The microbiome-nitric oxide axis in cancer immunotherapy: a targeted qualitative systematic review of mechanistic and translational evidence.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have transformed cancer treatment, but responses vary substantially among patients. Clinical studies have associated features of the gut microbiome with ICI outcomes, while experimental evidence suggests that microbial metabolites and nitric oxide (NO)-related pathways may influence antitumor immunity and tumor vascular biology. This qualitative systematic review evaluated the evidence supporting a proposed microbiome-NO-ICI framework. METHODS: Five databases were searched for peer-reviewed studies published between January 2000 and August 2025.

Development and validation of a cross-species breath metabolomics platform for translational VOC analysis.

The lack of universal cross-species sampling and quality control methods has limited the potential of breath metabolomics to advance from clinical discovery to mechanistic validation. To address this challenge, this study developed and systematically validated an integrated cross-species breath analysis platform, with its core components comprising a high-sensitivity mouse breath sampling system (FaunaScope) and a quality control (QC) strategy incorporating behavioral monitoring. By identifying ethyl acetate and dimethyl sulfide as characteristic interference markers, the platform exhibited hi

Macrophage-centered immunotherapy for osteosarcoma: mechanisms, repolarization, and translational strategies.

Despite multimodal treatment strategies, survival rates for osteosarcoma have remained stagnant, and immune checkpoint blockade targeting PD-1/PD-L1 alone has demonstrated limited clinical benefit. Tumor-associated macrophages (TAMs), which are abundant in osteosarcoma and predominantly exhibit an M2-like polarization, contribute significantly to immunosuppression, angiogenesis, metastasis, and chemotherapy resistance. This review outlines the origins and polarization dynamics of TAMs, and highlights central mechanisms through which they impair T-cell function (e.g., PD-1/PD-L1, TIM-3/galectin

Emerging liquid biopsy markers in thyroid cancer: from isolation technologies to clinical translation.

Diagnosis and treatment of thyroid cancer (TC) are undergoing radical changes, shifting from traditional tissue studies to liquid biopsies (LB). LB technologies provide effective, non-invasive solutions to these challenges. This paper reviews recent advances in the analysis of three important biomarker systems: circulating tumor cells (CTCs), circulating tumor DNA (ctDNA) and exosomes. Technically, microfluidics and bioaffinity-based physical capture strategies have significantly improved the efficiency with which CTCs can be separated and analysed at an individual cell level. Ultra-sensitive

From preclinical promise to regulatory reality: translation of small rodent acute injury therapies to human approval and safety outcomes: A systematic review.

BACKGROUND: Preclinical animal studies are considered essential for therapeutic development, yet translation to human benefit remains poor. The commonly cited translation rate of approximately 20% conflates trial entry with regulatory approval and may substantially overestimate the predictive validity of animal models for specific therapeutic indications. OBJECTIVES: To determine, for preclinical acute injury studies in small animal models: (a) the proportion progressing to human clinical trials; (b) the proportion achieving regulatory approval for the tested indication; (c) whether approvals

Adjunctive Therapies in Periodontitis: Current Concepts and the Future.

Periodontitis is a chronic, host-mediated inflammatory disease in which microbial dysbiosis and dysregulated immune responses drive the destruction of tooth-supporting tissues. Although conventional therapy remains centered on mechanical biofilm control, persistent inflammation and alveolar bone loss in susceptible individuals underscore the need for adjunctive strategies. Translating preclinical discoveries into predictable clinical outcomes, however, remains a major challenge in periodontal research. This narrative review integrates two interrelated themes, translational research methodology

T cell exhaustion: a two-sided blade in systemic lupus erythematosus, from molecular mechanisms to clinical translation.

Dysregulated immune responses and extensive inflammatory damage to several organs are hallmarks of systemic lupus erythematosus (SLE), a highly heterogeneous systemic autoimmune disease that significantly impairs patients' quality of life and prognosis. Persistent antigenic stimulation causes T cell exhaustion (Tex), a unique functional state that is carefully controlled by exogenous, temporal, and spatial factors. Tex plays a special bidirectional regulatory role in SLE: on the one hand, it exerts a protective effect by suppressing the excessive activation of autoreactive T cells, thereby mit

In Vivo CAR-T Therapy for Cancer Treatment: Mechanisms, Technological Advances, and Clinical Translation.

In vivo Chimeric Antigen Receptor (CAR)-T cell therapy reprograms a patient's own T cells directly inside the body, bypassing the complex and costly traditional manufacturing process. This is achieved by systemically delivering viral or non-viral vectors that genetically modify endogenous T lymphocytes to produce functional CAR-T cells de novo. By eliminating ex vivo cell processing, this strategy can simplify workflows, reduce costs, improve accessibility, and allow faster treatment. Key delivery platforms include engineered lentiviral and adeno-associated viral (AAV) vectors for lasting CAR

MAFB in Pancreatic β-Cell Development and Dysfunction: Implications for Diabetes and Translational Applications.

Large MAF transcription factors, including MafA and MafB, are essential for maintaining β-cell identity, function and survival. While MafA has been widely studied in pancreas development and type 2 diabetes, the extended roles of MafB in humans are still emerging. During embryogenesis, MafB promotes differentiation of β-cells. While MafB is downregulated in adult mouse β-cells, it remains active in adult human β-cells, indicating important species-specific functions. Mechanistically, MafB cooperates with other β-cell-specific transcription factors, including PDX1, NEUROD1 and NKX6.1, to regula

Gene Editing Strategies for Duchenne Muscular Dystrophy: From Molecular Mechanisms to Clinical Translation.

Duchenne muscular dystrophy (DMD) remains a major challenge in genetic medicine due to the difficulty of achieving durable, body-wide restoration of dystrophin in post-mitotic muscle tissues. Although current therapies-including exon skipping and micro-dystrophin gene replacement-have demonstrated clinical feasibility, their benefits are limited by incomplete efficacy, mutation specificity, and the need for repeated or high-dose interventions. These limitations highlight the need for strategies capable of directly and permanently correcting the underlying genetic defect. Recent advances in gen
