The influence of bone density and anisotropy in finite element models of distal radius fracture osteosynthesis: Evaluations and comparison to experiments.

Continuum-level finite element (FE) models can be used to analyze and improve osteosynthesis procedures for distal radius fractures (DRF) from a biomechanical point of view. However, previous models oversimplified the bone material and lacked thorough experimental validation. The goal of this study was to assess the influence of local bone density and anisotropy in FE models of DRF osteosynthesis for predictions of axial stiffness, implant plate stresses, and screw loads. Experiments and FE analysis were conducted in 25 fresh frozen cadaveric radii with DRFs treated by volar locking plate oste

Biomechanical performance of proximal tibia fracture plates: A review.

PURPOSE: Proximal fractures of the tibia (i.e., shin bone) are often treated using proximal tibia fracture plates (PTFPs) that are not always biomechanically optimal. This is a review of biomechanical papers that studied the effect of modifying PTFP plate and screw variables. METHODS: PubMed, Scopus, and Web of Science databases were searched for English-language papers published before February 2024 using the terms "biomechanics" plus "proximal or plateau" plus "tibia" plus "fracture or "plate." Eligibility criteria were applied: (1) biomechanical studies only; (2) optimization studies that

Biomechanical comparison of the pullout strengths of C1 lateral mass screws and C1 posterior arch screws.

BACKGROUND CONTEXT: Conditions of the atlantoaxial complex requiring internal stabilization can result from trauma, malignancy, inflammatory diseases, and congenital malformation. Several techniques have been used for stabilization and fusion. Posterior wiring is biomechanically inferior to screw fixation. C1 lateral mass screws and C1 posterior arch screws are used for instrumentation of the atlas. Previous studies have shown that unicortical C1 lateral mass screws are biomechanically stable for fixation. No study has evaluated the biomechanical stability of C1 posterior arch screws or compar

Finite element analysis of screw thread geometry and titanium plate materials in internal fixation of the human femur.

Plate and screw osteosynthesis remains essential in orthopedics especially for the fixation of femoral fracture and other long bone injury. The objective of this investigation was to analyze the behavior of a titanium-plated and screwed femur during realistic loading while analyzing stress, strain, and displacement. The square, round, trapezoidal, and triangular screw thread profiles were investigated in terms of their transmission behavior on the head, shaft, and plates. Square thread profiles distribute the stress more uniformly, which results in a reduction in both peak and global von Mises

Biomechanical analysis of second-generation headless compression screws.

INTRODUCTION: Headless Compression Screws (HCS) are commonly utilized for the fixation of small bone and articular fractures. Recently several new second generation HCS (SG-HCS) have been introduced with the purported benefits of improved biomechanical characteristics. We sought to determine and compare the biomechanical efficiencies of these screws. MATERIAL AND METHODS: Five HCS including four second generation (Mini-Acutrak 2 (Acumed), Twinfix (Stryker), Kompressor Mini (Integra), HCS 3.0 (Synthes)) and one first generation (Herbert-Whipple) were studied. Polyurethane foam blocks that repr

The Bony Density of the Pedicle Plays a More Significant Role in the Screw Anchorage Ability Than Other Regions of the Screw Trajectory.

OBJECTIVE: Osteoporosis is a crucial risk factor for screw loosening. Our studies indicate that the bone mineral density (BMD) in the screw trajectory is a better predictor of screw loosening than the BMD of the lumbar spine or the screw insertion position. Research has shown that anchorage on the screw tip is the most significant factor for screw anchorage ability, while others argue that decreased bony quality in the pedicle poses a significant risk for screw loosening. This study aimed to determine whether the bony quality of the screw tip, pedicle, or screw-anchored vertebral body plays th

Influence of insertion depth on stress distribution in orthodontic miniscrew and the surrounding bone by finite element analysis.

We aimed to elucidate stress distribution in miniscrews and the surrounding bone when miniscrews inserted at different depths were implanted vertically or obliquely. The distributions of the equivalent stress on the screw surface and the minimum principal stress in the surrounding bone were calculated using finite element models. When the miniscrews were inserted vertically and obliquely, screw head displacement, greatest equivalent stress on the miniscrew surface, and absolute value of minimum principal stresses in the surrounding bone decreased with increasing insertion depth. Stresses in th

Biomechanical Evaluation of Patient-Specific Titanium Fixation Systems for Segmental Le Fort I Osteotomy: A Finite Element Analysis.

Patient-specific Ti-6Al-4V fixation systems are increasingly used to stabilise the maxilla following segmental Le Fort I osteotomy; however, the biomechanical responses of complete locking and non-locking patient-specific fixation configurations remain insufficiently characterised. In this study, two patient-specific plate designs (a standard design and a reduced, minimally invasive design) were analysed by linear-elastic finite element analysis, each with locking and non-locking screws. Because they were developed for different indications and evaluated in distinct patient-specific anatomical

Comparison of calcaneal fixation of a retrograde intramedullary nail with a fixed-angle spiral blade versus a fixed-angle screw.

BACKGROUND: Retrograde intramedullary nailing is an established technique for tibiotalocalcaneal arthrodesis (TTCA). In poor bone stock (osteoporosis, neuroarthropathy), device fixation in the hindfoot remains a problem. Fixed-angle spiral-blade fixation of the nail in the calcaneus could be useful. MATERIALS AND METHODS: In seven matched pairs of human below-knee specimens, bone mineral density (BMD) was determined, and TTCA was performed with an intramedullary nail (Synthes Hindfoot Arthrodesis Nail HAN Expert Nailing System), using a conventional screw plus a fixed-angle spiral blade versu

Larger screw diameter may not guarantee greater pullout strength for headless screws - a biomechanical study.

Headless compression screws (HCSs) are commonly utilized devices for small bone fracture fixation. The Mini-Acutrak 2 and headless reduction (HLR) screws are the newer version types, in which both have fully threaded and variable pitch design. Specifically, the HLR is characterized by two thread runouts to facilitate implantation. With the thread runouts, the holding strength of the screw may be compromised. To the best of our knowledge, no study has examined the pullout force of the global sizes of a HCS. We sought to determine the pullout strength of the HLR and compare the strength of this
