Tuberculous meningitis (TBM) is the most severe form of central nervous system tuberculosis and carries high morbidity and mortality. We report a 35-year-old man from Ecuador who presented with sepsis-like systemic symptoms and rapidly progressive neurological deficits. Initial imaging revealed cavitary pulmonary lesions and hilar lymphadenopathy, suggestive of pulmonary tuberculosis. Neurological deterioration prompted magnetic resonance imaging (MRI) of the brain and cervical spine, demonstrating communicating hydrocephalus, basilar leptomeningeal enhancement, diffuse cervical cord edema, an

Intradural extramedullary tuberculoma of the spinal cord (IETSC) is a rare and unanticipated manifestation of CNS tuberculosis. A 28 years old female diagnosed case of tubercular meningitis (TBM), already on antitubercular therapy (ATT) for last nine months, developed sensory and motor weakness in lower limbs along with saddle anaesthesia. Magnetic resonance imaging (MRI) with contrast of the spine revealed the presence of multiple space occupying lesions in dorsal spinal cord. She received oral steroid therapy for six weeks with escalation of ATT after which neurological functions improved re

STUDY DESIGN: Prospective study.
PURPOSE: To evaluate the utility of whole genome sequencing (WGS) in drug resistance testing, lineage of the organisms, and organism- related factors responsible for bacilli settling in the spine.
OVERVIEW OF LITERATURE: The workstream for the diagnosis of tuberculosis (TB) involves isolation and culture of the organism and drug resistance testing using phenotypic methods. Xpert MTB/RIF Ultra is a genetic-based method that detects for Mycobacterium tuberculosis DNA in the rpoB gene. Meanwhile, WGS is a newer genetic-based method that assesses the whole genome

BACKGROUND CONTEXT: Posterior element tuberculosis is rare accounting for only 3% to 5% of all spinal tuberculosis. To our knowledge, no case of isolated facet joint tuberculosis with coronal decompensation has been reported in literature so far.
PURPOSE: The purpose of this study was to describe a case of tuberculous arthritis of the lumbar facet joint and its effect on coronal balance of the spine.
STUDY DESIGN: The study design was a case report.
METHODS: A 14-year-old boy presented with a 3-month history of back pain without radiation and with normal neurological findings. Computed tomo

Tuberculosis is an infectious disease with a decreasing incidence in Germany. In particular, the occurrence of reinfections and possible complications associated with this disease, such as tuberculous meningitis/encephalitis and Landouzy sepsis are rare in industrial countries. In this article the intensive care treatment of a patient who initially underwent surgery for spinal stenosis is reported. Due to recurrent appearance of neurological symptoms with increasing severity and abscess formation in the spine, further surgery was performed. Additionally, the patient developed sepsis and mening

Central nervous system (CNS) tuberculosis commonly manifests as tubercular meningitis. CNS tuberculomas are more common intracranially and less frequently involve the spinal cord. Combination of intramedullary with intracranial tuberculomas is extremely rare. We report a case of concurrent occurrence of intramedullary tuberculoma with multiple intracranial tuberculomas in a young 16-year-old boy, who presented with two weeks history of paresthesias and weakness of the lower limbs and diminution of vision in left eye, who had been treated for pulmonary tuberculosis. Magnetic resonance imaging (

OBJECTIVE: To explore the advantage and indication of combined anterior and posterior surgeries for lumbarsacral junction tuberculosis.
METHODS: Eleven cases of the lumbarsacral junction tuberculosis were treated with combined anterior (radical debridement and autograft) and posterior (instrumentation and fusion) surgeries in one stage between January 2002 and December 2006. There were 9 males and 2 females with the age of 20-56 years old. The course of disease was 4 to 15 months, 6 months on average. The lessons were located at L5, S1 in 7 patients, at L4,5, S1 in 2 patients and at L5, S2 in

PURPOSE: This study aims to systematically evaluate the efficacy and timeliness of various specimens and diagnostic methods used to identify pathogens in spinal infections, thereby informing clinical diagnostic pathways.
METHOD: We conducted a retrospective examination of patients diagnosed with spinal infections between January 2023 and January 2025. We compared positivity rates and time-to-detection for microbial culture, mNGS, qPCR and RBT/SAT across different types of specimens: blood, muscle/vertebral biopsy, intraoperative specimens, and postoperative drainage.
RESULTS: Among 287 patie

BACKGROUND AND PURPOSE: Pyogenic and tuberculous spondylitis can mimic malignancy. The purpose of this study was to deter mine the efficacy of diffusion-weighted magnetic resonance imaging in differentiating spinal infection and malignancy.
METHODS: Fifty-one consecutive patients with suspected spinal infection or malignancy were enrolled in the study. Apparent diffusion coefficients (ADCs) of paraspinal soft tissue mass and normal and abnormal vertebral bone marrow were determined on the diffusion-weighted magnetic resonance images of the spine. The mean ADCs of normal and abnormal vertebral

Spinal hydatid disease is rare, destructive parasitic infection of the spine. The disease often presents with nonspecific symptoms. that overlaps with spinal tuberculosis, pyogenic infection, and metastatic malignancy. We report a case of 65-year-old man with thoracic spine involvement of Hydatid disease. Magnetic resonance imaging demonstrated destructive vertebral disease with a characteristic cystic morphology. The imaging pattern favoured hydatid cyst as the most likely diagnosis. Correlation with epidemiologic history and serology was used to confirm the diagnosis and guide management. Du

A case of diffuse cerebrospinal gliomatosis with extensive leptomeningeal spread is presented. The patient, an 18-year-old girl, was admitted due to progressive weakness and paresthesia of both legs, following rapid neuropsychiatric deterioration. An initial magnetic resonance imaging (MRI) study of the T-spine showed diffuse high signal intensities from T9 to T12 spinal cords on a T2 sagittal image and diffuse cord bulging at T1WI. This suggested an inflammatory lesion such as tuberculosis or fungal meningoencephalitis. A limited autopsy was performed. A microscopic examination revealed multi

Tuberculosis (TB) is the most prevalent infectious disease in the world. In recent years there has been a significant increase in the incidence of TB due to the emergence of multidrug resistant strains of Mycobacterium tuberculosis (M. tuberculosis) and the increased numbers of highly susceptible immuno-compromised individuals. Central nervous system TB, includes TB meningitis (TBM-the most common presentation), intracranial tuberculomas, and spinal tuberculous arachnoiditis. Individuals with TBM have an initial phase of malaise, headache, fever, or personality change, followed by protracted h
