A 14-year-old girl with severe coronary artery disease due to mucocutaneous lymph node syndrome, "Kawasaki disease", admitted to our hospital. Though she had no angina on any effort in her daily life, coronary angiography (CAG) demonstrated 90% stenosis and a large aneurysm of left main trunk. Based on the CAG findings, we determined to do the coronary artery bypass grafting (CABG) to left anterior descending artery (LAD) and 1st diagonal branch (D1) to prevent the occurrance of fatal acute myocardial infarction. We attempted to use bilateral internal thoracic arteries (ITAs) at the beginning

For a 6-year period, 96 patients had 146 below-knee angioplasties. There were 31 total occlusions and 95 multiple stenoses. All patients had distal ischemia, and 40% had gangrene. The primary success rate was 97%, and the 2-year limb salvage rate was 83%. The ankle/brachial index increased from a mean of 0.25 before the procedure to 0.62 afterward. At 2 years (35 patients), the mean ankle/brachial index was 0.55. For the same period, 320 femorodistal bypasses were performed. The results of angioplasty are comparable to those of surgery, but angioplasty is only suitable in about 20-30% of patie

In this prospective study duplex scanning was performed in 21 consecutive patients (23 lower extremities) referred for descending phlebography. The grade of deep venous insufficiency (Grades 1-4) was estimated in a blinded fashion with phlebography and duplex technique. Agreement occurred in 15 of 23 lower extremities. Discrepancies were found particularly in patients with minor degrees of reflux. The patients with reflux below the knee according to phlebography were also identified with ultrasonography. The authors found duplex scanning to be accurate method of evaluating deep venous valvular

This is a prospective study comparing selective angiography and magnetic resonance (MR) imaging in the evaluation of the patency of coronary artery bypass grafts (CABGs). Twenty-eight patients with 52 grafts were studied (10 internal mammary artery grafts, 19 saphenous vein grafts including 2 sequential grafts, and 15 right coronary artery saphenous vein grafts). The mean interval between MR imaging and the surgical procedure was 13.2 months. Results obtained with angiography were as follows: 39 patent grafts; 4 patent but stenotic (greater than 50%); 9 occluded CABGs. Magnetic resonance imagi

A saphenous vein graft was implanted from the right subclavian to the right common carotid artery in seven dogs (group I) and between the right and left common carotid arteries in another seven dogs (group II). The recipient artery was ligated proximally to augment blood flow through the graft. Immediately after the anastomoses were completed, the average blood flow through the graft was 32 +/- 25 mL/min in group I and 122 +/- 22 mL/min in group II. At sacrifice 30 days later, angiography showed that all grafts in group I were thrombosed, whereas six (87.5%) of seven grafts in group II were pa

Superior vena cava (SVC) occlusion in hemodialysis patients presents significant therapeutic challenges, particularly when complicated by stent migration. Sharp recanalization may serve as a salvage intervention for refractory cases though high-risk. Here, we present a 72-year-old male with stent migration-induced recurrent SVC syndrome. After failed conventional guidewire recanalization, sharp recanalization was performed with a dual-snare snare-assisted approach. We innovatively performed vertical penetration through the migrated stent. A stent was then vertically deployed within the expande

AIMS: Transvenous lead implantation for pacemaker (PM) or implantable cardioverter defibrillators systems in children and young patients with congenital heart disease is widely practiced. With longer implantation times, the risk for vascular occlusion increases. Transvenous angioplasty may be used to maintain venous patency for system revision in the future. Retrospective analysis of the interventional techniques employed in our young patients undergoing PM revision may identify its clinical benefit.
METHODS AND RESULTS: Between May 2005 and August 2010, 28 procedures to maintain central veno

Efficient hemodialysis requires establishing a permanent stable vascular access. Our study was designed to evaluate formaldehyde-fixed arterial allografts as hemodialysis access for end-stage renal disease. Various parameters were determined for 68 formaldehyde-fixed, cadaver-derived allografts transplanted into 43 hemodialysis patients. The sources of the allografts were determined to be free of cytomegalovirus, hepatitis B and hepatitis C, and HIV infections. These allografts were monitored for rejection, blood flow, patency rates, and complications. Overall, antigenicity of the allografts w

Directional atherectomy alone or with supplemental percutaneous transluminal angioplasty was used to treat peripheral vascular lesions in 77 patients (85 procedures). Lesions involved 17 iliac arteries, 45 infrainguinal arteries, and 23 laser extremity vein bypass grafts. Technical success, defined as reduction of stenosis diameter to 30% or less of the normal vessel diameter, was achieved in 78 of 85 (92%) cases. The complication rate was 21% (18 of 85 procedures). Most complications were minor and were related to puncture sites. Patients underwent noninvasive follow-up studies, including mea

Arterial reconstructions in high flow, low resistance locations (aorta, iliac and common femoral arteries) usually have good long-term results. The late failure rate of below-knee arterial grafts is significantly higher because of their considerable length, the limited outflow bed and the small diameters (low flow, high resistance). In addition, there is the biomechanical stress with genuflexion. Apart from the operative technique and patient-specific vascular morphology, the long-term success of crural reconstructions depends largely on the type of arterial substitute. Based on a retrospectiv

Stent grafts have improved secondary patency in arteriovenous grafts (AVGs) by effectively treating venous outflow lesions and have contributed to prolonged secondary patency of AVGs. As AVGs remain functional for longer periods, degeneration of the prosthetic graft itself becomes more frequent, increasing the need for graft-side interventions. During surgical reconstruction of AVGs, surgeons may be required to manage previously implanted stent grafts. We report a case of AVG reconstruction using the transected end of a pre-existing stent graft as the venous anastomosis. A 78-year-old woman un

BACKGROUND: To find out a suitable procedure to dissect an arterialised thick walled vein to create secondary angioaccess for dialysis. Delicacy of the vein wall, high rate of thrombosis and difficulty to find out a suitable vein are the main obstacles to vascular surgeons during the creation of secondary angioaccess.
METHODS: Forty patients in need for secondary angioaccess for dialysis were admitted for basilic vein transposition. They were classified randomly into two equal groups matched for age and sex. Group A patients were submitted to traditional basilic vein transposition. In group B

In vascular surgery, vascular anastomosis is a common reconstructive technique used to restore blood flow. However, anastomotic restenosis is a frequent postoperative complication, primarily caused by surgery-induced vascular injury, intimal hyperplasia, and inflammatory responses. The mouse femoral artery anastomosis model is widely used to investigate the mechanisms of anastomotic restenosis and vascular repair. Microscopically guided end-to-end femoral artery anastomosis allows precise simulation of vascular injury and repair processes following surgery, providing a reliable experimental to

BACKGROUND: Venous hypertension is a significant cause of patient morbidity and decreased quality of life. Common etiologies of venous hypertension include deep venous thrombosis (DVT) or congenital abnormalities resulting in chronic outflow obstruction. We have implemented an aggressive endovascular approach for the treatment of iliac venous occlusion with angioplasty and stenting. The purpose of this study was to determine the patency rates with this approach at a large tertiary care center.
MATERIALS/METHODS: All patients undergoing iliofemoral venous angioplasty and stenting over a 4-year

OBJECTIVES: Over the last decade, the number of endovascular procedures performed on the superficial femoral (SFA) and popliteal arteries (PA) has significantly increased. There is no consensus on the optimal form of intervention used in this arterial segment. While some have advocated balloon angioplasty alone, others have championed either selective or primary stenting of these lesions. It is the purpose of this study to determine the efficacy and durability of primary stenting of the superficial femoral and popliteal artery.
METHODS: All patients undergoing peripheral angioplasty by a sing

By decreasing plaque burden, atherectomy provides an alternative to angioplasty and stenting as a means of revascularizing patients with peripheral arterial disease. A new atherectomy device (SilverHawk) has recently been approved by the Food and Drug Administration, but the results with its use are unclear. We analyzed a series of consecutive patients undergoing atherectomy. We retrospectively reviewed the charts of 35 patients undergoing infrainguinal (IF) atherectomy in 38 limbs. The Trans-Atlantic Inter-Society Consensus (TASC) classification and Society of Vascular Surgery runoff scores w

INTRODUCTION: Subintimal angioplasty (SIA) was introduced in the late 1980s and is a supplement to bypass surgery. Adaptation of the technique has been hampered by high rates of early intervention to maintain patency, but the long term assisted patency is good.
REPORT: The superficial femoral and popliteal artery containing a patent subintimal canal were explanted from a patient who died in the authors' ward. Histological analysis indicated that the lumen was created in the medial layer of the vessel wall. A collagen rich neointima and fragmented internal elastic lamina were observed, presuma

PURPOSE OF REVIEW: Catheter-related bacteremia (CRB) and catheter dysfunction are major sources of morbidity and mortality when central venous catheters are used as long-term vascular access in hemodialysis patients. Attempts have been made to minimize these complications by the prophylactic use of catheter locking solutions. This review aims to describe the recent advances in the field of lock solutions and to discuss the benefits and risks associated with the routine use of antimicrobial and/or antithrombotic lock solutions.
RECENT FINDINGS: Antithrombotic lock solutions may improve patency

A 53-year-old woman presented with an iatrogenic right hepatic artery pseudoaneurysm after a laparoscopic cholecystectomy. Approximately 1 year after the cholecystectomy, liver transaminases were elevated, and she complained of recurrent "crampy" right upper quadrant pain that radiated posteriorly to her back. Imaging studies demonstrated an aneurysm or pseudoaneurysm of the hepatic artery at the porta hepatis, with possible infiltration into the parenchyma between the right and left lobes of the liver. Selective celiac arteriography showed a 90% stenosis of the right hepatic artery with a lar

BACKGROUND: Central venous stenosis and obstruction (CVD) is a serious and prevalent challenge to both resolve the venous hypertension symptoms and maintain the pantency of the ipsilateral hemodialysis access in hemodialysis patients. This study aimed to summarize our experience of the endovascular management of the central venous stenosis or obstruction in hemodialysis patients.
METHODS: Twenty-four haemodialysis cases of central vein stenosis or obstruction with ipsilateral functional vascular access in our hospital between July 2006 and February 2012 were treated by interventional therapy
