Prevalence, incidence, and etiology of epilepsies in rural Honduras: the Salamá Study.

PURPOSE: Determination of epilepsy etiology in population-based studies is difficult because of the high cost of diagnostic tests. However, cost-effectiveness may be proven if preventive public-health strategies can be established from the test results. We report an epilepsy population-based study using clinical and laboratory techniques. METHODS: A medical team administered an epilepsy survey to 88% of the residents by census in the rural county of Salamá, Honduras. Ninety of 100 participants identified with active epilepsy underwent a neurologic examination, video-electroencephalography (vi

Determinants of weak femoral artery pulse in dogs with mitral valve prolapse.

In three substudies encompassing 247 dogs from two breeds predisposed to myxomatous mitral valve disease (MMVD), femoral artery pulse strength was palpated and related to potential explanatory factors, including quantitative echocardiographic measures of MMVD, aortic and femoral artery diameter and wall thickness and blood pressure. In addition, in 109 Cavalier King Charles Spaniels (of which 61 were included in the three substudies mentioned above), the relation between femoral artery pulse strength and presence of thrombocytopenia was investigated. In 26% of the dogs, a pulse </=50% of norma

The self-fulfilling prophecy as an intrafamily dynamic.

This research examined whether parents' and children's perceptions of one another have reciprocal self-fulfilling effects on each other's behavior. Parents and their adolescent children completed self-report surveys and engaged in dyadic videotaped interaction tasks. The surveys assessed mothers', fathers', and children's perceptions of their own and the other's hostility and warmth. Observers coded the videotaped interactions to assess the actual hostility and warmth exhibited by parents and children. Data from 658 mother-child dyads were consistent with the conclusion that children had a sel

Is listener comfort a viable construct in stuttering research?

This article reports the development of a tool for measuring how comfortable a person feels when communicating with someone who has undergone treatment for stuttering. The person rates the speaker on a 9-point Listener Comfort Scale (9 = extremely comfortable, 1 = extremely uncomfortable). In a preliminary investigation of the reliability and validity of the scale, 15 unsophisticated listeners rated video recordings of 10 adults before and after a prolonged-speech treatment for stuttering and of 10 matched controls. The results were compared with those of another 15 listeners who rated the sam

Xerostomia, xerophthalmia, and plasmacytic infiltrates of the salivary glands (Sjögren's-like syndrome) in a cat.

A 2.5-year-old domestic shorthair cat was evaluated because of dysphagia and weight loss of 4 weeks' duration. MIld blepharospasm and conjunctival hyperemia were evident in both eyes, oral mucous membranes were tacky on palpation, and salivary glands were enlarged. Results of a Schirmer tear test were 0 mm/min for both eyes. Administration of atropine did not cause salivation or caused secretion fo thick rope-like saliva. Examination of biopsy specimens of salivary glands revealed a plasmacytic infiltrate. Sjögren's syndrome (SS) was diagnosed. Oral administration of prednisone was instituted

Ideomotor apraxia, visuomotor control and the explicit representation of posture.

Ideomotor apraxia is normally viewed as a disorder of the representation or execution of action. However, the evidence from gesture imitation is that apraxic patients are unable to reproduce the final posture of a gesture but may not show abnormality in movement kinematics. This supports a hypothesis that impaired gesture imitation is due to a deficit in a conceptual representation of body posture rather than indicating a disorder specific to action control. The present study evaluated whether other aspects of apraxic behaviour might also be most consistent with a deficit in the representation

Basal release of endothelin-1 and the influence of the ETB receptor on single vessel hydraulic permeability.

BACKGROUND: Endothelin-1 (ET-1) has a direct permeability decreasing effect on the microvasculature. The present study was designed to test the hypothesis that this effect is mediated via the endothelin B (ETB) receptor located on the microvascular endothelium and to determine whether basal microvascular permeability is dependent on constitutive release of ET-1. To isolate the direct effect of ET-1, experiments were conducted under conditions in which hydraulic and oncotic pressures were controlled. METHODS: Postcapillary venules in the rat mesentery were perfused in situ, and paired measurem

Reliability between two observers using a protocol for diagnosing essential tremor.

Protocols with demonstrated reliability have been established for the diagnosis of numerous movement disorders. whereas in the essential tremor (ET) literature, there is no discussion about the reliability of diagnostic protocols. Lack of knowledge of the reliability of diagnostic protocols in ET limits the use of these protocols because reliability is an essential requirement for scientific quality in data management. The objective of this study was to determine the reliability of a protocol for diagnosing ET. The protocol consists of a Tremor Interview, a videotaped Tremor Examination, and a

Semiology of temporal lobe seizures: value in lateralizing the seizure focus.

PURPOSE: To determine the lateralizing value of the clinical manifestations of seizures in patients with temporal lobe epilepsy (TLE), we made a retrospective videotape analysis of complex partial seizures (CPS) in 55 patients who underwent temporal lobectomy and were seizure-free postoperatively for >2 years. METHODS: Blinded to clinical details, we reviewed videotapes from video-EEG telemetry monitoring with attention paid to seizure semiology. RESULTS: Useful lateralizing features included unilateral clonic activity (with the seizure focus contralateral in all patients), unilateral dyston

Quantitative videostroboscopic measurement of glottal gap and vocal function: an analysis of thyroplasty type I.

The goal of surgical medialization of the vocal fold is to attain complete glottic closure. The purpose of this study is to quantify the glottal gap and to examine the relationship between glottal gap and vocal function perioperatively in thyroplasty type I. Glottal gap area was measured in 20 patients at the point of maximum closure of vocal fold vibration in digitized laryngeal stroboscopic images and was normalized by the square of vocal fold length. Glottal gap area thus measured was correlated with results obtained from well-accepted acoustic, aerodynamic, and perceptual measures of vocal

Vital confocal microscopy in bone.

We wished to exploit confocal microscopy for high spatial and temporal resolution vital microscopy in bone. To this end, we evolved implants with glass windows supported in titanium, which were placed in the medial proximal tibial plateau of the rabbit, and special small, self-focussing objectives (dry 10/0.25, water immersion 20/0.45, and oil immersion 45/0.65 and 120/1.0) which mated and matched to the conical window entrance section of the metal components. At intervals of up to 21 months after implant healing, these lenses were used to study live tissue using two genera of confocal microsc

Positioning of the tibial tunnel for anterior cruciate ligament reconstruction.

Two mechanisms of unintentional anterior tibial tunnel axis shift can occur despite accurate placement of the guide wire within the proximal tibia. The first results from using a short-block reamer head joined to a shaft of smaller diameter. If the tibial tunnel is drilled obliquely, it is possible for the reamer head to displace anteriorly in the knee joint before completion of the posterior portion of the tibial tunnel. The second mechanism of anterior shift involves using two sequential drills to create the tibial tunnel. To delineate the causes of this unwanted shift, cadaveric studies and

Swallowing dysfunction and aspiration in neonates and infants.

In children, swallowing dysfunction and aspiration are common causes of recurrent pneumonia and can be fatal. The underlying mechanism is still unknown. In this study, we evaluated the swallowing function in 72 neonates and infants by fluoroscopy and followed the course of 39 patients for more than one year. The results of all the examinations were recorded on videotape or a digital imaging system. All 10 patients with neurologic disorder presented cricopharyngeal dysfunction. Of the 12 patients with near miss sudden infant death syndrome, 10 showed nasopharyngeal reflux, and 5 demonstrated cr

Surgical variables affecting postoperative swallowing efficiency in oral cancer patients: a pilot study.

This study examined the correlation between swallow function at 3 months postoperatively and surgical variables including volume resected, flap volume, ratio of flap volume to volume resected, percentage of oral tongue, tongue base, and anterior and lateral floor of mouth resected, and whether or not the mandible was preserved in 30 surgically treated oral cancer patients. Swallows of measured amounts of liquid and paste (pudding) materials were examined videofluoroscopically. Nine measures of swallow function were completed for each swallow. A factor analysis of all swallow variables was done

The effect of video interventions on improving knowledge and treatment compliance in the sexually transmitted disease clinic setting. Lesson for HIV health education.

Eight intervention studies investigating patient education and treatment adherence in the sexually transmitted disease (STD) clinic setting are reviewed. Across the eight studies selected for analysis, meta-analytic procedures were applied to compare the impact of educational and prevention approaches. The effect of video was compared with that of other modes of health education on: 1) knowledge and attitudes about STDs and condoms and 2) treatment compliance, as measured by return for test of cure, drug compliance, premature resumption of sexual activity, and condom coupon redemption rates. Intravenous angiography using digital video subtraction: intravenous cervicocerebrovascular angiography.

The clinical application of intravenous angiography to study the cervicocerebrovascular system using the digital video subtraction system described in a companion article is reported. About 0.75 ml/kg of a standard 76% iodine contrast solution is injected into an antecubital vein using a power injector. Then 15-20 exposures of the head and neck region at a 1/sec rate are made on the image intensifier. The images are recorded by a high performance video system and the output signal is digitized for subsequent computer manipulation. The subtraction images of these vessels produced by the compute

Effect of Personalized Feedback Intervention (PFI) with Fear Appeal on Alcohol Consumption Pattern in Patients with Alcohol-induced Delirium: Protocol of an Open-label Randomized Controlled Trial.

BACKGROUND: Alcohol dependence is prevalent, with significant morbidity and mortality linked to alcohol-induced delirium. A new but less studied strategy integrating personalized feedback intervention (PFI) with fear appeal has shown promise in modifying behavior. NOVELTY: There is limited evidence to suggest that using patients' own videotaped recordings in a delirious state can reduce relapse and enhance abstinence. Our study evaluates the combined effect of both PFI and fear appeal on reducing alcohol use and prolonging abstinence. OBJECTIVES: This study aims to evaluate the impact of PFI

The modified National Institutes of Health Stroke Scale: its time has come.

The National Institutes of Health Stroke Scale (NIHSS) is a well known, reliable and valid stroke deficit scale. The NIHSS is simple, quick, and has shown significant reliability in diverse groups, settings, and languages. The NIHSS also contains items with poor reliability and redundancy. Recent investigations (include assessing a new training DVD, analyzing webbased or videotape certifications, and testing foreign language versions) have further detailed reliability issues. Items recurrently shown to have poor reliability include Level of Consciousness, Facial Palsy, Limb Ataxia, and Dysarth

The Schedule for the Assessment of Drug-Induced Movement Disorders (SADIMoD): test-retest reliability and concurrent validity.

The SADIMoD is a newly developed instrument, consisting of a compilation of rating scales, to measure the severity of drug-induced movement disorders: dystonia, dyskinesia, Parkinsonism, akathisia, ataxia, and several types of tremors. The test-retest reliability of this scale and the concurrent validity with the Abnormal Involuntary Movement Scale (AIMS), the Simpson-Angus Scale (SEE) and the Barnes Akathisia Scale (BAS) was assessed in 31 patients [20 male/11 female; 57.1+/-16.5 yr (mean+/-S.D.)] with a variety of movement disorders by six teams of investigators. The teams were trained by me
