[Factors predisposing to cervical artery dissection].

INTRODUCTION: Cervical artery dissection is a major cause of cerebral ischemia in young adults and can lead to various clinical symptoms, some of which are benign (e.g. headache, neck pain, Horner's syndrome, and cranial nerve palsy). However, most patients have a stroke or transient ischemic attack. EPIDEMIOLOGY: The incidence of cervical artery dissection is low and is estimated to be around 2.6 per 100,000 inhabitants. Mortality and short-term recurrence rates are low: however; they may have been underestimated. PATHOPHYSIOLOGY: Although the pathophysiology is still unclear constitutional

Cervical arteries tortuosity and its association with dissection: A systematic review and meta-analysis.

BACKGROUND: This study investigated the link between arterial tortuosity and cervical artery dissection, focusing on carotid and vertebral tortuosity indices, as well as carotid tortuosity classifications (kinking, looping, and coiling). METHODS: We searched PubMed, SCOPUS, Web of Science, and Google Scholar from database inception to January 2024. The inclusion criteria encompassed human studies on tortuosity and cervical, carotid, or vertebral artery dissection. Exclusion criteria included case reports, non-English studies, and studies solely on connective tissue disorders and diseases. Qua…

Intracranial Extension of Extracranial Vertebral Dissection Is Associated With an Increased Risk of Ischemic Events.

Background and Purpose- Intracranial artery dissection can eventually lead to subarachnoid or intracerebral hemorrhage. Little is known about the clinical features and risks associated with extracranial vertebral artery dissection that extends intracranially. The clinical and imaging characteristics of extracranial vertebral artery dissection (eVAD) with (e+iVAD) or without (eVAD) intracranial extension were analyzed. Methods- The frequency of ischemic events, including ischemic strokes and transient ischemic attacks, was compared between e+iVAD and eVAD patients from a monocentric cohort stud…

Cervical artery dissection. A comparison of highly dynamic mechanisms: manipulation versus motor vehicle collision.

OBJECTIVE: To examine the similarities and dissimilarities between cervical chiropractic manipulative therapy and whiplash, and their respective relation to cervical artery dissection. DATA SOURCES: A literature synthesis used MEDLINE-PubMed and MANTIS literature searches. A total list of 99 relevant articles was generated. Additional references were collected from citations incorporated within the included articles. RESULTS: Both neck manipulation and motor vehicle collision events apply loads to the spinal column rapidly. While neck manipulation loads are slower to develop and displacement

A preschool-age child with first-time seizure and ataxia.

We present a case of a 4-year-old previously healthy child who had a possible first-time seizure at home, and upon a second Emergency Department evaluation was found to have gross cerebellar ataxia suggestive of acute stroke. Initial computed tomography scan and metabolic work-up were unrevealing. Subsequent neuroimaging demonstrated stroke in the left medulla and cerebellum secondary to left vertebral artery dissection. Cervical artery dissection may cause up to 20% of strokes in childhood and adolescence. Unlike typical adult presentations, antecedent or concurrent head and neck pain occurs …

Is it time to stop functional pre-manipulation testing of the cervical spine?

The combined extended and rotated cervical spine position has been postulated to affect vertebral artery blood flow by primarily causing a n … patient thoroughly, through careful history taking and physical examination, for the possibility of vertebral artery dissection. It is important to note that vertebral artery dissection (VAD) may present as pain only, and may not be associated with symptoms and signs of brainstem ischaemia. (2) If there is a strong likelihood of VAD, provocative pre-manipulation tests should …

The efficacy of coil embolization at the dissection site of ruptured dissecting vertebral aneurysms.

Proximal occlusion of the parent artery has been widely used for treatment of vertebral dissecting ruptured aneurysms, but this does not always completely prevent re-rupture. In this series, the efficacy of occlusion at the dissection site using detachable coils was compared with proximal balloon occlusion. Over a five year period, 25 patients suffering from subarachnoid hemorrhage with dissecting vertebral aneurysms were treated by endovascular surgery. The first three of these 25 patients were treated with proximal balloon occlusion of the parent artery. The remainder underwent platinum coil

Spontaneous cervicocephalic arterial dissection with headache and neck pain as the only symptom.

BACKGROUND AND OBJECTIVE: Cervicocephalic arterial dissection can cause both ischemic stroke and hemorrhagic stroke. However, spontaneous ce … the patients (3 males, 4 females) ranged from 35 to 79 (mean, 51.0 ± 16.2) years. Six patients had vertebral artery dissection, one had internal carotid artery dissection, and one had an association of vertebral and internal carotid artery dissection. With the exception of one patient, the headache and neck pain were unilateral. All patients with vertebral artery dissection …

Occipital artery-to-PICA bypass: how I do it.

BACKGROUND: Posterior inferior cerebellar artery (PICA) aneurysms are uncommon, and their management is challenging because of the complex angioarchitecture of the PICA and the frequently nonsaccular aneurysm presentation. Endovascular therapy may not be feasible. METHODS: We describe our technique of clip trapping with occipital artery (OA)-to-PICA bypass to treat a PICA aneurysm. Because the aneurysm affected the ipsilateral, dominant PICA, an OA-PICA bypass was chosen to ensure adequate flow and reduce risk to the contralateral PICA supply. CONCLUSION: The OA-PICA anastomosis is a safe an

[Spontaneous dissection of the subclavian artery and its branches].

A 51-year-old woman suffering from vertigo presented to our cerebrovascular laboratory. MRI revealed a right-sided cerebellar infarction. On ultrasound examination, we found a dissection with wall hematoma of the right subclavian artery extending to the first segment of the vertebral artery and to the thyrocervical trunc, as well as a dissection of the left vertebral artery in the cranio-cervical junction. On follow-up evaluation, all stenoses resulting from dissection were recanalized within 2 months and findings of wall hematoma disappeared completely within 7 months. There was no evidence o

Vertebral Artery Pseudoaneurysm in a Single-Vessel Posterior Circulation With Chronic Contralateral Occlusion and Recent Methamphetamine Use.

Bilateral vertebral artery disease presents a critical risk for posterior circulation stroke due to impaired collateral flow. This report details an adult presenting with acute neck pain and visual disturbances. Computed tomography (CT) angiography revealed left vertebral artery dissection at the C2 level with a 0.5 cm dissecting pseudoaneurysm and complete occlusion of the right vertebral artery. Magnetic resonance imaging (MRI) demonstrated no acute infarction. Symptoms improved with conservative management, including antiplatelet therapy. The case highlights the importance of recognizing hi…

Congenital vertebral duplication: a predisposing risk factor for dissection.

Spontaneous cervical-artery dissection (CAD) is a major cause of cerebral ischaemia in young adults. While their pathophysiology remains still poorly understood, CAD is considered today as a multifactorial disease determined by general and local predisposing factors; these predisposing factors being predominantly constitutional abnormalities of the arterial wall. Here, we report an MRI-confirmed dissection of a duplicated vertebral artery, in a patient with attacks of vertigo due to cerebellar infarction following a minor neck trauma. Knowing that another case of vertebrobasilar CAD on a dupli
