AIMS: Pulmonary vein isolation (PVI) alone in persistent atrial fibrillation (AF) is not as successful as in paroxysmal AF, and recent data indicate the key role of non-PV triggers. We aimed to assess the long-term safety and efficacy of left atrial appendage isolation (LAAi) as an adjunct to PVI using cryoballoon (CB) in persistent AF.
METHODS AND RESULTS: We compared 144 persistent AF patients (59 ± 10 years, 51% females) who underwent PVI combined with LAAi with a propensity-score matched cohort of 138 persistent AF patients (59 ± 6 years, 52% female) in whom PVI-only was performed. Baseli…

AIMS: Paroxysmal atrial fibrillation (PAF) commonly recurs after radiofrequency catheter ablation (RFCA). This study aimed to assess left atrial appendage (LAA) volume and function by transesophageal echocardiography (TEE) and to explore its value in predicting PAF recurrence after RFCA.
METHODS: Eighty patients with PAF were recruited. The left atrial (LA) and LAA volume and function were measured by transthoracic echocardiography (TTE) and TEE before ablation. Patients were followed up for 12 months after RFCA, and recurrence was recorded. Odds ratios of candidate risk indicators were deter…

BACKGROUND: Transesophageal echocardiography (TEE) is routinely performed before atrial fibrillation (AF) transcatheter ablation to exclude the presence of left atrial (LA) or LA appendage (LAA) thrombi. The aim of the study is to evaluate if easily accessible clinical parameters may relate to the presence of LA or LAA thrombi to identify patients who could potentially avoid TEE.
METHODS AND RESULTS: Between January 2012 and September 2014, data from 1539 consecutive patients undergoing TEE, as a work-up before AF transcatheter ablation, in six large volume centers were collected. Baseline cl

BACKGROUND: Left atrial appendage (LAA) closure using the WATCHMAN™ device (WM) may be considered in patients with non-valvular atrial fibrillation (AF) and a high-risk of stroke who are ineligible for long-term oral anticoagulation (OAC).
AIM: To report our single-centre preliminary experience, focusing on feasibility, safety and short-term efficacy of this procedure.
METHODS: Patients implanted from December 2013 to February 2014 were included. The procedure, performed under general anaesthesia, was guided by fluoroscopy and transoesophageal echocardiography (TOE). Efficacy was defined as …

INTRODUCTION: Evidence of left atrial appendage thrombogenic milieu (LAA TM) on transesophageal echocardiography (TEE) is recognized as a surrogate marker for an increased stroke risk. Although the CHA2DS2-VASc scale is commonly used as a measure of thromboembolic risk in patients with atrial fibrillation (AF), it was shown to have only low-to-moderate ability to predict the presence of LAA TM. The potential role of transthoracic echocardiography (TTE) in the refinement of clinical scales for the detection of LAA TM in patients with AF has been readdressed recently.
OBJECTIVES: The aim of the…

OBJECTIVES: To report procedural outcome of sequential occlusion of the left atrial appendage (LAA) and an interatrial septal communication and discuss possible indications.
BACKGROUND: There are some patients who may have indications for both closure of patent foramen ovale (PFO) or atrial septal defect (ASD), as well as closure of the LAA. The optimal procedural strategy is not known.
METHODS: A retrospective review of LAA and PFO/ASD cases at our center was performed. Demographic, echocardiographic, and procedural data were recorded.
RESULTS: Closure of LAA and then PFO/ASD (Group I: 11 …

Rigid-wall left atrium (LA) and left atrial appendage (LAA) computational fluid dynamics models are practical but suppress deformation-driven transport, whereas image-derived wall motion is data- and processing-intensive. We evaluated whether a reduced-order, volume-constrained wall-motion prescription changes computed LA/LAA hemodynamic descriptors relative to a static-wall approximation. A patient-specific LA/LAA geometry was combined with literature-informed phasic volumes and boundary conditions to simulate healthy dynamic, grade-II diastolic dysfunction (DD II) dynamic, and DD II static c…

BACKGROUND: Pre- and post-procedural imaging in left atrial appendage occlusion (LAAO) adds to patient inconvenience and risks and may not materially influence outcomes. A more focused approach involving only peri-procedural imaging may suffice.
METHODS AND RESULTS: Data from patients who had LAAO at our centre between 2009 and 2024 were examined. Those with pre- and post-procedure imaging studies were excluded. Efficacy outcomes including procedural success and periprocedural complication were recorded. Long-term outcomes comprising cerebrovascular events, major bleeding, cardiovascular and …

The case regards a 73-year-old male who was referred for consideration of percutaneous left atrial appendage closure. His left atrial appendage was ligated during prior mitral valve surgery, but was noted to be patent on a recent transesophageal echocardiography. An Amulet device was selected based on CT planning. This report describes a challenging appendage anatomy unsuitable for the commercial devices, managed successfully with an off-label device.

AIMS: The optimal short-term antithrombotic strategy following left atrial appendage occlusion (LAAO) remains uncertain, with the need to balance thromboembolic prevention and bleeding risk presenting a critical challenge. Recent evidence suggests that direct oral anticoagulants (DOACs) may provide a favourable safety-efficacy profile, with low-dose regimens showing potential benefits during the device endothelialization period. This network meta-analysis (NMA) aimed to compare the efficacy and safety of various antithrombotic strategies, including DOAC dosing, following LAAO.

BACKGROUND AND OBJECTIVE: Pulmonary vein isolation (PVI) is a common intervention for symptomatic atrial fibrillation (AF). Transesophageal echocardiography (TEE) and cardiac computed tomography (CCT) are commonly used pre-PVI to assess left atrial (LA) and left atrial appendage (LAA) thrombus and visualize LA and pulmonary vein anatomy. This retrospective study aims to compare CCT and TEE in detecting LAA thrombus in patients with symptomatic paroxysmal or persistent AF scheduled for PVI.
METHODS: Seven hundred five patients underwent routine TEE and CCT within 24 h. Patients with prior LAA …

Congenital right atrial appendage aneurysm (RAAA) represents a rare cardiac anomaly whose exact etiology, prognostic implications, and optimal management remain undetermined. RAAA characteristic echocardiographic features include a cystic structure demonstrating continuity with the right atrial chamber, with confirmed hemodynamic communication between the two structures. While fetal RAAA is typically detected during the third trimester of pregnancy, our case represents an unusually early diagnosis during the first trimester, with comprehensive longitudinal follow-up extending to 2 years postna…

BACKGROUND: Incomplete left atrial appendage (LAA) closure ("leak") is a challenge of LAA occlusive techniques and carries thromboembolic risk. Closure of significant leaks after surgical AtriClip is rare and less described.
CASE SUMMARY: An 81-year-old man with atrial fibrillation on apixaban, prior LAA AtriClip (2023), and multiple recent falls underwent transesophageal echocardiography that showed a residual severe central LAA ostium. Percutaneous ostia closure was performed using an atrial septal defect (ASD) plug via transseptal puncture approach. Residual flow ceased in the LAA, and ant…

BACKGROUND: The benefit of prophylactic concomitant left atrial appendage closure (LAAC) in cardiac surgery patients without preoperative atrial fibrillation is unclear. We aim to evaluate whether LAAC reduces the risk of late stroke in patients without preoperative atrial fibrillation.
METHODS: A systematic search was conducted through July 2025 for studies investigating LAAC vs no LAAC in cardiac surgery patients without atrial fibrillation. Random-effects meta-analysis was performed, and Kaplan-Meier curves were digitized to reconstruct individual patient data. Primary outcome was stroke a…

INTRODUCTION: Left atrial appendage closure (LAAC) has become a recommended addition to oral anticoagulation for patients with atrial fibrillation, who undergo cardiac surgery. The procedure significantly reduces the risk of stroke and systemic arterial embolism, potentially making oral anticoagulation (OAC) unnecessary or even harmful, when considering the associated increased risk of bleeding. This publication describes the rationale and design of a randomized trial, testing the hypothesis that stopping OAC is noninferior to continuing OAC after surgical LAAC in terms of the primary endpoint…

BACKGROUND: Gastrointestinal bleeding in patients with atrial fibrillation is an indication for left appendage occlusion. All endovascular devices mandate antithrombotic therapies: rebleeding risk remains an issue. To date, there are no reports on gastrointestinal rebleeding and stroke prevention by left appendage occlusion without any antithrombotic therapy in this category of patients.
METHODS: A total of 129 patients (male 85, mean age 76.6 ± 7.1, CHA2DS2Vasc 3.8 ± 1.5, HASBLED 3.3 ± 1.0; upper GI bleeding 10%, lower GI bleeding 86%, obscure occult 4.6%, on NOACS full dose 77.5%, NOACs red

Amplatzer and Watchman devices are the most used left atrial appendage occlusion (LAAO) systems. Real-world studies comparing their newer generation systems, the Amulet and Watchman FLX, are scarce. The goal of this study is to compare procedural and clinical outcomes of Amulet and Watchman FLX recipients. We analyzed the early adoption of the Amulet (n = 131) at a large health system between 9/2021 and 6/2023 and compared it to Watchman FLX (n=194) cases for procedural success, presence of peri-device leak (PDL) and device-related thrombosis (DRT) as well as clinical outcomes during follow-up…

Congenital left atrial appendage aneurysm (CLAAA) is a rare congenital cardiac anomaly that can lead to significant cardiac and systemic complications. Early recognition through fetal echocardiographic scanning is crucial for establishing a comprehensive management plan prior to delivery, including baseline postnatal assessment and close follow-up to allow timely surgical resection before the development of severe local or systemic complications such as heart failure or thromboembolism. We report a case of giant CLAAA initially suspected as a fetal cardiac mass on routine obstetric ultrasound,…
