Fine-tuned structural modifications enable specific drug design against multidrug-resistant pathogens.

In this issue of Structure, Krah et al.1 present a comprehensive study combining molecular dynamics (MD) simulations, free-energy calculations, and in vivo mutagenesis experiments to investigate how water molecules interact with the F1FO-ATP synthase c-ring domain. Their findings highlight the potential of this bacterial enzyme as a drug target.

[Shigella: from the rupture to the invasion and to the destruction of the colonic epithelium].

CROSSING THE EPITHELIAL BARRIER: Shigella acts on the immune system in order to cross the intestinal epithelial barrier which is normally impermeable to intestinal flora. Among the effectors of the immune system of the colonic mucosa are translocation sites corresponding to lymphatic follicles carrying a follicular epithelium in the presence of M cells. These cells capture molecules, particles and microorganisms from the intestinal barrier that they translocate to immune competent cells present in the follicles (antigen presenting cells including resident macrophages and then B and T lymphocyt

Lacticaseibacillus paracasei bacteremia and mediastinitis secondary to esophageal invasion by left hilar pleomorphic carcinoma: case report.

Lacticaseibacillus paracasei bacteremia associated with probiotic use is rare. We report a 71-year-old man with advanced lung cancer who developed L. paracasei bacteremia while ingesting a commercially available probiotic-fermented milk. Autopsy demonstrated direct tumor invasion and perforation of the mid-esophagus, providing a structural route for bacterial translocation from the gastrointestinal lumen into the bloodstream. Genetic analysis showed that the blood isolate was genetically identical to the probiotic strain contained in the ingested product. Bacteremia resolved with antibiotics, …

Massive emphysematous pancreatitis associated with duodenal microperforation.

Emphysematous pancreatitis (EP) is a rare variant of necrotizing pancreatitis which may result from bacterial superinfection of pancreatic tissue with gas-forming organisms such as Escherichia coli and Klebsiella pneumoniae. Gas formation is a consequence of mixed acid fermentation by these species, which may colonize the inflamed pancreatic tissue by intestinal translocation, hematogenous spread or direct seeding by penetrating ulcer. Previously described cases of EP associated with penetrating ulcer are exceedingly rare and typified by focal emphysema confined to the site of fistulation, oft

Dissection of the role of p62/Sqstm1 in activation of Nrf2 during xenophagy.

Upon infection of a cell by Salmonella, p62/Sqstm1 assembles on the microbes; simultaneously, p62/Sqstm1 is phosphorylated at Ser351, leading to inactivation of Keap1, which is responsible for degrading Nrf2. Thus, cytoprotective Nrf2 targets are induced at the same time that autophagosomes entrap the microbes (xenophagy). However, the detailed role of p62/Sqstm1 during xenophagy has remained unclear. Here we show that translocation of p62/Sqstm1 to invasive Salmonella precedes Ser351 phosphorylation. Furthermore, in addition to Ser351 phosphorylation, oligomerization of p62/Sqstm1 is also req

Spontaneous bacterial peritonitis caused by Listeria monocytogenes: two case reports and literature review.

Two new cases of spontaneous bacterial peritonitis (SBP) caused by Listeria monocytogenes are reported. Listeria monocytogenes was recovered from the ascitic fluid but not from the blood cultures of two adult diabetic inpatients with hepatic cirrhosis and SBP that had been treated empirically with cefotaxime. These two cases add to the 17 cases of Listeria monocytogenes SBP reported previously, stressing the relevance of this microorganism to this clinical condition. The recovery of Listeria monocytogenes from blood has been achieved in only half of the cases reported, suggesting the possibili

Histopathological Investigation of Progressive Encephalomyelitis with Rigidity and Myoclonus: An Autopsy Case Characterized by Oculomotor Dysfunction and Autonomic Failure.

Progressive encephalomyelitis with rigidity and myoclonus (PERM) is a rare disease associated with the presence of anti-glycine receptor (GlyR) antibodies. We herein report an autopsy case of an 80-year-old man diagnosed with anti-GlyR antibody-positive PERM who presented with symptoms of oculomotor dysfunction and autonomic failure. Despite intensive immunotherapy, the neurological symptoms showed almost no improvement, and the patient succumbed to aspiration pneumonia and bacterial translocation. Postmortem pathology revealed mild inflammatory changes and neuronal loss that were disproportio…

Pathophysiological-based treatments of complications of cirrhosis.

Detailed knowledge and understanding of the pathophysiological mechanisms and changes in hepatic and splanchnic function leading to the development of haemodynamic changes and portal hypertension in patients with cirrhosis are essential since it guides the search for targets to ameliorate liver-related abnormalities. Recent research has focused on the gut-liver axis, changes in intestinal permeability, translocation of bacterial products, and inflammation as important drivers of haemodynamic alterations and thereby targets for treatment. Additionally, treatment strategies should focus on micro

Rules of Engagement: The Type VI Secretion System in Vibrio cholerae.

Microbial species often exist in complex communities where they must avoid predation and compete for favorable niches. The type VI secretion system (T6SS) is a contact-dependent bacterial weapon that allows for direct killing of competitors through the translocation of proteinaceous toxins. Vibrio cholerae is a Gram-negative pathogen that can use its T6SS during antagonistic interactions with neighboring prokaryotic and eukaryotic competitors. The T6SS not only promotes V. cholerae's survival during its aquatic and host life cycles, but also influences its evolution by facilitating horizontal

Rifaximin: beyond the traditional antibiotic activity.

Rifaximin is a non-systemic oral antibiotic derived from rifampin and characterized by a broad spectrum of antibacterial activity against Gram-positive and -negative, aerobic and anaerobic bacteria. Rifaximin was first approved in Italy in 1987 and afterwards in many other worldwide countries for the treatment of several gastrointestinal diseases. This review updates the pharmacology and pharmacodynamics of rifaximin highlighting the different actions, beyond its antibacterial activity, such as alteration of virulence, prevention of gut mucosal adherence and bacterial translocation. Moreover, …

Monoamine transporter structure, function, dynamics, and drug discovery: a computational perspective.

With the breakthrough crystallization of the bacterial leucine transporter protein LeuT, the first available X-ray structure for the neurotransmitter/sodium symporter family, development of 3-D computational models is suddenly essential for structure-function studies on the plasmalemmal monoamine transporters (MATs). LeuT-based MAT models have been used to guide elucidation of substrate and inhibitor binding pockets, and molecular dynamics simulations using these models are providing insight into conformations involved in the substrate translocation cycle. With credible MAT models finally in h

Farnesoid X Receptor Activation Attenuates Intestinal Ischemia Reperfusion Injury in Rats.

INTRODUCTION: The farnesoid X receptor (FXR) is abundantly expressed in the ileum, where it exerts an enteroprotective role as a key regulator of intestinal innate immunity and homeostasis, as shown in pre-clinical models of inflammatory bowel disease. Since intestinal ischemia reperfusion injury (IRI) is characterized by hyperpermeability, bacterial translocation and inflammation, we aimed to investigate, for the first time, if the FXR-agonist obeticholic acid (OCA) could attenuate intestinal ischemia reperfusion injury. MATERIAL AND METHODS: In a validated rat model of intestinal IRI (lapar…

Identification of Conserved and Species-Specific Functions of the Listeria monocytogenes PrsA2 Secretion Chaperone.

The Gram-positive bacterium Listeria monocytogenes is a facultative intracellular pathogen that relies on the regulated secretion and activity of a variety of proteins that sustain life within diverse environments. PrsA2 has recently been identified as a secreted peptidyl-prolyl cis/trans isomerase and chaperone that is dispensable for bacterial growth in broth culture but essential for L. monocytogenes virulence. Following host infection, PrsA2 contributes to the proper folding and activity of secreted proteins that are required for bacterial replication within the host cytosol and for bacter

Bacterial porphyrins in healthy skin: Microbiota components impact melanogenesis and age-related processes leading to Porphyr'ageing.

OBJECTIVE: Porphyrins are ubiquitous metabolites and are constitutive of the bacterial metabolome of healthy skin. Their consideration has until now been limited to their pro-inflammatory activity in acne vulgaris. The present work suggests a new role for these molecules in the onset of skin ageing. METHODS: A mixture of coproporphyrin III and protoporphyrin IX, representative of skin microbiota metabolites, was defined and applied in different skin models. Finally, an in vivo study was conducted to determine the association between porphyrin's abundance and ageing signs. RESULTS: Bacterial

Enterococcal Endocarditis: Hiding in Plain Sight.

Enterococcus faecalis is a major opportunistic bacterial pathogen of increasing clinical relevance. A substantial body of experimental evidence suggests that early biofilm formation plays a critical role in these infections, as well as in colonization and persistence in the GI tract as a commensal member of the microbiome in most terrestrial animals. Animal models of experimental endocarditis generally involve inducing mechanical valve damage by cardiac catheterization prior to infection, and it has long been presumed that endocarditis vegetation formation resulting from bacterial attachment t

The Chlamydia effector CT622/TaiP targets a nonautophagy related function of ATG16L1.

The obligate intracellular bacteria Chlamydia trachomatis, the causative agent of trachoma and sexually transmitted diseases, multiply in a vacuolar compartment, the inclusion. From this niche, they secrete "effector" proteins, that modify cellular activities to enable bacterial survival and proliferation. Here, we show that the host autophagy-related protein 16-1 (ATG16L1) restricts inclusion growth and that this effect is counteracted by the secretion of the bacterial effector CT622/TaiP (translocated ATG16L1 interacting protein). ATG16L1 is mostly known for its role in the lipidation of the

Structure of the Legionella Effector, lpg1496, Suggests a Role in Nucleotide Metabolism.

Pathogenic Gram-negative bacteria use specialized secretion systems that translocate bacterial proteins, termed effectors, directly into host cells where they interact with host proteins and biochemical processes for the benefit of the pathogen. lpg1496 is a previously uncharacterized effector of Legionella pneumophila, the causative agent of Legionnaires disease. Here, we crystallized three nucleotide binding domains from lpg1496. The C-terminal domain, which is conserved among the SidE family of effectors, is formed of two largely α-helical lobes with a nucleotide binding cleft. A structural
