluates the implementation of the Six Core Elements (6CE) of Health Care Transition, which was a fundamental component of the cluster-randomised Sickle Cell Trevor Thompson Transition Project (ST3P-UP) study.

BACKGROUND: Adolescents with complex urological conditions are at risk of bladder dysfunction, metabolic disturbances, neoplastic changes and deterioration in renal function. Hence they require appropriate transition to the adult service to ensure lifelong urological care is adequately provided.
OBJECTIVE: Barriers and difficulties to the transition process have been identified in the literature. To overcome these difficulties an Adolescent Transition Urology Clinic (ATUC) was established in 2009 where the patients are seen by a paediatric and an adult urologist, a urology nurse and if needed

The twin epidemics of HIV and incarceration impact Puerto Rico, which has limited resources to address the social and structural determinants of health in incarcerated populations. A Special Programs of National Significance grant supported a Puerto Rican community-based organization to implement the evidence-informed Transitional Care Coordination intervention among incarcerated persons living with HIV, targeting changes at the individual, organization, and systems levels. After implementation (November 2015-July 2018; n = 69), 93.1% of eligible clients were linked to community-based HIV care…

OBJECTIVES: Most patients with sickle cell disease (SCD) survive to adulthood; however, young adults with SCD have high rates of morbidity and mortality and may struggle in the transition to a healthy adulthood. The purpose of this study was to determine the readiness of a rural SCD population for the transition to adulthood and adult medical care.
METHODS: Adolescents and young adults who were current/former patients of a rural pediatric practice were surveyed. Assessments included difficulty dealing with daily care issues and perceived/encountered barriers to transition. Adolescents were as

BACKGROUND: Transition from Child and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS) is poorly managed, with discontinuity of care commonplace, leading to poorer outcomes, while evidence-based interventions to improve transition are scarce. This study is a secondary analysis of the MILESTONE trial, aiming to determine whether managed transition increases the proportion of young people who appropriately transition from CAMHS to AMHS.
METHODS: The MILESTONE trial was a multicenter, two-arm, cluster-randomized controlled trial across eight countries at 40 CAMHS

BACKGROUND: Neuromuscular diseases (NMDs) are rare, progressive conditions that require lifelong, multidisciplinary care. Advances in diagnosis and treatment have increased survival into adulthood, making the transition from paediatric to adult care a critical stage of its management. However, evidence suggests that transition practices remain inconsistent across Europe. This study aimed to map and evaluate the current transition practices for patients with NMDs across Europe.
METHODS: A cross-sectional survey was conducted by the European Reference Network for Rare Neuromuscular Diseases (ER

With an estimated 85% of individuals with spina bifida (SB) surviving into adulthood, SB-specific transition to adult healthcare guidelines are warranted to address the diverse and complex medical, adaptive, and social needs particular to this condition. This commentary discusses the SB Transition Healthcare Guidelines from the 2018 Spina Bifida Association's Fourth Edition of the Guidelines for the Care of People with Spina Bifida, reviews current transition care models in which such guidelines can be implemented, and explores further research topics in SB transition care.

Women are the fastest growing prison population, but little is known about the quality of maternal care for pregnant women in prison. In partnership with the Indiana Department of Correction (IDOC) Transitional Healthcare Team (THT), we developed a maternal standard of care (MSOC) audit tool to identify existing strengths and weaknesses and recommend evidence-based strategies for improvement. After creating the MSOC audit tool, we used it to audit 52 charts and conducted interviews of IDOC staff regarding maternal health care processes. Identified strengths include prenatal care, screening/tre

In many states, budget constraints are prompting earlier release of prison inmates. Prior studies have demonstrated elevated mortality rates in the post-release period but little is known about the health experiences of former inmates in the transition from prison to the community. The objective of this study was to understand the health-seeking experiences, perceptions of risk, and medical and mental health needs of former prisoners in the first two months after release from prison. Participants consisted of 29 former inmates within the first two months after their release from prison to the

PURPOSE: No studies have explored the experiences of Canadian mechanical ventilator-assisted adolescents (VAAs) living at home as they transition from paediatric to adult health providers. A better understanding of the needs of this growing population is essential to provide transition services responsive to VAAs and caregiver-identified needs.
METHODS: We conducted semistructured telephone interviews with adolescents and family caregivers who had recently initiated or completed transition to adult care recruited from three Canadian university-affiliated paediatric home ventilation programs.

BACKGROUND: With the optimization of medical care structures and the rapid progress in the development of new therapeutic methods, an increase in life expectancy is observed in patients with neuromuscular diseases. This leads to an expansion of the phenotypic spectrum, whereby new or previously less relevant disease manifestations in different organ systems gain more importance. The care of adolescents and young adults with neuromuscular diseases, therefore, requires increasingly close interdisciplinary collaboration within neuromuscular centers.
RESEARCH QUESTION: How can the transition proc

INTRODUCTION: Chronic kidney disease (CKD) presents unique medical and psychosocial challenges in children and adolescents that differ from adult patients with CKD. Transitioning from pediatric to adult care is a critical period. Insufficient planning and inadequate preparation increase the risk of medication nonadherence, loss to follow-up, graft failure in transplant patients, and increased morbidity. Transition models address barriers to successful transition, yet vary in their approach.
METHODS: We conducted a scoping review of the transition of care in pediatric CKD in accordance with PR

BACKGROUND: Transition from pediatric to adult health care is a vulnerable period for adolescents and young adults. Challenges include paucity of validated measures to assess patients' transition readiness. We evaluated the Transition Readiness Assessment Questionnaire (TRAQ) in adolescents and young adults with rheumatic, gastrointestinal, and endocrine disorders. We examined whether baseline TRAQ scores and other demographic variables predicted transition to adult care over a three year follow up period.
METHODS: In this descriptive study at a single institution, eighty-nine adolescents at

Recent medical advances are allowing more youth with neuromuscular disorders to live into adulthood, thus increasing the necessity to transition youth from pediatric to adult medical care. Examining salient factors and processes for successful transition planning from pediatric to adult medical care in youth with neuromuscular disorders is a nascent field of study. The current study aims to address the key quality improvement question of identifying factors to consider when planning for transition by exploring the views of youth with neuromuscular disorders, caregivers, and providers within a

Congenital heart defects are the most common singular congenital organ malformation. Due to advances in medical care, more than 95% of affected persons now reach adulthood. However, congenital heart defects are often associated with residuals or sequelae that require lifelong specialized care. Lack of or irregular follow-up care leads to worse outcomes. Nevertheless, follow-up care for adults with congenital heart disease (ACHD) is still often inconsistent in reality, and many patients receive care outside of qualified structures. The transition process-the shift from pediatric to adult medical
