[Laparoscopic treatment of perforated duodenal ulcer].

OBJECTIVE: The aim of this study was to determine if laparoscopic simple closure for perforated duodenal ulcer is possible, efficient and safe. METHODS: Thirty-five consecutive patients with perforated duodenal ulcer and peritonitis were treated by simple closure, peritoneal lavage and omentoplasty. This treatment was performed laparoscopically. RESULTS: Diagnosis was confirmed in all cases. Conversion to laparotomy was necessary in two patients due to a technical problem and poor tolerance to pneumoperitoneum, respectively. Mean operative time was 120 min. Morbidity and mortality rates were…

The treatment of peptic ulcer disease.

There is a continuation of the debate on the management of dyspepsia while the role of Helicobacter pylori in duodenal ulcer disease is being questioned with renewed vigor, specifically in the United States. The interaction of NSAIDs and H. pylori provided some interesting, if at times confusing, literature while the debate on the safety of long-term acid suppression remained unresolved. There were some interesting developments with regard to therapeutic agents during this period. A fourth proton pump inhibitor was introduced to the market while cisapride, a drug previously considered safe, wa…

[Acid-base balance in ulcer patients].

Acid-base balance was studied in 60 ulcer patients in exacerbation and after treatment with losek and blockers of H2-histamine receptors. In exacerbation, all the patients had acidosis of metabolic or respiratory type. Losek changed blood pH insignificantly, and one acidosis type changed for the other. A single ranitidin dose shifted pH to alkalosis in healthy subjects while ulcer patients given famotidine restored acid-base balance. The latter is shown to correlate with gastric secretion of bicarbonates. Acidosis may contribute to exacerbation of ulcer.

Solitary rectal ulcer syndrome in children.

The solitary rectal ulcer syndrome (SRUS) is an unusual disorder in childhood. Although well recognized in adult literature, the pediatric experience with this condition is limited, so SRUS often goes unrecognized or misdiagnosed. There are very few pediatric case reports in the English literature. This report describes four patients who presented with rectal bleeding, constipation, mucous discharge, and lower abdominal pain, with a diagnosis of SRUS. The diagnosis was made by rectoscopy, defecogram, anorectal manometry and histopathological evaluation. In two patients, defecogram showed a rec…

Clinico-histopathological findings of Buruli ulcer.

Here, we report the clinico-pathological findings of Buruli ulcer. The patients were 2 females, 9 and 23 years of age and one male, 47 years of age from the Ashanti Country of Ghana. Clinically, cutaneous lesions were classified as nodular, ulcero-nodular and ulcerative. Histopathologically, lesions involved cutaneous and subcutaneous tissue, which showed lympho-epithelioid cell proliferation and panniculitis with characteristic fat necrotic changes. Vascular inflammation, with the nerve tissue involvement, are prominent features on the chronological spectrum of the 3 cases. In all but the ear…

Leg ulcer in hydroxyurea-treated patients.

A cutaneous ulcer is a lesser known complication of hydroxyurea treatment. Out of 39 patients [18 polycythaemia vera (PV), 13 essential thrombocythaemia (ET), 4 chronic myeloid leukemia (CML), 4 undefined myeloproliferative diseases (MPD)] treated with hydroxyurea, 6 (4ET, 1PV, 1CML) developed a cutaneous ulcer during a period of less that 2 years' treatment. In all but one of the patients the ulcers were situated in the ankle region. At the time of onset of ulceration, none of them had extreme values in their peripheral blood counts. All had one or more of the predisposing factors such as min…

Rectal carcinoma with stercoral ulcer perforation.

We report a case of ruptured stercoral ulceration due to chronic constipation which is caused by rectal carcinoma. This case suffered from difficulty of stool passage for 5 months. Periumbilical pain and current-jelly stool were experienced before his admission. Physical examination revealed diffuse abdominal rebounding pain and laboratory data showed leukocytosis. Computed tomography demonstrated marked dilatation of the sigmoid colon with stool impaction due to neoplastic growth in the rectosigmoid junction. Thickening and edematous change of the colonic wall were noted. There was amorphous …

Pediatric gastritis and peptic ulcer disease.

Inflammation of the gastric and duodenal mucosa is the end result of an imbalance between mucosal defensive and aggressive factors. The degree of inflammation and imbalance between defensive and aggressive factors can then result in varying degrees of gastritis and/or mucosal ulceration. Gastritis and ulcers of the duodenum or stomach can be classified as primary or secondary. The majority of children with chronic gastritis and ulcers in the stomach or duodenum have secondary inflammation or mucosal ulceration. These secondary ulcers generally occur due to a systemic condition like head trauma…

[The hemorrhagic complication of peptic ulcer].

The hemorrhagic complication of peptic ulcer is a pathology which is still responsible for a high mortality. After reviewing the etiopathogenesis and physiopathology of hemorrhagic complications in the light of the recent discoveries regarding H. pylori, the authors report their experience and confirm the important role of surgery in the treatment of this pathology. Resolution is improved by the use of surgical staplers which are regarded as particularly valuable in emergency surgery where the need to cut operating times has undoubted advantages in terms of patient survival.

[Bleeding in chronic refractory peptic ulcer].

The authors reviewed the clinical problems of the bleeding in refractory ulcers. Incidence of refractory and easily-relapsing ulcers are about 40% of all chronic ulcers and their bleeding rate seemed to be more than 10% of them. We discussed the relationship between bleeding and risk factors such as age, NSAID use, H. pylori infection, etc. The consideration to them is very important in the present time, being difficult to predict ulcer bleeding.

[Ulcer disease: new facts--new issues].

Involvement of Helicobacter pylori in pathogenesis of ulcer, the role of inflammation induced by the bacteria and possible mechanisms of hypergastrinemia are considered. The results of therapy directed to eradication of H. pylori from the clinical and pathogenetic point of view are presented. The role of H. pylori in asymptomatic gastritis and in gastric and duodenal ulcer is discussed. The information on genetic variability of H. pylori and its strains with different pathogenicity distinguished on the basis of genes encoding vacuolising cytotoxin and cytotoxin-associated protein is provided.

[Postop complications of perforative ulcer surgery].

Of 342 patients operated on for gastroduodenal perforative ulcer the complications have occurred in 63 (18.4%) and a progressive peritonitis in particular--in 45 (71.4%). In 4 (2.8%) of patients a peritonitis progress was noted after truncal vagotomy and ulcer excision conduction without the infection-toxical shock signs.

Bacterial corneal ulcer: a multivariate study.

A retrospective clinicomicrobiological review of 314 patients with bacterial corneal ulcers from January 1982 to December 1992 was performed. Multivariate statistical analysis was done with multiple logistic regression using PROC LOGIST of SAS statistical software. Positive cultures were grown from 134 (42.7%) of the patients. Pseudomonas aeruginosa, staphylococci, and Acinetobacter spp. were the most frequent pathogens. Significant associations between contact lens use and P. aeruginosa (odds ratio, OR 8.16), between previous herpes simplex keratitis and Streptococcus spp. (OR 18.2) were foun…

Non-traumatic rhabdomyolysis with gluteal ulcer.

The case is presented of a 34-year-old woman with rhabdomyolysis due to massive intake of nimetazepam, a benzodiazepine hypnotic. On admission, the patient had numerous blisters all over the body. One of the blisters in the gluteal region developed into a deep ulcer accompanied by muscle necrosis although it was not at a pressure point. The ulcer needed surgical intervention. Rhabdomyolysis is caused by the lysis and necrosis of muscle due to direct or indirect injury, high fever, ischaemia, hypoxia or enzyme defects. Release of myocyte components into the circulation then may induce major pro…

[Postbulbar ulcer. An eclectic surgical approach].

The only possible explanation for the large variability in therapeutic results following surgery for postbulbar ulcer (PBU) may be the main attitude towards the postbulbar lesion. It is always desirable to remove the ulcer, but occasionally this may submit the patient to high risks, unacceptable for a benign condition. Pathogenic operation may be performed, regardless the handling of the ulcer, so only the main ulcer complication may decide the need for ulcer removal. The aim of our retrospective study upon 78 cases operated between 1984-1994 was consistency between actual clinical reality of …
