Treatments and outcomes of neonatal disseminated intravascular coagulation with and without neonatal asphyxia: A retrospective study using nationwide data in Japan.

BACKGROUND: Although neonatal disseminated intravascular coagulation (DIC) is associated with high mortality and severe complications, few studies have reported its clinical course. We aimed to describe the characteristics, treatments, and outcomes of neonatal DIC by using a national inpatient database. METHODS: Using the Japanese Diagnosis Procedure Combination database, we identified 5533 patients with neonatal DIC who were admitted to neonatal intensive care units between July 2010 and March 2020. We categorized the patients into those with asphyxia (n = 2911) and those without asphyxia (n

How Should Decision Science Inform Scarce Blood Product Allocation?

Blood products are a scarce resource in our health care system. This article discusses a pediatric case involving large quantities of blood products transfused at the end of life. It argues that decision aids could help clinicians determine when to request ethics consultation or re-evaluation of blood product usage in a specific patient care situation and considers questions about scarce resource allocation, futility, and parental involvement in decision making.

Factors influencing plasma transfusion practices in paediatric intensive care units around the world.

BACKGROUND AND OBJECTIVES: Plasma transfusions are a frequent treatment worldwide, but many studies have reported a wide variation in the indications to transfuse. Recently, an international paediatric study also showed wide variation in frequency in the use of plasma transfusions: 25% of the centres transfused plasma to >5% of their patients, whereas another 25% transfused plasma to <1% of their patients. The objective of this study was to explore the factors associated with different plasma transfusion practices in these centres. MATERIALS AND METHODS: Online survey sent to the local invest

Platelet-rich plasma extract prevents pulmonary edema through angiopoietin-Tie2 signaling.

Increased vascular permeability contributes to life-threatening pathological conditions, such as acute respiratory distress syndrome. Current treatments for sepsis-induced pulmonary edema rely on low-tidal volume mechanical ventilation, fluid management, and pharmacological use of a single angiogenic or chemical factor with antipermeability activity. However, it is becoming clear that a combination of multiple angiogenic/chemical factors rather than a single factor is required for maintaining stable and functional blood vessels. We have demonstrated that mouse platelet-rich plasma (PRP) extrac

Efficacy of fresh-frozen plasma and cryoprecipitate in dogs with von Willebrand's disease or hemophilia A.

Here we report the comparative efficacy of fresh-frozen plasma (FFP) and cryoprecipitate in the treatment of 2 inherited bleeding disorders in dogs. The dogs were divided into 3 groups, consisting of 4 Doberman Pinschers with type I von Willebrand's disease (vWD) (group 1), 1 Scottish Terrier with type III vWD (group 2), and 4 German Shepherd Dogs with hemophilia A (group 3). In vWD, therapeutic efficacy was determined by the ability of the products to increase von Willebrand factor antigen (vWf:Ag) concentrations above 35 canine units (CU)/dL and to correct the prolonged buccal mucosal bleedi

A randomized phase II trial of low-dose aclarubicin vs very low-dose cytosine arabinoside for treatment of myelodysplastic syndromes.

We performed a randomized phase II trial comparing low-dose aclarubicin (LC-ACR) with very low-dose cytosine arabinoside (VLD-AC) in 39 consecutive untreated patients with myelodysplastic syndromes (MDS), including refractory anemia (RA), RA with excess of blasts (RAEB) and RAEB in transformation (RAEB-t). Nineteen patients received the VLD-AC therapy; 2 good responses (GR) and 2 partial responses (PR) were obtained in 11 patients with RAEB and RAEB-t, while 2 PR were obtained in 8 RA patients. Eighteen patients received the LD-ACR therapy; 2 GR and 4 PR were obtained in 11 RAEB/RAEB-t patient

Transfusion-Transmitted Hepatitis E: NAT Screening of Blood Donations and Infectious Dose.

The risk and importance of transfusion-transmitted hepatitis E virus (TT-HEV) infections by contaminated blood products is currently a controversial discussed topic in transfusion medicine. The infectious dose, in particular, remains an unknown quantity. In the present study, we illuminate and review this aspect seen from the viewpoint of a blood donation service with more than 2 years of experience in routine HEV blood donor screening. We systematically review the actual status of presently known cases of TT-HEV infections and available routine NAT-screening assays. The review of the literatu

Anti-M-Induced Delayed Hemolytic Transfusion Reaction.

BACKGROUND: Anti-M is most often assumed to be naturally occurring and can be comprised of a mixture of predominantly immunoglobulin(Ig)M with a lesser IgG component. Anti-M-antibodies usually do not react at 37°C and therefore are considered to be of little clinical significance. METHODS: A 28-year-old man presented with hemorrhagic shock from numerous injuries sustained in a motor vehicle collision. The patient received several units of red blood cells (RBCs). The antibody screen, the direct antiglobulin test (DAT), and the RBC genotype were sent for laboratory evaluation. RESULTS: A total

Blood Donor Screening for West NiIe Virus in Oklahoma and Its Contribution to Disease Surveillance, 2003 -2013.

Upon recognition that West Nile virus (WNV) was transmissible by transfusion, universal testing of blood donors by nucleic acid testing (NAT) was initiated in 2003. A retrospective review of 2003-2013 blood donor records and public health surveillance data in Oklahoma was undertaken to determine the percentage of WNV-positive blood donors who developed clinical symptoms post-donation and to examine the incidence and timing of WNV viremic donors in the context of WNV disease reported statewide. Among all WNV NAT-positive blood donors, 19% had self-described symptoms consistent with WNV disease.

Lookback investigations of Babesia microti-seropositive blood donors: seven-year experience in a Babesia-endemic area.

BACKGROUND: Human babesiosis in the United States is primarily attributable to infection with the intraerythrocytic protozoan parasite, Babesia microti. Transfusion-transmitted Babesia (TTB) is a mounting blood safety concern; approximately 100 US cases of TTB have been reported since 1980. In response, market withdrawal (MW) and/or lookback (LB) has been advocated for cellular components derived from Babesia-positive blood donors. STUDY DESIGN AND METHODS: Immunofluorescence assay (IFA) and selective polymerase chain reaction (PCR) testing of Connecticut donors was conducted from 1999 throug

[Anesthesia for severe genital hemorrhage].

Severe genital hemorrhage is one of the three leading causes of death in gyneco-obstetrics and oncology. Since this problem is a surgical emergency leaving no room for delay, the causes for this phenomenon are analysed in this paper, stressing the importance of detection and clinical assessment in order to determine the effective and immediate type of corrective surgical procedure. The principal objective is to specify the role of the anesthesiologist during the patient's preoperative, transoperative and postoperative periods. Some general and some specific recommendations are made with regard

Massive transfusion of low-titer cold-stored O-positive whole blood in a civilian trauma setting.

BACKGROUND: Based on the improved outcomes achieved with fresh whole blood in cases of military trauma as well as with 1:1:1 transfusion strategies for massive traumatic hemorrhage in civilian settings, there has been resurgent interest in using whole blood for civilian trauma patients. There have been reports of giving up to 4 units of low-titer cold-stored O-positive to these patients. This is the first modern report of a massive transfusion with unrestricted low-titer group O whole blood (LTOWB) use in a civilian trauma patient. STUDY DESIGN AND METHODS: This is a case report describing th

Impact of ROTEM on transfusion strategy in the management of coagulopathy during postpartum hemorrhage ≥1000 mL: a matched pre-post study.

BACKGROUND: Rotational thromboelastometry (ROTEM) provides rapid, point-of-care coagulation data. However, the specific effect of its use for coagulopathy management on blood component transfusions during severe postpartum hemorrhage remains undetermined. We aimed to assess whether the use of ROTEM was associated with reduced administration of hemostatic blood products and blood transfusions in women with postpartum hemorrhage ≥1000 mL. METHODS: In this single-center retrospective pre-post matched study, administration of hemostatic blood products and packed red blood cells was compared in tw…

DOES WHOLE BLOOD RESUSCITATION INCREASE RISK FOR VENOUS THROMBOEMBOLISM IN TRAUMA PATIENTS? A COMPARISON OF WHOLE BLOOD VERSUS COMPONENT THERAPY IN 3,468 PATIENTS.

Background: Whole blood (WB) resuscitation has been shown to provide mortality benefit. However, the impact of whole blood transfusions on the risk of venous thromboembolism (VTE) remains unclear. We sought to compare the VTE risk in patients resuscitated with WB versus component therapy (COMP). Methods: Trauma patients aged 18 and older, admitted to two Level 1 trauma centers between 2016 and 2021, who received at least one unit of emergency-release blood products were identified. Clinical and transfusion data were collected. Patients that received any WB during resuscitation were compared to

Anemia in the critically ill.

The anemia of critical illness is a distinct clinical entity with characteristics similar to that of chronic disease anemia. Several solutions to the processes of anemia, such as blunted erythropoietin production and erythropoietin response and abnormalities in iron metabolism have been developed. The transfusion of RBCs provides immediate correction of low hemoglobin levels, which may be of value in patients with life-threatening anemia. Avoidance of RBC and blood component transfusion, however, is becoming increasingly important as data of adverse clinical outcomes in critically ill patients…

Improved Outcomes of Enhanced Recovery After Surgery (ERAS) Protocol for Radical Cystectomy with Addition of a Multidisciplinary Care Process in a US Comprehensive Cancer Care Center.

INTRODUCTION: Although enhanced recovery after surgery (ERAS) components include both anesthesia and surgical care processes, it is unclear whether a multidisciplinary approach to implementing ERAS care processes improves clinical outcomes. The addition of multidisciplinary care with anesthesiology-related components to an existing ERAS protocol for radical cystectomy at a US comprehensive cancer center provided an opportunity to compare short- and long-term outcomes. METHODS: We retrospectively compared the outcomes of 116 consecutive patients who underwent cystectomy after implementation of

Thrombopoietin Receptor Agonist (Avatrombopag) Preoperative Administration to Ameliorate Sepsis-Related Thrombocytopenia in Jehovah's Witness Patients Undergoing Redo Cardiac Surgery for Acute Endocarditis: A Case Report.

Patients with infective endocarditis may present with severe thrombocytopenia, identifying higher-risk groups with worse clinical outcomes and mortality rates. Frequently, in the surgical setting, these patients may benefit from platelet transfusions; however, this is not always feasible. Jehovah's Witness (JW) patients refuse blood component transfusions on religious grounds, making the management of these patients extremely complex, to the point of deeming them inoperable. Thrombopoietin receptor agonists are now available for the treatment of thrombocytopenia in a variety of diseases. We re…

Trial Of Pathogen-reduced Cryoprecipitate vs. cryoprecipitated AHF to Lower Operative Transfusions (TOP-CLOT): a single center, prospective, cluster randomized trial in cardiovascular surgery.

BACKGROUND: Low perioperative fibrinogen levels potentiate hemorrhage and increase transfusions in cardiac surgery patients. Cryoprecipitated anti-hemophilic factor (Cryo) requires on-demand thawing, often delaying fibrinogen supplementation; its short post-thaw shelf life limits pre-emptive availability. In contrast, Pathogen Reduced Cryoprecipitated Fibrinogen Complex (PR CRYO) can be stored post-thaw at room temperature for up to five days, allowing immediate availability without the need for on-demand thawing. This single-center, randomized trial compared PR CRYO to Cryo in cardiac surgery

Massive transfusion practices and low-titer group O whole blood utilization before and after implementation in a low-volume level I trauma center: A six-year review.

BACKGROUND: Low-titer O whole blood (LTOWB) has been increasingly incorporated into trauma resuscitation and massive transfusion protocols (MTPs). Recent randomized trials have not demonstrated superiority of prehospital LTOWB over component-based resuscitation, while the utilization and sustainability of LTOWB programs, particularly in low-volume trauma systems, remain incompletely characterized. STUDY DESIGN AND METHODS: This was a 6-year review (April 2019-April 2025) of trauma MTPs at a level 1 trauma center with a low MTP frequency. Transfusion service logs, trauma flowsheets, prehospita
