The postoperative complication rate of laparoscopic cholecystectomy (LC) is about 5-6%. The most frequent complications are residual gallstones in the common bile duct, biliary leckage, biliary fistula and duct stenosis. In the period between 01.11.94 and 01.04.96 we performed 1620 endoscopic retrograde cholangio pancreatographies including 410 papillotomies at the second Department of Medicine in the Clinic of Suhl. Biliary tracts were investigated in 3420 migrants and 1445 aborigines (Evens, Evenkis) living in Evenkia and Yakutia. Acalculous chronic cholecystitis and cholelithiasis were found in 8.5 and 8.8% of the migrants, respectively. In aborigines these diseases occurred in 4.5 and 1.5%, respectively. The risk to develop biliary disease for migrants grows after 4-5 years of stay in the Far North. It is concluded that biliary pathology in the Far North population was related to ethnic and geographical factors.

The excretion of cefotetan, a 7 alpha-methoxy-cephalosporin, was studied in 27 patients undergoing biliary surgery. Pharmacokinetic parameters were determined after a single intravenous bolus dose of 1 g (10 patients) or 2 g (17 patients). Bile electric conductivity and pH were studied in dynamics in 165 patients with acute cholecystitis. The bile was obtained from drains of the gallbladder, choledochus and its terminal part and from percutaneous-transhepatic cholangiostoma. The authors studied the effect of sonic waves on the motility of extrahepatic biliary tract and duodenum in 32 patients with hypotensive, hypokinetic dyskinesias. It was found that application of sound (the parameters are described in detail) on the region of the duodenum increased the frequency of contractions, medium biopotential, amplitude and tone of this organ to practically normal levels, resulted in normalization of the motility of extrahepatic biliary tract, favours decompression of the bile-excretory system.

The authors studied the site of draining operations on the biliary tract in 160 patients with biliary pancreatitis. Among 64 patients with chronic pancreatitis 41 (64.1%) were subjected to various draining operations on the biliary system, one patient died. The results of endoscopic percutaneous transhepatic cholelithotripsy in seven patients with stones in the deep biliary passages which could not be treated by endoscopic papillotomy (EST) or extracorporeal shock wave lithotripsy (ESWL) are presented. Sonography was used to investigate the prevalence of symptomatic and silent biliary tract disease, in free living urban population in Kashmir. A randomly drawn sample of 1695 subjects aged 15 years or above was interviewed by a questionnaire. Twenty six had previous cholecystectomies, all for gall stones.
