Benefits of early intramedullary nailing in femoral metastases.

PURPOSE: Pathological fractures of the long bones are common complications of metastatic disease; however, the outcome of different surgical techniques for the treatment of these fractures has not been clearly defined. The aim of this study was to evaluate differences in prophylactic and therapeutic intramedullary nailing in femoral metastasic implants. METHODS: Sixty-five patients with metastasis of the femur were analysed retrospectively (37 females; 28 males) between 1995 and 2011 (follow-up 15 months). Forty-four presented with pathological fractures and 21 impending fractures (Mirel ≥7).

The treatment of fractures of the femoral shaft with the Brooker-Wills distal locking intramedullary nail.

Ninety-two fractures of the femoral shaft in eighty-five patients were followed after treatment with the Brooker-Wills distal locking intramedullary nail. Twenty-one (22.8 per cent) of the fractures were open and seventy-one (77.2 per cent) were closed; 84.8 per cent of the fractures were comminuted. All but one fracture united, in a mean time of 4.4 months. Four fractures healed with mild angulation, and three had shortening of more than one but less than two centimeters. Significant rotatory deformities did not occur. No patient required postoperative traction or external stabilization, and

Treatment of Periprosthetic Joint Infection in Total Knee Arthroplasty with a Temporary Intramedullary Nail: Is a Long or Short Nail Better?

To our knowledge, no studies have compared postoperative outcomes between patients who received a temporary short or long intramedullary (IM) nail in the setting of infected total knee arthroplasty (TKA). Therefore, the aim of this study was to compare short-term outcomes for patients who underwent long or short IM nail insertion for treatment of periprosthetic knee infection. Specifically, we compared: (1) success rates; (2) patient reported/functional outcomes; and (3) complications between patients implanted with a short or a long IM nail following PJI of the knee. A retrospective chart rev

Efficacy and complications of blocking screws fixation in the treatment of lower limb long bone fracture: a meta-analysis.

BACKGROUND: Long bone fractures, especially in the lower limbs, are highly prevalent in orthopedic practice. These fractures can significantly impair patients' mobility and quality of life. Intramedullary nails are a mainstay treatment, offering reliable fracture fixation. However, the addition of blocking screws has introduced an element of uncertainty regarding surgical outcomes. This meta-analysis evaluated the efficacy and complications of blocking screw fixation for lower limb long bone fractures (LLLBF). METHODS: A comprehensive and systematic search was conducted across eight databases

Fingernail injury in children.

Injuries to the nail complex are common in children. Most children who present with fingertip trauma have experienced a crush injury, usually from the hinge side of a door and often in the living room. This article reviews the literature on the management of fingertip crush injuries, fractures of the distal phalanx, nail bed injury and subungual hematoma to establish, where possible, best evidence.

Gluteus Medius Insertion Damage: A Comparison of Reconstruction and Hip Fracture Nails.

This study assessed damage to the gluteus medius tendon insertion when performing trochanteric nailing with either a reconstruction nail or a cephalomedullary nail. Ten cadavers were randomized to receive proximal reaming for either a reconstruction nail or cephalomedullary nail; the contralateral hip served as an internal control. One specimen was found to have a displaced femoral neck fracture and was excluded from data analysis. Reconstruction nailing led to tendon damage in 4 of 9 hips, compared to 8 of 9 with hip fracture nailing (p = 0.29). In the reconstruction nail group, the average a

Tibio-Talo-Calcaneal Arthrodesis: Evaluation of the Effectiveness of a Specific Surgical Technique in 17 Cases.

INTRODUCTION: Ankle arthrodesis is a crucial surgical intervention for advanced hindfoot conditions, aiming to restore plantigrade walking and alleviate pain. This study evaluates the effectiveness of a specific surgical approach for tibiotalocalcaneal arthrodesis (TTCA), focusing on rigorous risk factor control, corticocancellous grafting, and internal fixation using an angled retrograde femoral nail in the sagittal plane, and assesses the outcomes of this approach in terms of bone fusion and reduction of postoperative complications. MATERIALS AND METHODS: This retrospective analysis include

Tibiotalocalcaneal arthrodesis with combined retrograde intramedullary nail and lateral L-plate.

Achieving stable fixation when performing tibiotalocalcaneal or tibiocalcaneal arthrodesis can be challenging. Patients undergoing these procedures often have osteopenia, poor bone stock, fragmentation of the bones of the foot and ankle, joint subluxation, or even dislocation. The author describes a technique of tibiotalocalcaneal arthrodesis with intramedullary nail fixation augmented by lateral plating through the transfibular approach. This can provide excellent exposure of the ankle and subtalar joints, morselized bone for grafting, and better construct stability.

Dual Fixation in Complex Long Bone Fractures: A Case Series of 30 Patients.

INTRODUCTION: Complex long bone fractures characterized by comminution, segmental patterns, metaphyseal extension, or associated joint instability pose significant challenges to stable fixation. While intramedullary nailing remains the standard treatment for diaphyseal fractures, certain fracture configurations are prone to mechanical failure, delayed union, or non-union when treated with a single fixation modality. Dual fixation using a combination of intramedullary nailing and plate augmentation may provide superior biomechanical stability and improve healing outcomes. OBJECTIVES: To evalua

Does the Proximal Humerus Nail with 2 Distal Screws Provide Sufficient Rotational Stability?

BACKGROUD: Proximal humerus nails, frequently used for managing proximal humerus fractures, significantly enhance rotational stability and reinforce fractured fragments. Few research exists regarding the optimal number and positioning of distal screws. This study aimed to assess the stability of diverse screw configurations and scrutinize screw distribution and bone stress via finite element analysis. METHODS: The humerus intramedullary nail (Humerus Interlocking Nail System; TDM) underwent assessment using finite element analysis applied to a humerus model. Three groups were established base

Complications after intramedullary nail fixation of pathological versus non-pathological femoral shaft fractures: a retrospective cohort study in 233 patients.

BACKGROUND: Postoperative adverse events after intramedullary nailing have been reported in patients with metastatic pathological and non-pathological femoral fractures. Other consequences to be considered are readmission and reoperation. Few studies have compared the risks of postoperative adverse events, reoperation, and readmission after intramedullary nailing of pathological and non-pathological femur fractures. This study was designed to test the hypothesis that patients with pathological femoral fractures had more adverse events, readmission, and reoperation following surgical fixation t

Avulsion of flexor digitorum profundus secondary to recurrent enchondroma.

We report a case of avulsion of the flexor digitorum profundus secondary to recurrent enchondroma. The tumor was curetted with bone grafting of cancellous bone from the distal radius, and the fragment avulsed by the flexor digitorum profundus tendon was fixed to its original site using a pullout suture through the distal phalanx to the nail. The lesion healed and function recovered 6 years after surgery.

Fat embolism syndrome in an adolescent before surgical treatment of an isolated closed tibial shaft fracture.

Fat embolism syndrome (FES) occurs most commonly in adults with high-energy trauma, especially fractures of the long-bones and pelvis. Because of unique age-related physiologic differences in the immature skeleton, as well as differences in fracture management in pediatric patients, FES is rare in children. To our knowledge, this is the first case report of FES occurring before surgical fixation of a closed tibial shaft fracture in an adolescent. A 16-year-old, 109 kg, Caucasian adolescent boy developed FES after closed diaphyseal fractures of the distal tibia and fibula, showing signs of resp

[Minimally invasive therapeutic concepts in fracture surgery].

Recent publications show a clear tendency towards minimally invasive procedures for fracture care in trauma patients. Intramedullary stabilization has become the first choice in reconstructing axis and length of the diaphyseal fractures of the long bones. Indications for intramedullary nailing have become wider with the development of unreamed or retrograde nailing. Essential modifications of plate osteosynthesis from limited contact implants to percutaneous plating and the development of an internal fixator have made this procedure minimally-invasive as well. Techniques of closed reduction an

Do two distal locking screws in two planes offer the best option in the nailing of diaphyseal tibial fractures?

BACKGROUND: Although the intramedullary nail represents the treatment for most tibia diaphyseal fractures, there is no agreement on the configuration of the distal locking screws. Therefore, this study aimed to investigate the impact of the configuration of distal locking bolts on the healing of a tibial shaft fracture. METHODS: We retrospectively analyzed 170 closed fractures of the tibia diaphyseal operated consecutively between 2018 and 2021. Patients with postoperative unacceptable reduction (>5° in any plane or gap >4 mm), nail-canal ratio <0.78, and less than 12 months of follow-up were

Treatment of Angular Deformity and Limb Length Discrepancy With a Retrograde Femur Magnetic Intramedullary Nail: A Fixator-assisted, Blocking Screw Technique.

BACKGROUND: Fixator-assisted nailing techniques that incorporate magnetic internal lengthening nails (MILNs) permit acute deformity correction and then gradual limb lengthening without needing postoperative external fixators. PURPOSES: We sought to investigate the safety and accuracy of a fixator-assisted, blocking screw technique using retrograde MILNs for the correction of LLD and limb malalignment. METHODS: Forty-one patients (13 patients with genu varum and 28 patients with genu valgum) with LLD treated with fixator-assisted, blocking screw retrograde MILN reconstruction were included. P

Femur malunion treated with open osteotomy and intramedullary nailing in developing countries.

INTRODUCTION: In developing countries, malunion, after diaphyseal femur fractures initially untreated by internal fixation, is not rare. Their difficult management contrasts with the deficiency of the technical operating room facilities. PATIENTS AND METHODS: Our prospective study, conducted over a 1-year period, reports 16 open osteotomies fixed using Küntscher intramedullary nailing in patients who presented malunion of the femoral diaphysis. Twelve males and four females (mean age, 34.5 years; range, 18-67 years) were managed with a mean time to surgery of 8 months (range, 4-14 months). Al
