Roscovitine ameliorates endotoxin-induced uveitis through neutrophil apoptosis.

Neutrophils have been recognized as critical response cells during the pathogenesis of endotoxin‑induced uveitis (EIU). Apoptosis of neutrophils induced by roscovitine has previously been demonstrated to ameliorate inflammation in several in vivo models. The present study aimed to assess whether roscovitine ameliorates EIU. EIU was induced in female C57BL/6 mice by a single intravitreal injection of lipopolysaccharide (LPS; 250 ng). The mice were divided into three groups as follows: LPS alone, LPS plus vehicle, LPS plus roscovitine (50 mg/kg). The mice were euthanized 12, 24, 48 and 72 h afte…

Uveitis in juvenile idiopathic arthritis: recent therapeutic advances.

Uveitis is a common association of juvenile idiopathic arthritis (JIA) that has previously been characterized by poor visual prognosis with limited options for effective treatment. Since corticosteroid treatment is not a preferred long-term option for most patients with this condition, systemic immunosuppressive therapy is frequently employed. The medical options for the treatment of JIA-associated uveitis have recently expanded beyond conventional immunosuppressive drugs to the biological agents. The biological drugs that are most commonly employed for JIA-associated uveitis are the tumor nec…

Unilateral snow banking in tuberculosis-related intermediate uveitis.

Snow banking is usually a term coined to describe the accumulation of vitreous exudates over the pars plana and the peripheral retina in pars planitis. Snow banking is very rare in tubercular intermediate uveitis. A 32-year-old male was diagnosed to have intermediate uveitis due to tubercular etiology in the right eye. Laboratory investigations include an increased erythrocyte sedimentation rate, positive Mantoux test, and computed tomography thorax showing mediastinal lymphadenopathy. Transbronchial needle aspiration of the lymph nodes showed chronic granulomatous inflammation with caseation.…

HLA-A29-Positive Uveitis: Birdshot Chorioretinopathy, What Else?

Birdshot chorioretinopathy (BSCR) is a relatively rare form of uveitis, which is strongly correlated with the histocompatibility leukocyte antigen (HLA)-A29 class I type. Nevertheless, HLA typing is not diagnostic. The purpose of the present study was to retrospectively evaluate the ocular manifestations associated with the presence of HLA-A29 other than typical BSCR. Charts of consecutive patients with a diagnosis of intraocular inflammation and who were found to be positive for the presence of HLA-A29 were retrospectively reviewed. Only 7 patients met the criteria for a definite diagnosis of…

Behçet disease-associated uveitis successfully treated with golimumab.

Over the past decade, the off-label use of biologic agents such as TNF-α antagonists, including infliximab and adalimumab, has improved the treatment armamentarium for refractory immune-mediated uveitis, with particular success in Behçet disease-associated uveitis. Golimumab is a novel fully human anti-TNF-α monoclonal antibody that has been approved for the treatment of rheumatoid arthritis, psoriatic arthritis, and ankylosing spondylitis, with very promising results. Herein, the authors present the use of GLM in a case of Behçet uveitis refractory to other TNF-α blockers. There are only two …

Recurrent anterior uveitis with hypopyon revealing relapsing polychondritis.

Relapsing polychondritis (RP) is a rare disease involving cartilaginous structures, predominantly the ears, nose and laryngotracheobronchial tree. The eyes, cardiovascular system, peripheral joints, skin, and central nervous system may also be affected. Involvement of all ocular structures has been described. Non-granulomatous uveitis is a common feature; however hypopyon is uncommon. We herein present the case of a 42-year-old man who had been diagnosed with bronchial asthma for a long period; and who presented with recurrent hypopyon uveitis as a revealing manifestation of relapsing polychon…

What is new HLA-B27 acute anterior uveitis?

Acute anterior uveitis (AAU) is the most common form of uveitis, accounting for approximately 90% of all cases. Half of all cases of AAU are HLA-B27 positive. The disease is typically acute in onset, unilateral, nongranulomatous inflammation involving the iris and ciliary body, with a tendency to recurrent attacks. Approximately 50% of all patients with HLA-B27 AAU develop an associated seronegative arthritis (SNA), while approximately 25% of the patients initially diagnosed with HLA-B27 SNA develop AAU. Environmental factors play a critical role in the pathogenesis of AAU; in particular, bact…

Idiopathic retroperitoneal fibrosis presenting with recurrent bilateral uveitis.

Patients with recurrent uveitis are often evaluated for the presence of underlying systemic disease. The authors describe a 55-year-old black female who presented with isolated recurrent anterior uveitis. Laboratory evaluations were notable for elevated inflammatory markers. She subsequently developed left lower extremity painless swelling; ultrasound evaluation was negative for deep venous thrombosis. CT scan of her abdomen and pelvis demonstrated multiple amorphous soft tissue densities throughout the small bowel mesentery and retroperitoneum, associated with left-sided hydronephrosis. Histo…

Pigment dispersion syndrome masquerading as acute anterior uveitis.

INTRODUCTION: Signs and symptoms of pigment dispersion may be confused with those of acute anterior uveitis. This case series is intended to aid the ophthalmologist in the clinical differentiation between these two disorders. CASE SERIES: The authors present a series of 6 patients with pigment dispersion who were initially diagnosed as having acute anterior uveitis and treated with anti-inflammatory medication, including corticosteroids. The patients were referred for a second opinion due to poor or no response to therapy and were found to have pigment dispersion instead of uveitis. DISCUSSI…

[Can tumor necrosis factor inhibitors induce sclero-uveitis?].

We report three cases of female patients who presented a first episode of unilateral scleritis or acute anterior uveitis while they were treated for 12-16 months by TNF inhibitor: etanercept (Enbrel((R))) 25mg twice weekly for rheumatologic diseases (rheumatoid arthritis or ankylosing spondylitis). Ocular inflammation was resistant to the usual treatment. Some cases of scleritis and uveitis are known to be drug-induced. TNF inhibitors seem to be part of them. Ocular inflammation is usually the first episode. It appears generally in the first year of the treatment by TNF inhibitors and resists …

Granulomatous inflammation in juvenile idiopathic arthritis-associated uveitis.

PURPOSE: The uveitis that is associated with juvenile idiopathic arthritis (JIA) has typically been described as nongranulomatous. This study reports the prevalence of granulomatous disease in a population with JIA-associated uveitis. METHODS: We conducted a retrospective analysis of all patients with JIA-associated uveitis seen between 1973 and 2006 at a hospital-based uveitis service. Patients with uveitis were included if they were diagnosed with JIA by a specialist and experienced at least 6 weeks of arthritis starting before the age of 16 years. Granulomatous disease was defined as Busac…

Unilateral granulomatous post-streptococcal uveitis with elevated tension.

PURPOSE: We document a presumptive case of unilateral post-streptococcal uveitis. The affected eye manifested a granulomatous anterior uveitis with elevated intra-ocular pressure (IOP) compared with the contra-lateral eye. This presentation was preceded by a recent streptococcal pharyngitis. CASE REPORT: An asymptomatic, otherwise healthy 13-year-old black female presented with anterior chamber cells, mutton fat resembling keratic precipitates, and elevated IOP in the left eye relative to the right eye. The patient had been diagnosed with streptococcal pharyngitis approximately 1 week before …
