A genetic approach to the aetiology of giant cell arteritis: depletion of the CD8+ T-lymphocyte subset in relatives of patients with polymyalgia rheumatica and arteritis temporalis.

OBJECTIVE: To investigate the CD8+ T-cell concentrations in first degree relatives of patients with giant cell arteritis (GCA) in order to evaluate whether the low CD8+ T-cell values found in most patients with GCA are acquired or hereditary. METHODS: Probands were 5 patients with arteritis temporalis (TA) and 15 with polymyalgia rheumatica (PMR). Forty of 60 available relatives participated in the study. They were all interviewed concerning signs of previous illness. A blood sample screening was performed, including IgM-RF and ANA on Hep-2 cells. Age- and sex-matched controls consisted of 29…

An Unusual Case of Renal-Limited Giant Cell Arteritis.

It is widely accepted that giant cell arteritis (GCA) is a disease of the carotid branches. There are very few cases describing disseminated disease or visceral involvement. Isolated renal GCA presenting with renal failure is even more unusual. A 76-year-old male with no known medical history presented to the emergency department with three weeks of weakness, swelling, oliguria, and shortness of breath. He was started on hemodialysis for renal failure. After microscopic urinalysis revealed red blood cell casts, a kidney biopsy was performed and showed granulomatous arteritis suggestive of GCA …

Severe aortic regurgitation revealing Takayasu arteritis: a case report.

BACKGROUND: Takayasu arteritis (TA) is a rare chronic autoimmune vasculitis affecting large and medium-sized arteries, mainly in young women, and is more common in Asia. It may cause stenosis, occlusions, aneurysms, aortic dilation, and aortic valve disease. We are reporting a rare case of severe aortic valve insufficiency and aortic dilation in the context of TA. CASE PRESENTATION: We report the case of a 39-year-old patient presenting with progressively worsening dyspnoea, associated with fatigue, weight loss, and febrile sensations. Physical examination revealed signs of heart failure (ort…

Reversible visual loss in biopsy-proven giant cell arteritis.

A late 60s man presented with a 1-week history of recurrent transient monocular visual loss in his left eye, followed by sudden, severe visu … yte glycoprotein and AQP4 antibodies, was unremarkable. Temporal artery biopsy confirmed giant cell arteritis. Intravenous methylprednisolone 1 g/day for 3 days, followed by high-dose oral prednisolone, led to substantial visual recovery, with best-corrected acuity improving to 20/63. This case highlights that even severe, evolving visual loss with segmenta …

Bilateral parotitis revealing giant cell arteritis: A case report.

We report the atypical case of giant cell arteritis (GCA) in an 88-year-old woman, whose primary symptom was parotitis. The patient presented with a three-week history of painful bilateral parotid enlargement and mild undated morning headaches that were relieved with acetaminophen, but she did not have classic GCA symptoms such as jaw claudication or scalp tenderness. Biological tests revealed biological inflammation. Parotid glands did not show any structural abnormalities. A Doppler ultrasound of the temporal arteries showed inflammation, and a positron emission tomography/computed tomograph…

Takayasu's Arteritis with Systemic Lupus Erythematosus: A Case Report.

Takayasu arteritis (TA) is a rare granulomatous vasculitis affecting the aorta and its major branches, while systemic lupus erythematosus (SLE) is a multisystem autoimmune disorder known for vascular and serological involvement. Although both diseases share certain immunopathological pathways, their co-occurrence is exceedingly rare. We report a case of a 44-year-old Indian female with a prior diagnosis of SLE who presented with breathlessness, palpitations, and nodular episcleritis. Cardiovascular evaluation revealed carotid bruit, diminished peripheral pulses, blood pressure discrepancies, a…

Neuro-ophthalmic Manifestations of Giant Cell Arteritis: A Review.

Giant cell arteritis (GCA) is the most common vasculitis of large and medium vessels affecting adults over the age of 50. Due to its acute ischemic damage through a systemic inflammatory process, GCA is a medical emergency with the risk of permanent vision loss. Therefore, early intervention is critical. Patients often present with well-documented systemic manifestations such as jaw claudication, headache, scalp tenderness, and fatigue. These patients are usually subject to blood tests for inflammatory markers and temporal artery biopsy. However, clinical manifestations vary considerably and m…

Eosinophilic giant cell arteritis: A different subset of disease?

OBJECTIVES: To describe the clinical findings, response to therapy and course of patients with transmural eosinophilic infiltration at temporal artery biopsy (TAB). METHODS: The study consisted of a retrospective cohort of 254 consecutive GCA patients with evidence of transmural inflammation at TAB seen at the Santa Maria Nuova Hospital over a 28-year period. The findings of the 22 patients with eosinophilic infiltration (≥ 20 eosinophils/hpf) at TAB were compared with those of 232 patients without. Among these 232 patients, we sampled 42 GCA patients matched for age, sex and follow-up durati

Posterior segment manifestations of Takayasu arteritis: A narrative review.

Ocular symptoms can be the presenting manifestation of Takayasu arteritis (TA) or could be indicative of disease reactivation. A review of published literature related to posterior segment manifestations of TA by using the keywords "Takayasu arteritis," "ophthalmic manifestations," "retina," "retinopathy," "ocular," "optic nerve," and "optic neuropathy" was performed. In total, 62 case reports and 12 case series were included. The majority of the articles were from Asia (n = 47, 64%). Females outnumbered males in the ratio of 7:1. The mean age of patients was 33 years (range: 8-78 years, SD: 1…

Acute heart failure due to myocarditis in Takayasu's arteritis.

Takayasu's arteritis (TA) is a granulomatous vasculitis that involves the aortic artery and its branches, resulting in stenosis, occlusion, and aneurysmal dilatation. Cardiovascular involvement is one of the main complications and a major cause of mortality in these patients. Herein, we describe the case of a woman with TA who presented with severe acute heart failure secondary to myocarditis and responded well to immunosuppressive therapy.

Difficult-to-treat Takayasu arteritis: a case-based review.

Takayasu arteritis is a rare chronic inflammatory large vessel vasculitis which affects the aorta and its large branches. The diagnosis is based on the 2022 ACR/EULAR classification criteria for Takayasu arteritis. The management of this vasculitis is challenging. Although it is corticosteroid-responsive, relapses and disease progression are common. Thus, it is possible to resort to alternative conventional synthetic disease-modifying anti-rheumatic drugs and biologics, as second-line such as tumor necrosis factor-alpha inhibitors, tocilizumab, or JAK inhibitors as second-line agents is possib…

Predictors of Permanent Vision Loss in Giant Cell Arteritis.

To report on the occurrence and characteristics of eye manifestations and determine the predictors of permanent vision loss (PVL) in patients with giant cell arteritis. Case-control study. Retrospective cohort study of 258 patients diagnosed with giant cell arteritis (GCA) over a 20- year period at a single institution. Eighty nine of 258 patients (34.5%) with GCA had vision symptoms and 56 (21.7%) developed PVL. Acute loss of vision with no preceding ocular symptoms occurred in 28.5%; for those with symptoms, median time to vision loss was 4.5 days. Bilateral clinical eye involvement was pres…
