Vestibular dysfunction and compensation after removal of acoustic neuroma.

OBJECTIVE: To evaluate vestibular function after unilateral acoustic neuroma surgery via a retrosigmoid approach. METHODS: Thirty-eight patients were tested using caloric irrigation, static posturography, and the Dizziness Handicap Inventory (DHI) before, and one week to nine months after surgery. RESULTS: Twenty-six patients were categorized as a response group and 12 as a no-response group on the basis of preoperative caloric irrigation findings. The posturographic parameters and DHI scores at one week after surgery showed significant deterioration in the response group, but not in the no-

[Seeing sounds? The visualization of acoustic phenomena in heart diagnostics].

During the nineteenth century physiologists and clinicians developed several graphical recording systems for the mechanical registration of heart sounds. However, none of these replaced traditional methods of auscultation. The paper describes criticism of the aural sense as one of the driving forces behind the development of phonocardiography and analyses its variants from a technological and clinical perspective. Against the background of the physiological “method of curves,” the parameters that prevented the implementation of phonocardiography against overwhelming odds are highlighted. Conte

Comparisons of acoustic function in SCA31 and other forms of ataxias.

OBJECTIVE: To investigate whether acoustic impairment can be one of the characteristic extracerebellar symptoms in sporadic and hereditary ataxias including spinocerebellar ataxia type 31 (SCA31). METHODS: We investigated genotypes of dominant ataxia families, and determined a frequency of each form in our cohort of 154 families. Acoustic function in the groups of various forms of ataxia with multiple system atrophy of cerebellar predominance (MSA-C), cortical cerebellar atrophy (CCA), and hereditary ataxias including SCA31 was evaluated by using audiogram and brainstem auditory evoked potent

Location and acoustic scale cues in concurrent speech recognition.

Location and acoustic scale cues have both been shown to have an effect on the recognition of speech in multi-speaker environments. This study examines the interaction of these variables. Subjects were presented with concurrent triplets of syllables from a target voice and a distracting voice, and asked to recognize a specific target syllable. The task was made more or less difficult by changing (a) the location of the distracting speaker, (b) the scale difference between the two speakers, and/or (c) the relative level of the two speakers. Scale differences were produced by changing the vocal

Visualisation of hypopharyngeal cavities and vocal-tract acoustic modelling.

The hypopharyngeal cavities consist of the laryngeal cavity and bilateral piriform fossa, constituting the bottom part of the vocal tract near the larynx. Visualisation of these cavities with magnetic resonance imaging (MRI) techniques reveals that during speech, the laryngeal cavity takes the form of a long-neck flask and the piriform fossa takes the form of a goblet of varying shapes: the former diminishes greatly in whispering and the latter disappears during deep inhalation. These cavities have been shown to exert significant acoustic effects at higher frequency spectra. In this study, aco

Management of patients with acoustic neuromas: a Markov decision analysis.

OBJECTIVES/HYPOTHESIS: The management of patients with small (<1.5 cm) acoustic neuromas is controversial. Immediate treatment via microsurgical resection or radiosurgery is often advocated. A period of observation is sometimes advised followed by microsurgery or radiosurgery for tumors that demonstrate growth during the observation period. The purpose of this study is to calculate quality-adjusted life expectancy for the most commonly applied management strategies in hypothetical cohorts of patients of various ages. STUDY DESIGN: Markov decision analysis; societal perspective. METHODS: Assu

Acoustical screening for obstructive sleep apnea during wakefulness.

In this paper a new non-invasive method for screening patients with obstructive sleep apnea (OSA) during wakefulness is proposed. Eight people with OSA and eight non-apneic individuals participated in this study. The tracheal breath sound was recorded in supine and upright positions during both nose and mouth breathing maneuvers. Spectral analysis of the respiratory sound signals showed the variation in the average power of the sounds at different positions to be a characteristic feature discriminating the two groups. Using this feature, the OSA and non-apneic participants were classified by q

Neuronal circuits involved in the middle-ear acoustic reflex.

Human and animal studies have shown that certain aromatic solvents such as toluene can cause hearing loss and can exacerbate the effects of noise. The latter effects might be due to a modification of responses of motoneurons controlling the middle-ear acoustic reflex. In the present investigation, the audition of Long-Evans rats was evaluated by measuring cubic (2f1 - f2) distortion otoacoustic emissions (f1 = 8000 Hz; f2 = 9600 Hz; f1/f2 = 1.2) prior to, during, and after activation of the middle-ear acoustic reflex. A noise suppressor was used to modify the amplitude of the 2f1 - f2 distorti

Acoustic and neurophysiologic observations related to lingual nerve impairment.

The purpose of this study was to determine the acoustic effects of lingual nerve impairment on speech. Neurophysiologic examination and thermal quantitative sensory testing (QST) were carried out to determine if the profile, type or severity of sensory nerve impairment had effects on the degree of speech changes. The study group consisted of 5 women and 5 men with lingual nerve damage following an oral and maxillofacial surgery procedure. Time interval between the examination and the nerve damage ranged from 1 month to 20 years. Formants and fundamental frequency and duration of vowel sounds w

Correlating the cochleovestibular deficits with tumor size of acoustic neuroma.

CONCLUSION: The tumor size of acoustic neuroma correlates with cochleovestibular deficits. Those tumors with global frequency hearing loss, bilateral gaze nystagmus, or absent caloric and VEMP responses may indicate a tumor size >2.5 cm. OBJECTIVE: This study aimed to investigate the correlation between cochleovestibular deficits and the size of acoustic neuroma. PATIENTS AND METHODS: A total of 44 patients with acoustic neuroma were enrolled in this study. Pure tone audiometry, electronystagmography, caloric test, vestibular evoked myogenic potential (VEMP) test, and MRI were conducted. RE

Hydatid cysts of the internal acoustic canal and jugular foramen.

Brain involvement with hydatid disease occurs in 1-2% of all Echinococcus granulosus infections. Cerebral hydatid cysts are usually supratentorial, whereas infratentorial lesions are quite rare. Here we report a 19-year-old man with hydatid cysts in the right cerebellopontine cistern with the involvement of internal acoustic canal and jugular foramen. The patient presented with signs of increased intracranial pressure and multiple cranial nerve palsies. Surgery was performed in the semi-sitting position using a lateral suboccipital approach for a right-sided craniotomy. Magnetic resonance imag
