Evaluation of sciatic nerve damage following intraneural injection of bupivacaine, levobupivacaine and lidocaine in rats.

OBJECTIVE: The local anesthetics may cause neurotoxicity. We aimed to compare the neurotoxic potential of different local anesthetics, local anesthetic induced nerve damage and pathological changes of a peripheral nerve. METHODS: Sixty Wistar rats weighing 200-350g were studied. Rats were assigned into 3 groups and 26-gauge needle was inserted under magnification into the left sciatic nerve and 0.2mL of 0.5% bupivacaine, 5% levobupivacaine, and 2% lidocaine were injected intraneurally. An individual who was blind to the specifics of the injection monitored the neurologic function on postopera…

Modification of endothelium-dependent relaxation by propofol, ketamine, and midazolam.

Since volatile anesthetics, barbiturates, and local anesthetics have been reported to inhibit endothelium-dependent relaxation, we hypothesized that any drug with anesthetic action would suppress this relaxation. In the present study, using rat thoracic aortae, we attempted to determine whether nonbarbiturate intravenous anesthetics, including midazolam, propofol, and ketamine, suppress endothelium-dependent relaxation, and to clarify the mechanism(s) involved. Acetylcholine-induced, endothelium-dependent relaxation was significantly attenuated by propofol and ketamine, but was unaffected by m

Editorial Commentary: Addition of Dexmedetomidine to Local Analgesia for Knee Arthroscopy.

The search for the ideal pain management strategy after knee arthroscopy continues. For patients unable to receive regional anesthesia, peri-articular or intra-articular injections of local anesthetics with other medications offer a promising solution. Dexmedetomidine (a short-term sedative analgesic marketed under the names Dexdor and Precedex), when added to local anesthetics, may offer an increase in the length of time between surgery and the need for a rescue analgesic agent. Whether the addition of dexmedetomidine results in lower pain scores or decreased opioid consumption remains to be

[Similarity of Clinically Significant Neuropsychiatric Adverse Reactions Listed in Package Inserts between the Anti-influenza Drugs Oseltamivir and Amantadine (Possibility Attributable to Common Pharmacological Effects)].

The anti-influenza virus drug oseltamivir has been reported to have several pharmacological actions including blocking of nicotinic acetylcholine receptor channels and activation of the dopaminergic system. These pharmacological actions highly overlap those of amantadine, another anti-influenza virus drug authorized in Japan, and ester-type local anesthetics. Moreover, oseltamivir and amantadine can clinically induce similar adverse neuropsychiatric reactions. In the present study, from the database of the Pharmaceuticals and Medical Devices Agency (PMDA), we surveyed 2576 drugs for which neur

Delayed-type hypersensitivity to lidocaine.

BACKGROUND: Lidocaine hydrochloride is the preferred anesthetic agent used in outpatient surgical procedures. While type I hypersensitivity reactions to lidocaine are uncommon, type IV hypersensitivity is reported even less frequently. OBSERVATIONS: Between January 1, 2001, and December 31, 2001, 183 patients were patch tested at the Penn State Milton S. Hershey Medical Center (Hershey, Pa) to the North American Contact Dermatitis Group tray. All patients who had a positive patch test reaction to lidocaine were challenged with 0.1 mL of preservative-free 1% lidocaine intradermally. Of the 183

[The efficacy of intravenous lidocaine on various types of neuropathic pain].

We examined the efficacy of systemic local anesthetics on various types of neuropathic pain in 89 patients. Lidocaine 1.5 mg.kg-1 was infused intravenously for one minute. Pain score (PS) by visual analogue scale (VAS, 0-10) was measured 1, 5, 15 and 35 min after the infusion. The efficacy of intravenous lidocaine was evaluated by PS which was lowest after infusion. PS decreased to less than 50 percent of pre-infusion value in more than 75 percent of cases of cancer pain, postherpetic neuralgia, trigeminal neuralgia, low back pain with signs of root pain or spinal canal stenosis, peripheral ne

[Case report: bilateral femoral and sciatic regional anesthesia in a polytraumatized patient].

PURPOSE: To present a case requiring regional anesthesia for both lower limbs, and to highlight the considerations to avoid potential local anesthetic toxicity, as a result of high cumulative doses of local anesthetic in this setting. CLINICAL FEATURES: A 32-yr-old, obese (body mass index = 30.4 kg.m(-2)) woman required urgent, open reduction of bilateral ankle fractures. She also had facial, odontoid and pelvic fractures, and, in view of full stomach considerations, the anesthetic plan was to use regional anesthesia. Bilateral femoral and sciatic nerve blocks were performed, using a combinat

Anesthesia in the emergency setting: Part I. Hand and foot injuries.

Topical application and local infiltration of anesthetic agents are useful for localized areas, while a circular field block provides superficial anesthesia within the encircled area. Regional anesthesia is accomplished by injecting the anesthetic agent near a nerve or nerve group. Useful anesthetic techniques for repair of hand injuries include wrist blocks, intermetacarpal blocks and local infiltration anesthesia. Median nerve block, ulnar nerve block, radial nerve block or a combination of these blocks provides satisfactory anesthesia without inducing muscle paralysis in the anesthetized re

Adjuvant Drugs for Peripheral Nerve Blocks: The Role of Alpha-2 Agonists, Dexamethasone, Midazolam, and Non-steroidal Anti-inflammatory Drugs.

Adjuvant drugs for peripheral nerve blocks are a promising solution to acute postoperative pain and the transition to chronic pain treatment. Peripheral nerve blocks (PNB) are used in the brachial plexus, lumbar plexus, femoral nerve, sciatic nerve, and many other anatomic locations for site-specific pain relief. However, the duration of action of a PNB is limited without an adjuvant drug. The use of non-opioid adjuvant drugs for single-shot peripheral nerve blocks (sPNB), such as alpha-2 agonists, dexamethasone, midazolam, and non-steroidal anti-inflammatory drugs, can extend the duration of

[New aspects of common antagonists used in the perioperative period: preface and comments].

Anesthesia is a state made by multiple pharmacological agents that affect the functions of central, peripheral and autonomic nervous systems. Antagonists are often used to reverse the effects of anesthetic agents, muscle relaxants, and so forth. Antagonists often have side effects other than their specific antagonistic effect. Some side effects may be clinically useful. Ketamine was found to be a NMDA receptor antagonist. Ketamine potentially has neuroprotective, and anti-tumor effect. Local anesthetic agent also has been known to have anti-NMDA receptor effect. Although the mechanisms of loca

[3H]Batrachotoxinin A 20-alpha-benzoate binding to sodium channels in rat brain: characterization and pharmacological significance.

Attempts were made to find whether [3H]batrachotoxinin A 20-alpha-benzoate provides a specific probe for measuring interactions of local anesthetics with the sodium channel complex. [3H]Batrachotoxinin A 20-alpha-benzoate binding, [14C]guanidine and 22Na uptake were investigated in rat brain crude synaptosomal preparations and the potencies of drugs belonging to various chemical and pharmacological classes were compared. The results show that [14C]guanidine uptake seems to be a good model for measuring Na+ fluxes. An allosteric interaction between site 2 and 3 of the Na+ channel is apparent si

Pumiliotoxin alkaloids: a new class of sodium channel agents.

Pumiliotoxin B (PTX-B) and a variety of congeneric alkaloids and synthetic analogs stimulated sodium flux and phosphoinositide breakdown in guinea pig cerebral cortical synaptoneurosomes. The effects of PTX-B and active congeners and analogs on sodium flux in synaptoneurosomes were potentiated markedly by scorpion venom (Leiurus quinquestriatus). In neuroblastoma cells, PTX-B and active congeners had no effect on sodium flux unless synergized by alpha-scorpion toxin or scorpion venom. Certain inactive congeners, lacking hydroxyl groups in the 6-alkylidene side chain, inhibited sodium flux elic

Ophthalmologic complications after intraoral local anesthesia: case report and review of literature.

Intraoral administration of local anesthetics is one of the most common dental procedures. Ophthalmologic complications can occur after maxillary as well as mandibular local anesthetic injections and may be underreported and sometimes misinterpreted. A review of the literature from the years 1936-2011 shows 131 cases with this type of complication. This case report presents a patient with right lateral rectus muscle palsy and blurred vision after bimaxillary anesthesia. Signs, symptoms, and pathophysiologic hypotheses of these complications are presented.

Neuromuscular blockers.

The use of corrosive, injectable neuromuscular blockers has been a treatment option for many years; however, the more recent advent of botulinum toxin (BTX) treatment has revived interest in localized treatments. This article reviews the use of local anesthetics, alcohol, phenol, and BTX treatment for localized muscular overactivity syndromes.

Therapeutic aspects of mucociliary clearance.

Measurements of mucociliary activity are still very approximate. It is therefore difficult to determine the particular effect of certain drugs on patients and on experimental animals. Adrenergic drugs increase ciliary efficiency; local anesthetics, barbiturates and narcotics strongly depress ciliary action. Further research is required on mucociliary clearance mechanisms.

Hypersensitivity to procaine, tetracaine, mepivacaine, and methylparaben: report of a case.

The patient who comes for dental treatment with a history of hypersensitivity to local anesthetics is often denied the benefits of adequate pain control during dental therapy. Ninety percent of the patients challenged by Incaudo and associates were being evaluated because they needed local anesthesia for dental treatment. These same patients may some day need local anesthetic agents for minor surgery or for the treatment of cardiac arrhythmias. For those cases in which an allergic reaction is suspected, consultation with an allergist may prove beneficial to the patient by providing information

An Updated Systematic Review and Meta-Analysis of Fluoroscopic Caudal, Interlaminar, and Transforaminal Epidural Injections in Lumbar Disc Herniation.

BACKGROUND: Epidural injections are among the most frequently used interventions for management of low back and lower extremity pain. These injections are administered through caudal, interlaminar, and transforaminal approaches. Despite widespread use, controversy persists regarding effectiveness, appropriateness, medical necessity, and clinical indications. Although numerous studies have evaluated efficacy, there has been limited advancement in high-quality randomized evidence. In addition, multiple systematic reviews have repeatedly analyzed the same clinical trials, resulting in an evidence
