Surgical treatment of locally advanced anal cancer after male-to-female sex reassignment surgery.

We present a case of a transsexual patient who underwent a partial pelvectomy and genital reconstruction for anal cancer after chemoradiation. This is the first case in literature reporting on the occurrence of anal cancer after male-to-female sex reassignment surgery. We describe the surgical approach presenting our technique to avoid postoperative complications and preserve the sexual reassignment.

[High incidence of non-AIDS-defined cancers among HIV-infected patients in Germany. A 3-year nationwide review].

OBJECTIVE: It was the main aim of this study to obtain data on the epidemiology of AIDS- and not AIDS-defined malignancies in HIV-positive persons, the results to provide an epidemiological overview and to be the basis for further research initiatives. Additionally it sought to gain an impression of the realities of treatment of patients with HIV-associated malignant tumors in Germany. PATIENTS AND METHODS: Over a period of 3 years (from the beginning of 2000 to the end of 2002) data were retrospectively collected on the incidence of malignant tumors in HIV-positive patients. A questionnaire

Physical and psychological effects of anogenital warts on female patients.

Symptoms and psychological reactions of 82 patients with anogenital warts were recorded in a descriptive study at the beginning and after treatment. A majority of the patients had substantial and longstanding symptoms like local pain, tenderness, and discharge. Anxieties about cancer and cure were common. With a mood adjective checklist, the patients were compared to 58 referents, matched for background variables. There were significant differences in experiences of pleasantness, activation, and social orientation, which suggest a conspicuous emotional effect of the disease. The clinical pictu

Bowenoid change in perianal condyloma acuminatum associated with ulcerative colitis.

A 35-year-old male patient with a 20-year history of ulcerative colitis developed an itching perianal condyloma acuminatum with discharge. This was surgically removed after two years. Histology revealed bowenoid change in the condyloma. Six months later a local recurrence was excised. He has now been free of further recurrence for 5 years. Electron microscopy showed moderately differentiated keratinocytes in the areas of bowenoid change. There was no evidence of virus particles in the keratinocytes. A causal relationship has been assumed between the ulcerative colitis, via chronic irritation,

Primary radiation therapy in the treatment of anal canal carcinoma.

A retrospective analysis was undertaken of 51 patients with primary anal canal carcinoma who were treated by radiation therapy, with surgery being reserved for those with residual carcinoma. The five-year uncorrected survival rate was 59 per cent, and the corrected survival rate was 71 per cent. The primary tumor was controlled by radiation alone in 29 of 51 patients (57 per cent) and by subsequent radical surgery in eight of ten patients. Abnormal lymph nodes were controlled by radiation alone in eight of 11 patients. Only three of 29 patients required surgery for complications associated wit

[Practical proctology (author's transl)].

Hemorrhoidal complaints are the most frequent diseases of the rectal region and form the starting point for practically all other diseases of the rectal area. Acute bleeding often first brings the patient to the doctor, who must establish the source of the hemorrhage with all diagnostic means available. Further stages of the disease are temporary or complete prolapse of the hemorrhoids, under certain circumstances even prolapse of the rectum. Anal cryptitis is the first member of a chain of inflammatory sequelae such as perianal abscess and fistular diseases. It is frequently overlooked, yet i

Analysing the implementation and transferability of an enhanced care pathway integrating systematic supportive cancer care and patient-partner support for people living with anal cancer: Protocol for a mixed-methods study in two French cancer centres.

BACKGROUND: Supportive cancer care (SCC) is essential for comprehensive oncology care. It addresses treatment-related adverse effects, reduces long-term sequelae and improves patients' quality of life. Patient-partner (PP) programmes positively impact psychological well-being, patient empowerment and supportive care. Nevertheless, access to SCC remains inconsistent, fragmented and unequally distributed, while PPs are rarely integrated into routine cancer care pathways. Anal cancer is a predominantly localised cancer with high survival rates. Treatment is associated with acute toxicity and ofte

Duration of Time CD4/CD8 Ratio is Below 0.5 is Associated with Progression to Anal Cancer in Patients with HIV and High-Grade Dysplasia.

BACKGROUND: A CD4/CD8 ratio < 0.5 is associated with increased risk of advanced anal disease (AAD) but it is unknown if duration below 0.5 matters. The purpose of this study was to determine if duration of a CD4/CD8 ratio < 0.5 is associated with increased risk of invasive anal cancer (IC) in people living with HIV and high-grade dysplasia (HSIL). METHODS: This single institution, retrospective study used the University of Wisconsin Hospital and Clinics Anal Dysplasia and Anal Cancer Database. Patients with IC versus HSIL alone were compared. Independent variables were mean and percentage of

High-grade anal intraepithelial neoplasia treated with endoscopic submucosal dissection: a case report.

Anal intraepithelial neoplasia is a premalignant lesion for anal squamous cell carcinoma. Current treatment options, consisting of topical therapy and local ablative procedures with electrocautery or radiofrequency ablation, are effective although recurrence rates are high. Experience with endoscopic submucosal dissection for anal lesions is limited, with only a few cases of anal intraepithelial neoplasia and early anal squamous cell carcinoma. We present a 65-year-old woman with high-grade anal intraepithelial neoplasia successfully removed by endoscopic submucosal dissection with no complica

Do immune inflammatory markers correlate with anal dysplasia and anal cancer risk in patients living with HIV?

PURPOSE: People living with HIV (PLWH) are at an elevated risk for developing anal cancer. As screening is invasive, markers predicting those at highest risk for anal cancer could guide individualized screening. Neutrophil lymphocyte ratio (NLR), platelet lymphocyte ratio (PLR), and prognostic nutritional index (PNI) are surrogate inflammatory/immune markers known to correlate with cancer outcomes. This study aims to assess whether these markers correlate with anal cancer risk in PLWH. METHODS: This is a retrospective single-institution cohort study of PLWH at a single academic medical center

Inflammatory cloacogenic polyp in an adolescent - case report and review of the literature.

Inflammatory cloacogenic polyp is a rare lesion arising in the anal transitional zone. It is usually benign, but rare cases of malignant transformation are known. It is most commonly seen in the adult population from the fourth to the sixth decade of life, but it can be found among children and adolescents as well. The most common clinical symptoms include rectal bleeding and altered bowel habits, although some patients may be asymptomatic. Treatment involves transanal endoscopic microsurgery followed by a bowel regimen with stool softeners. We present the case report of a 14-year-old boy pres

Our experience in the short-term diagnostic-therapeutic management of the patient with anal dysplasia.

INTRODUCTION: Anal intraepithelial neoplasia (AIN) is a premalignant lesion of anal squamous cell carcinoma. HIV-positive males who have sex with males, are the most affected at-risk population. Cytology and anuscopy are the best accepted methods for its diagnosis, although it is controversial which patients should complete it with a biopsy. Neither which patients should undergo treatment nor which is the best treatment is not well established. With this study, we would like to present our experience in the diagnostic-therapeutic management of AIN in the short term. METHODS: Retrospective obs

Interobserver agreement in the diagnosis of anal dysplasia: comparison between gastrointestinal and gynaecological pathologists and utility of consensus conferences.

AIMS: Management of anal dysplasia relies upon the accurate diagnosis of anal biopsy specimens. As institutions move towards subspecialty signout (SSSO), decisions must be made regarding whether to assign anal biopsies to the gastrointestinal (GI) or gynaecological (GYN) pathology service. MATERIALS AND RESULTS: We identified 200 archival tissue biopsies of anal mucosa and circulated them among three GI pathologists and three GYN pathologists. Each pathologist separately scored each biopsy as normal, atypical, low-grade squamous intra-epithelial lesion (LSIL) or high-grade squamous intra-epit

A Pattern of Care Report on the Management of Patients with Squamous Cell Carcinoma of the Anus-A Study by the Italian Association of Radiotherapy and Clinical Oncology (AIRO) Gastrointestinal Tumors Study Group.

Background and objectives: The diagnosis and therapy of squamous cell carcinoma of the anus may vary significantly in daily clinical practice, even if international guidelines are available. Materials and Methods: We conducted a pattern of care survey to assess the management of patients with anal cancer in Italy (38 questions). We analyzed 58 questionnaires. Results: Most of the respondents work in public and/or university hospitals (75.8%) in northern Italy (65.5%). The majority (88.0%) treat less than 20 patients/year. Common examinations for diagnosis and staging are anorectal endoscopy (8

Trends in incidence of anal cancer in Austria, 1983-2016.

BACKGROUND: Recent reports have noted increasing rates of anal cancer among high-income countries worldwide; however, little is known about these trends in Austria. METHODS: Data on anal cancer from 1983 to 2016 were obtained from Statistics Austria. All tumors (n = 3567) were classified into anal squamous cell carcinomas (ASCC), anal adenocarcinomas (AADC), and others (unspecified carcinoma and other specific carcinoma). Anal cancer incidence rates were calculated in 5‑year cycles and incidence average annual percentage change (AAPC) to evaluate trends by sex, histology and age group. RESUL

Elevated beta human chorionic gonadotropin in a non-pregnant female diagnosed with anal squamous cell carcinoma.

INTRODUCTION: Elevated serum beta human chorionic gonadotrophin (β-hCG) in a female normally indicates pregnancy or possibly, gestational trophoblastic disease or ovarian germ cell tumours. Expression of β-hCG has been demonstrated in cervical and endometrial carcinoma and other non-germ cell tumours of the ovary, vulva, breast, prostate, lung, colon, oral/facial tissue and stomach. CASE REPORT: We report a 43-year-old premenopausal woman with p16 positive squamous cell anal cancer. Pre-treatment urinary screening was positive for β-hCG (218 IU/L), which was confirmed on serum and expressed i
