Atrioventricular conduction patterns suggestive of dual A-V nodal pathways have been reported in patients with and without a history of paroxysmal A-V nodal re-entrant tachycardia (PSVT). The purpose of this study was to determine whether significant association exists between this conduction pattern and the occurrence of PSVT in man. The pattern of A-V conduction was evaluated at similar pacing rates in 13 patients with documented PSVT and 135 patients with PSVT. Patients without PSVT were divided into groups with normal PR intervals (106 patients), PR intervals of 120 msec. or less (12 patie

The Wenckebach phenomenon of entrance block in intermittent atrioventricular junctional parasystole is reported for the first time in a 40-year-old woman. In this case the presence of dual pathways in the atrioventricular junction is demonstrated. One of them is a pathway without conduction disturbance. The other is a pathway containing both the ectopic focus and the site of second-degree entrance block. This site is located a considerably long distance above the focus. Entrance block occurs also at some site below the ectopic focus, which is a part of the pathway containing the focus. It is s

A histological study by the method of seriated sections in one case of "double-outlet left ventricle" made it possible to confirm and to describe the embryology of the nodal-His junction: not only the atrio-ventricular node (AV N) but also the His bundle (H) have a double origin, atrial and ventricular. Fusion of these two buds, inexistant in the "lone ventricle" is certainly imperfect. Two macroscopically normal hearts provided examples of nodal buds in one case, of a persisting cleft between the two His buds in the other.

Exposure to maternal anti-Ro (SS-A) and anti-La (SS-B) antibodies is a well-described risk factor for the development of fetal atrioventricular (AV) block. The role of maternal fluorinated steroids in the treatment and prevention of antibody-mediated fetal AV block is controversial. Fetal atrial flutter has rarely been described in association with maternal antibodies. This report describes a case of fetal exposure to maternal anti-Ro antibodies with associated second-degree AV block and atrial flutter. Interestingly, the reported patient had 2:1 AV conduction during both normal atrial rates (

The study aimed to evaluate the effectiveness of atrioventricular nodal slow pathway ablation in patients with atrioventricular nodal reentrant tachycardia (AVNRT) and examine its impact on the atrioventricular conduction function. A retrospective analysis was performed on the clinical data of 90 AVNRT patients who underwent radiofrequency ablation at our institution between August 2018 and February 2021. Based on the presence or absence of slow pathway conduction during the procedure, patients were classified into the slow pathway elimination group (SPE group) and the slow pathway improvement

INTRODUCTION: The slow atrioventricular nodal pathway (SP) typically has a single atrial breakthrough site.
METHODS AND RESULTS: We report a case of fast-slow type AVNRT with two distinct atrial breakthrough sites during tachycardia. The earliest atrial activation site (EAAS) was at the right inferior septum, followed by the inferolateral wall of the tricuspid annulus. Ablation at the EAAS shifted it to the inferolateral wall.
CONCLUSION: This case demonstrates two distinct atrial breakthrough sites from a single SP in AVNRT.

We performed transcatheter AV junction ablation with high frequency energy in four patients with AV nodal reentrant tachycardia where extensive trials of several antiarrhythmic drugs failed to prevent further recurrences of tachycardia. Initially high frequency catheter ablation induced complete AV block in all patients. A recuperation of AV 1:1 conduction followed some time later, persisting in follow-up. No complications have been encountered in either the acute phase or the follow-up (from 6 to 8 months; mean +/- SD: 8.7 +/- 2.5 months). The electrophysiological study was carried out 6 week

An intracardiac electrophysiological investigation was performed in 15 patients with frequent episodes of supraventricular tachycardias (SVT) which are regarded, in terms of the present electrophysiological criteria, as episodes of reciprocal atrioventricular nodal tachycardia. It was demonstrated that accessory atrionodal junctions functioning only in the retrograde direction formed the basis for arrhythmias in 6 patients. This electrophysiological phenomenon was proposed to be termed the latent Clerc-Lery-Cristesco (CLC) syndrome. The main electrophysiological criteria for diagnosing the syn

BACKGROUND: Mahaim fibers are rare types of accessory pathways with atrioventricular node-like decremental conduction properties. They present a significant challenge in terms of diagnosis and treatment.
CASE SUMMARY: We present the case of a patient with bouts of left bundle branch block-morphology wide complex tachycardia, ultimately diagnosed as atrioventricular reciprocating tachycardia with antegrade conduction over an atriofascicular accessory pathway. We demonstrate a systematic approach to diagnosis of left bundle branch block-morphology tachycardia along with several classic features…

We analyzed role of cardiac α1-adrenoreceptors for the torsadogenic action of IKr blocker nifekalant in isoflurane-anesthetized atrioventricular block rabbits. Bradycardia was induced by atrioventricular node ablation, and the ventricle was electrically driven at a constant rate of 60 beats/min throughout the experiments to prevent rate-dependent modification by the IKr blocker in ventricular repolarization phase. Nifekalant (3 mg/kg per 10 min, n = 5) prolonged the duration of monophasic action potential (MAP90) by +178 ± 43 ms, increased the short-term variability of repolarization (STV) to …

A retrospective study of 192 patients centered on the outcome of supraventricular arrhythmias after catheter ablation of the atrioventricular junction provided some useful information concerning the choice of pacing mode in these patients. With the exception of atrioventricular bloc after ablation of the rapid nodal pathway where simple DDD pacing is adequate, rate adaptive pacing would seem to be essential. The VVIR mode should be the mode of choice in atrial flutter, permanent atrial fibrillation, poorly controlled atrial fibrillation and paroxysmal atrial fibrillation of elderly subjects (o

Myocarditis can be complicated by various conduction abnormalities, including supraventricular and ventricular arrhythmias. It is rare to see conduction disorders as the first and only presentation of myocarditis, which may be reversible or persistent after the acute phase of myocarditis. Here, we present a case of probable myocarditis with preserved right and left ventricular function, isolated atrial cardiomyopathy, and severe conductive disturbances, including sinoatrial node dysfunction, atrial flutter, interatrial block, transient loss of atrial electromechanical activity and persistent c

An 11-year-old boy presented with dilated cardiomyopathy in association with a fasciculoventricular pathway and sinus node and atrioventricular conduction abnormalities. Whole exome sequencing revealed a novel variant of uncertain significance in LIM domain-binding protein 3 (LDB3) which has not been heretofore described. This variant was predicted to be deleterious by the computational prediction tools: Polyphen2 and SIFT. The mutated allele was heterozygous in the patient. Segregation analysis revealed that his father carried the same variant in heterozygous form. The case is reported for it

Common atrium (CA), also called three-chambered heart, is one of the rare congenital anomalies, defined by a complete absence of the atrial septum, eventually associated with malformation of the atrioventricular (AV) valves. We report the case of a 57-year-old woman with CA complicated with Eisenmenger syndrome and inferior vena cava interruption, who suffered from symptomatic persistent atrial fibrillation (AF). She underwent an initial successful pulmonary vein isolation procedure. A repeat procedure for perivalvular atrial flutter was complicated with inadvertent complete AV block, due to u

Patients with heart failure (HF) may develop a range of bradyarrhythmias including sinus node dysfunction, various degrees of atrioventricular block, and ventricular conduction delay. Device implantation has been recommended in these patients, but the specific etiology should be sought as it may influence the choice of the type of device required (pacemaker vs. implantable cardiac defibrillator). Also, pacing mode must be carefully set in patients with heart failure (HF) and left ventricular systolic dysfunction. In this chapter, we summarize the knowledge required for a tailored approach to br

There are cases of cardiomyopathy which are not easily classifiable into hypertrophic, dilated or restrictive type. They often show ventricular arrhythmia, right or left bundle branch block, intraventricular conduction disturbance, atrioventricular conduction disturbance, and sinus node dysfunction (sick sinus syndrome). Endomyocardial biopsy in such cases reveals advanced myocardial disease, for which we propose the simpler term "electric disturbance type cardiomyopathy (ECM)". In our consecutive series of studies of cardiomyopathy employing endomyocardial biopsy in 573 cases, 264 (46%) were

Nodal reentrant supraventricular paroxysmal tachycardia corresponds to a reentry circuit established between fibers with different conduction velocities and refractory periods in the atrioventricular node. These are the slow and fast nodal pathways. That ventricular tissue does not form part of the circuit of this arrhythmia is accepted nowadays, and the involvement of atrial tissue is discussed. We report a 57 years old male with a nodal reentrant tachycardia. In the electrophysiological study he presented an atrial and ventricular dissociation during the tachycardia. These findings allow a b…
