[Clinical case of the month. Treatment of obstructive hypertrophic cardiomyopathy by percutaneous septal ablation].

Hypertrophic obstructive cardiomyopathy is a rare pathology characterized by the presence of an interventricular septal hypertrophy leading to an ejectional obstruction of the left ventricular outflow tract. Several therapeutic approaches have been proposed. Besides pharmacological and surgical (septal myomectomy or cardiac pacing) therapies, a more recent therapy is represented by a septal reduction obtained by percutaneous occlusion of the septal coronary branch supplying this myocardial segment. A clinical case leads to a review of the literature.

Successful Balloon Valvuloplasty in a Case of Neonatal Aortic Critical Stenosis With Bicuspid Aortic Valve (BAV): A Case Report.

A bicuspid aortic valve (BAV) refers to a condition in which the valve has two cusps rather than three. Usually ignored, this manifests itself later in life. This bicuspid valve may manifest earlier in children with significant aortic stenosis because they have a severe left ventricular outflow tract restriction that worsens over time. This syndrome commonly results in congestive heart failure in newborns and early neonatal life. There can be a small amount of significant risk in pediatric patients having this BAV causing stenosis and being one of the reasons for sudden, unexpected death. Morp

Three-dimensional echocardiography can simulate intraoperative visualization of congenitally malformed hearts.

BACKGROUND: The new technique of three-dimensional echocardiography can display the studied anatomy in any desired view plane. We sought to establish whether the reconstructions produced could provide views of the heart comparable with those obtained by the surgeon in the operating room. METHODS: Ninety-four patients, aged 1 day to 19 years (mean, 4.3 years), were examined. The ultrasound probe was placed either on the chest or subcostally and acquired 80 to 100 perpendicular parallel images of the heart with electrocardiographic and respiratory gating. Any plane, in particular oblique planes

Anomalous anterior papillary muscle as an autopsy finding in two cases.

Anomaly in the anterior papillary muscle (APM) is known to cause left ventricle outflow tract (LVOT) obstruction, and this rare congenital condition could be a cause of sudden cardiac death. This anomaly and its hemodynamic effects is similar to valvular heart disease. In our two described cases, in which cause of death was, respectively, hypertrophic cardiomyopathy and suicide by hanging, the cephalad portion of the left APM was inserted directly into the ventricular surface of the anterior mitral leaflet and chordae tendineae were absent in the area of the direct anomalous muscle insertion;

Echocardiography Guided Liwen Procedure™ for the treatment of obstructive hypertrophic cardiomyopathy in a patient with prior aortic valve replacement surgery: Liwen procedure for intra-myocardial radiofrequency ablation.

We successfully treated a patient who was diagnosed of having hypertrophic obstructive cardiomyopathy after the aortic valve replacement surgery for the concomitant aortic stenosis. We report this first in kind new procedure exclusively developed in our center, Liwen procedureTM (percutaneous intramyocardial septal radiofrequency ablation), for patients with left ventricular outflow tract obstruction in spite of maximal medical therapy. The procedure was performed under transthoracic echo guidance. We discussed the technical details, safety, and effectiveness with corresponding images. The pat

Unusual hypertrophic cardiomyopathy with severe right ventricular outflow tract obstruction and two septal aneurysms: a case report.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is characterized by left ventricular wall thickening, while right ventricular involvement in HCM is also common and has been identified to relate to worse prognosis. Right ventricular outflow tract (RVOT) obstruction is a rare clinical disorder with diverse etiologies, among which HCM is one of the most prevalent specific causes. Currently, severe RVOT obstruction caused by HCM remains rarely reported. Its phenotypic and genetic characteristics remains uncharacterized, warranting more attention. CASE PRESENTATION: This case presented a 33-year-old

Decreased prevalence of hypoplastic left heart syndrome in Malta.

Hypoplastic left heart syndrome (HLHS) is the most severe form of congenital heart disease (CHD), which until recently was considered to be fatal. However, surgical intervention is now possible using neonatal heart transplantation or three-stage palliative surgery (Norwood's staged operations). In Malta, during the period 1977-1994, there were four cases of classic HLHS, with an additional three cases of HLHS-like cases. The birth prevalence of classical HLHS was 0.04 per 1000 live births, whereas that of combined HLHS-like cases was 0.06 per 1000 live births for the same period. One neonate w

Echocardiographic assessment of obstructive lesions in atrioventricular septal defects.

OBJECTIVES: We sought to determine the accuracy of transthoracic echocardiography (TTE) in identifying risk factors in patients with an atrioventricular septal defect (AVSD). BACKGROUND: Atrioventricular septal defect is a common lesion, and many decisions about it are based on echocardiography alone. The identification of associated left-sided inflow and outflow obstructive lesions is important, as they are responsible for mortality and morbidity. METHODS: Between 1983 to 1998, 549 patients with AVSD underwent repair. The TTE findings were correlated with surgery, angiocardiography, autopsy

Results of operation for coexistent obstructive hypertrophic cardiomyopathy and coronary artery disease.

This study describes the operative management and outcome of 28 patients with obstructive hypertrophic cardiomyopathy and hemodynamically significant coronary artery disease. Each patient underwent coronary artery bypass grafting and concomitant left ventricular myotomy-myectomy or mitral valve replacement. The mean age at operation was 59 years (range 42 to 74). Five patients (18%) died as a result of operation, four in the immediate postoperative period and one at 2 months postoperatively. Three patients died after the immediate postoperative period of causes unrelated to the operation. The

Interventional management of an unusual cause of cyanosis in repaired tetralogy of Fallot.

Arterial desaturation following surgical repair of tetralogy of Fallot (TOF) is rare. In most instances, it results from residual right ventricular outflow tract obstruction, causing right-to-left shunt across residual interatrial or interventricular communication. In this report, we present an unusual scenario of arterial desaturation due to a recanalized left cardinal vein in a child with repaired TOF. We also discuss stepwise evaluation that led to successful identification and occlusion of the abnormal venous channel by percutaneous device closure.

Cardiogenic Shock in Obstructive Hypertrophic Cardiomyopathy Plus Apical Ballooning: Management With VA-ECMO and Myectomy.

A patient with known obstructive hypertrophic cardiomyopathy developed worsening left ventricular outflow tract obstruction, severe mitral regurgitation, and apical ballooning leading to cardiogenic shock, a combination in which treatment of each component could worsen the others. Emergency veno-arterial extracorporeal membrane oxygenation, levosimendan, and noradrenaline transiently restored adequate systemic perfusion and gas exchange. Surgical myectomy offered a more definitive solution. (Level of Difficulty: Intermediate.).

Results of operation for obstructive hypertrophic cardiomyopathy in the elderly. Septal myotomy and myectomy in 20 patients 65 years of age or older.

The results of operative treatment in 20 patients with hypertrophic cardiomyopathy 65 years of age or older are described. All 20 patients were severely symptomatic before operation. Seventeen of the 20 patients had marked left ventricular outflow tract obstruction under basal conditions (average pressure gradient 107 mm Hg); the other three patients had outflow gradients of more than 50 mm Hg only with provocation. One patient died at operation, and one died 4 years after operation of circumstances unrelated to hypertrophic cardiomyopathy. Sixteen of the 18 long-term survivors have had persis

Mitral regurgitation due to ruptured chordae tendineae in patients with hypertrophic obstructive cardiomyopathy.

Mitral valve regurgitation in association with hypertrophic obstructive cardiomyopathy is usually caused by the systolic anterior motion of the anterior mitral leaflet. Recently, five patients were encountered with hypertrophic obstructive cardiomyopathy who had mitral regurgitation due to ruptured chordae tendineae. The diagnosis was confirmed in all patients during operation for left ventricular septal myectomy-myotomy (Morrow procedure). Preoperative identification of ruptured chordae tendineae as the cause of mitral regurgitation was established by transesophageal echocardiography in the t

Successful pregnancy and delivery in a young-onset hypertrophic cardiomyopathy patient with a novel doublet-base substitution in the MYH7 gene.

UNLABELLED: Herein, we report the case of a young-onset female hypertrophic cardiomyopathy (HCM) patient with severe left ventricular outflow tract obstruction who had a family history of premature sudden cardiac death. Despite the severe HCM phenotype, the patient was successfully managed by low-dose bisoprolol during two peripartum periods after a percutaneous transluminal septal myocardial ablation. We performed whole-genome sequencing analysis and found a novel doublet-base substitution (DBS) in the MYH7 gene (c.2608_2609delinsTT, p.R870F) in this patient. Her children were also genetically

Surgical implantation of balloon-expandable heart valves for the treatment of mitral annular calcification.

OBJECTIVES: The treatment of mitral valve disease in the presence of mitral annular calcification (MAC) is associated with an increased risk of cardiovascular and all-cause mortality. Various surgical and transcatheter techniques for the treatment of mitral disease with severe MAC have been described. However, these procedures are associated with high risk of operative morbidity and mortality. We describe our experience with open surgical implantation of a balloon-expandable valve (BEV) in patients with severe MAC as an alternative approach. METHODS: BEV implantation was performed with direct

Association between the 4p16 genomic locus and different types of congenital heart disease: results from adult survivors in the UK Biobank.

Congenital heart disease is the most common birth defect in newborns and the leading cause of death in infancy, affecting nearly 1% of live births. A locus in chromosome 4p16, adjacent to MSX1 and STX18, has been associated with atrial septal defects (ASD) in multiple European and Chinese cohorts. Here, genotyping data from the UK Biobank was used to test for associations between this locus and congenital heart disease in adult survivors of left ventricular outflow tract obstruction (n = 164) and ASD (n = 223), with a control sample of 332,788 individuals, and a meta-analysis of the new and ex

Critical aortic stenosis in the neonate: a multi-institutional study of management, outcomes, and risk factors. Congenital Heart Surgeons Society.

OBJECTIVES: We sought to determine factors that would predict whether a biventricular repair or Norwood procedure pathway would give the best survival in neonates with critical aortic stenosis. METHODS: Survival and risk factors were determined with parametric time-dependent event analysis for patients undergoing either the Norwood procedure or biventricular repair, and predicted optimal pathway and survival benefit were derived from multivariable linear regression. RESULTS: From 1994 to 2000, 320 neonates with critical left ventricular outflow obstruction were entered into a prospective mul

Modified Konno-Rastan procedure for subaortic stenosis: indications, operative techniques, and results.

BACKGROUND: Diffuse or unresectable subaortic stenosis (SAS) necessitates an aggressive surgical approach for the elimination of left ventricular outflow tract obstruction. In this article we report our experience with the modified Konno-Rastan procedure, with inherent preservation of the native aortic valve and annulus, in the treatment of diffuse or unresectable SAS. METHODS: Sixteen children (age range, 21 months to 18 years) underwent the modified Konno-Rastan procedure through either a transventricular (n = 12) or a transatrial approach (n = 4) to the conal septum.
