Barriers to community engagement during the response to an Ebola virus disease outbreak in Uganda.

BACKGROUND: Uganda reported an outbreak of Ebola virus disease (EVD) in 2022. As part of the outbreak response, government and partners promoted community engagement, which seeks to involve communities in the design, implementation and evaluation of interventions to raise awareness, build trust between communities and partners and create ownership of interventions. This study, therefore, explored barriers to community engagement during the 2022-2023 EVD outbreak response in Uganda. METHODS: This qualitative study, conducted in five districts (Kampala, Kassanda, Kyegegwa, Mubende and Wakiso), …

Epidemiology, clinical spectrum, and outcomes of severe malaria in Eastern Uganda: a prospective study.

BACKGROUND: In sub-Saharan Africa, malaria remains a public health problem despite some reports of declining incidence in the period 2000-2018. Since 2019, there have been some reports of disease epidemics and resurgences in areas that had registered steep declines and unusual clinical presentations. This study aimed to describe the epidemiology, clinical spectrum, and outcomes of severe malaria in children among malaria-endemic Eastern Uganda, a region that has recently experienced disease epidemics. METHODS: This prospective study was conducted at Mbale Regional Referral Hospital, Uganda, f…

The implementation gap: a national evaluation of Uganda's Prevention of vertical transmission retention package.

BACKGROUND: Uganda has made progress in eliminating vertical transmission (VT) of HIV, but VT rates have plateaued in recent years. A major factor contributing to high VT rates is the loss of mother-baby pairs from HIV care. To improve retention, Uganda introduced a national Prevention of Vertical Transmission (PVT) retention package in 2015. This package included seven facility-based interventions: five core ones (appointment tracking, EDD/EID cohort monitoring, Bring-Back Mother-Baby initiative, color-coded DBS reminder stickers, birth cohort monitoring) and two additional interventions (poi…

Seroepidemiology of SARS-CoV-2 in a cohort of pregnant women and their infants in Uganda and Malawi.

BACKGROUND: Data on SARS-CoV-2 infection in pregnancy and infancy has accumulated throughout the course of the pandemic, though evidence regarding asymptomatic SARS-CoV-2 infection and adverse birth outcomes are scarce. Limited information is available from countries in sub-Saharan Africa (SSA). The pregnant woman and infant COVID in Africa study (PeriCOVID Africa) is a South-South-North partnership involving hospitals and health centres in five countries: Malawi, Uganda, Mozambique, The Gambia, and Kenya. The study leveraged data from three ongoing prospective cohort studies: Preparing for Gr…

Understanding women's and men's perspectives on cervical cancer screening in Uganda: a qualitative study.

BACKGROUND: Cervical cancer remains a significant but preventable threat to women's health throughout much of the developing world, including Uganda. Cervical cancer screening and timely treatment of pre-cancerous lesions is a cost-effective means of mitigating cervical cancer morbidity and mortality. However, only 5% of women in Uganda have ever been screened. Barriers to screening, such as social stigma and access to safe conditions, have been previously identified, but insights into the role of male spouses in encouraging or discouraging screening have been limited. To our knowledge, no stu…

Implementation of a fast triage score for patients arriving to a low resource hospital in Uganda.

BACKGROUND: The Kitovu Fast Triage (KFT) score predicts imminent mortality from mental status, gait and either respiratory rate or oxygen status. As some non-life-threatening conditions require immediate attention, the South African Triage System (SATS) assigns arbitrary rankings of urgency for specific patient presentations. AIM: Establish the feasibility of determining and then comparing the KFT score and explicitly defined SATS urgency rankings. METHODS: A computerized proforma used standardized methods of assessing and measuring mental status and gait, and respiratory rate and collected

Preventive measures against HIV among Uganda's youth: Strategies, implementation, and effectiveness.

Preventing HIV among Uganda's youth is a critical public health priority due to the high prevalence of HIV/AIDS and the disproportionate burden of new infections among young people. This paper examines the preventive measures implemented to combat HIV/AIDS among Uganda's youth, focusing on strategies, implementation efforts, and the effectiveness of interventions. Comprehensive sexuality education programs have been implemented in schools and communities to provide young people with accurate information on HIV transmission, prevention methods, and reproductive health. Condom distribution and p…

Identification of urological anomalies associated with anorectal malformation in southwestern Uganda: Limitations and opportunities.

INTRODUCTION: Anorectal malformations (ARMs) may be associated with congenital anomalies affecting other body parts namely vertebral, anorectal, cardiac, tracheoesophageal, renal, and limb (VACTERL) with varying incidences of 7%-60% . Genitourinary defects might occur approximately in 50% of all patients with anorectal malformations hence patients should be evaluated from birth to rule out these defects. OBJECTIVE: To identify urological anomalies associated with anorectal malformation in southwestern Uganda. STUDY DESIGN: This was a descriptive retrospective cohort study conducted at our re…

Suggested solutions to barriers in accessing healthcare by persons with disability in Uganda: a qualitative study.

BACKGROUND: There are 1.3 billion people with disabilities globally, and they frequently face barriers to accessing healthcare, contributing to their worse health and higher mortality. However, little research has explored patient-reported approaches to improve healthcare for persons with disabilities. Consequently, this study aimed to explore possible solutions and recommendations to address the existing barriers to access to healthcare for persons with disabilities in rural Uganda. METHODS: We conducted 27 semi-structured interviews with individuals with various disabilities in rural Luuka …

Prevalence and Correlates of Frailty Among Older People With and Without HIV in Rural Uganda.

BACKGROUND: The relationship between HIV and frailty, a predictor of poor outcomes in the face of stressors, remains unknown in older people in sub-Saharan Africa. METHODS: We analyzed data from the Quality of Life and Ageing with HIV in Rural Uganda cohort study to estimate the prevalence and correlates of frailty among older people with HIV (PWH) on long-term antiretroviral therapy and among age- and sex-matched HIV-uninfected comparators. Frailty was defined as a self-report of 3 or 4 (and pre-frailty as 1 or 2) of the following phenotypic variables: weight loss, exhaustion, low activity, …

Molecular detection of Coxiella burnetii in ticks collected from animals and the environment in Uganda.

AIMS: Coxiella burnetii is a highly infectious organism that is easily spread through aerosols causing Q fever in humans. Ticks can harbour and transmit C. burnetii to animals, contributing to disease maintenance. Our aim was to examine the presence of C. burnetii in ticks in Uganda. METHODS AND RESULTS: In this study, ticks were collected from five Ugandan districts and tested by real-time PCR for C. burnetii (Coxiella outer membrane protein 1 gene). A total of 859 tick pools (9602 individual ticks) were tested, and pool positivity for C. burnetii was 5.5% (n = 47). Pooled prevalence differe…

Genomic surveillance of antimicrobial-resistant Escherichia coli in fecal sludge and sewage in Uganda.

The global increase of antimicrobial resistance (AMR) is a major public health concern. An effective AMR surveillance tool is needed to track the emergence and spread of AMR. Wastewater surveillance has been proposed as a resource-efficient tool for monitoring AMR carriage in the community. Here, we performed genomic surveillance of antimicrobial-resistant Escherichia coli obtained from fecal sludge and sewage in Uganda to gain insights into E. coli epidemiology and AMR burden in the underlying population. Selective media containing different antibiotic combinations (cefotaxime, ciprofloxacin,…

Consequences of school closures due to COVID-19 in DRC, Nigeria, Senegal, and Uganda.

In 2020 and 2021, Governments across the globe instituted school closures to reduce social interaction and interrupt COVID-19 transmission. We examined the consequences of school closures due to COVID-19 across four sub-Saharan African countries: the Democratic Republic of Congo (DRC), Nigeria, Senegal, and Uganda. We conducted a qualitative study among key informants including policymakers, school heads, students, parents, civil society representatives, and local leaders. The assessment of the consequences of school closures was informed by the Diffusion of Innovations theory which informed t…

The gendered socioeconomic impact of the COVID-19 pandemic on women with fistula repair in Uganda.

Public health mitigation strategies for SARS-COV-2 are effective in limiting the spread of COVID-19; however, these restrictions can create gendered socioeconomic impacts and further isolate marginalized communities from essential resources. Our qualitative study aimed to understand the gendered effects of the COVID-19 lockdown on a population of women in Uganda with a history of prior obstetric fistula repair, a group experiencing greater vulnerability and less support due to intersecting stigmatized identities. We conducted in-depth interviews among 30 women with prior genital fistula and co…

Sero-prevalence and factors associated with anti-Brucella antibodies in slaughter livestock in Uganda.

INTRODUCTION: Brucellosis is endemic in Uganda and is a major cause of production losses in livestock. Early detection and quantification of the disease is vital for its control and eradication. The aim of this study was to assess the sero-prevalence and factors associated with anti-Brucella antibodies in slaughtered livestock. MATERIALS AND METHODS: Sera from 886 cattle, 925 small ruminants, and 900 pigs were collected from regional abattoirs in Northern, Eastern and Central Uganda. To estimate sero-prevalence, sera were serially tested using a combination of the Rose Bengal Test (RBT) and N…

Trypanosome diversity in small mammals in Uganda and the spread of Trypanosoma lewisi to native species.

Uganda's diverse small mammalian fauna thrives due to its rich habitat diversity, which hosts a wide range of blood parasites, including trypanosomes, particularly the subgenus Herpetosoma typical for rodent hosts. We screened a total of 711 small mammals from various habitats for trypanosomes, with 253 microscopically examined blood smears and 458 tissue samples tested by nested PCR of the 18S rRNA gene. Of 51 rodent and 12 shrew species tested, microscopic screening reaches 7% overall prevalence (with four rodent species positive out of 15 and none of the shrew species out of four), while ne…

Impact of different mosquito collection methods on indicators of Anopheles malaria vectors in Uganda.

BACKGROUND: Methods used to sample mosquitoes are important to consider when estimating entomologic metrics. Human landing catches (HLCs) are considered the gold standard for collecting malaria vectors. However, HLCs are labour intensive, can expose collectors to transmission risk, and are difficult to implement at scale. This study compared alternative methods to HLCs for collecting Anopheles mosquitoes in eastern Uganda. METHODS: Between June and November 2021, mosquitoes were collected from randomly selected households in three parishes in Tororo and Busia districts. Mosquitoes were collec…

Investigation of a COVID-19 outbreak at a regional prison, Northern Uganda, September 2020.

Despite implementing measures to prevent introduction of COVID-19 in prisons, a COVID-19 outbreak occurred at Moroto Prison, northern Uganda in September 2020. We investigated factors associated with the introduction and spread of COVID-19 in the prison. A case was PCR-confirmed SARS-CoV-2 infection in a prisoner/staff at Moroto Prison during August-September 2020. We reviewed prison medical records to identify case-patients and interviewed prison and hospital staff to understand possible infection mechanisms for the index case-patient and opportunities for spread. In a retrospective cohort st…
