Panic disorder affects an estimated 1.4 percent of the general population. Nearly 70 percent of patients with panic disorder may also suffer from a major depressive disorder at some time in their lives. Both disorders respond to tricyclic antidepressants and monoamine oxidase inhibitors. Alprazolam is also an effective antipanic agent. Because avoidance behaviour (agoraphobia) often follows the onset of panic disorder, behavioral interventions are an important adjunct to management.

Anxiety is a problem for millions of Americans. It poses special challenges for women as they grow into advanced age. This paper provides a general overview of anxiety disorders, including panic disorder, agoraphobia, specific phobia, social phobia, obsessive compulsive disorder, and generalized anxiety disorder. Etiology, assessment and treatment strategies are then addressed. Special focus is directed at biological and psychosocial issues as they relate to older women in the development, experience, treatment and prevention of anxiety disorders.

Pharmacologic treatment of phobic disorders, in particular agoraphobia, has been proved to be effective, especially when used in combination with behavior therapy. Favorable responses to tricyclic antidepressants, monoamine oxidase inhibitors, and benzodiazepines have been reported. The limitations of drug therapy for phobias include high dropout rates and the tendency to relapse after discontinuation of therapy. Successful treatment depends on the careful selection of patients and medications and on coordination of drug and behavior therapies.

Psychogenic dyspnea may be part of two main clinical pictures. The first one is acute with dyspnea in the foreground of a panic disorder with or without agoraphobia. The second one is chronic, dyspnea being a background symptom of a generalized anxiety disorder. A dozen experimental studies have assessed the effectiveness of paradoxical interventions with agoraphobia, depression, insomnia and procrastination. The studies suggest that paradoxical interventions are more effective than no treatment and placebo treatment and are as effective and, in some instances, more effective than other behavioral interventions.
