White matter alterations in college football players: a longitudinal diffusion tensor imaging study.

The aim of this study was to evaluate longitudinal changes in the diffusion characteristics of brain white matter (WM) in collegiate athletes at three time points: prior to the start of the football season (T1), after one season of football (T2), followed by six months of no-contact rest (T3). Fifteen male collegiate football players and 5 male non-athlete student controls underwent diffusion MR imaging and computerized cognitive testing at all three timepoints. Whole-brain tract-based spatial statistics (TBSS) were used to compare fractional anisotropy (FA), radial diffusivity (RD), axial dif

An atlas of the heterogeneous viscoelastic brain with local power-law attenuation synthesised using Prony-series.

This review addresses the acute need to acknowledge the mechanical heterogeneity of brain matter and to accurately calibrate its local viscoelastic material properties accordingly. Specifically, it is important to compile the existing and disparate literature on attenuation power-laws and dispersion to make progress in wave physics of brain matter, a field of research that has the potential to explain the mechanisms at play in diffuse axonal injury and mild traumatic brain injury in general. Currently, viscous effects in the brain are modelled using Prony-series, i.e., a sum of decaying expone

Near‑Infrared Photobiomodulation in White‑Matter Disease: From Microglial States to Measurable Endpoints.

White-matter (WM) injury contributes to disability across multiple sclerosis, traumatic brain injury, Alzheimer's disease and related dementias, and small-vessel disease. We use microglial state programs as an organizing axis for WM injury-to-repair logic, while emphasizing that WM outcomes are multicellular and involve oligodendrocyte-lineage cells, astrocytes, axons/neurons, and vascular factors. Microglia span an injury-repair continuum, from inflammatory programs that increase oxidative stress and debris burden to repair-competent programs that support debris handling, remyelination, and a

Preserved Limbic White Matter Predicts Favorable 2-Year Outcomes in Neonatal Hypoxic-Ischemic Encephalopathy.

OBJECTIVE: Accurate prediction of both favorable and unfavorable long-term neurodevelopmental outcomes of neonatal hypoxic-ischemic encephalopathy (HIE) is essential for clinical care, yet the brain structures underlying a favorable prognosis remain unclear. This study aimed to identify diffusion magnetic resonance imaging (MRI) measures that can predict the 2-year outcomes in neonates with HIE. METHODS: We analyzed whole-brain diffusion MRI scans from a retrospective cohort of 40 neonates with HIE, acquired within 14 days of birth. Diffusion measures were quantified across 16 cerebrum region

Hypoxic-ischemic brain injury: imaging findings from birth to adulthood.

Global hypoxic-ischemic injury (HII) to the brain is a significant cause of mortality and severe neurologic disability. Imaging plays an important role in the diagnosis and treatment of HII, helping guide case management in the acute setting and providing valuable information about long-term prognosis. Appropriate radiologic diagnosis of HII requires familiarity with the many imaging manifestations of this injury. Factors such as brain maturity, duration and severity of insult, and type and timing of imaging studies all influence findings in HII. Severe hypoxia-ischemia in both preterm and ter

Embedded finite element modeling of the mechanics of brain axonal fiber tracts under head impact conditions.

Investigating and understanding the biomechanical kinematics and kinetics of human brain axonal fibers during head impact process is crucial to study the mechanisms of Traumatic Axonal Injury (TAI). Such a study may require the explicit incorporation of brain fiber tracts into the host brain in order to distinguish the mechanical states of axonal fibers and brain tissue. Herein we extend our previously developed human head model by using an embedded element method to include fiber tracts reconstructed from diffusion tensor images in a host brain with the purpose of numerically tracking the def

Mutism after closed head injury.

Prospective study of patients admitted to a hospital for closed head injury showed that nine patients (nearly 3%) became mute for varying periods despite recovery of consciousness and communication through a nonspeech channel. Computed tomography (CT) showed subcortical lesions situated primarily in the putamen and internal capsule of four patients, whereas four of the five patients without subcortical lesions had left-hemisphere cortical injury. The patients without subcortical injury visualized by CT exhibited a longer duration of impaired consciousness consistent with severe diffuse brain i

Traumatic axonal injury influences the cognitive effect of non-invasive brain stimulation.

Non-invasive brain stimulation has been widely investigated as a potential treatment for a range of neurological and psychiatric conditions, including brain injury. However, the behavioural effects of brain stimulation are variable, for reasons that are poorly understood. This is a particular challenge for traumatic brain injury, where patterns of damage and their clinical effects are heterogeneous. Here we test the hypothesis that the response to transcranial direct current stimulation following traumatic brain injury is dependent on white matter damage within the stimulated network. We used

Effect of different helmets against ground impact based on the in-depth reconstruction of electric two-wheeler accidents.

Skull fracture and brain injury are frequent head injuries in electric two-wheeler (ETW) accidents, and the type of helmet and impact conditions affect the effectiveness of the helmet in protecting the rider's head. The purpose of this study was to conduct in-depth reconstructions of rider's head-to-ground impacts in ten ETW accidents by using a multi-body system combined with a finite element approach and to evaluate the effect of two typical full-face helmets (FFH) and one half-coverage helmet (HCH) through head accelerations and intracranial biomechanics injury metrics in ground impacts. Regional specificity of magnetic resonance imaging and histopathology following cerebral ischemia in preterm fetal sheep.

Early and accurate evaluation of the nature and extent of cerebral injury in the preterm infant brain is important for prognostication and decision making in the neonatal intensive care unit. The capability of magnetic resonance imaging (MRI) to define acute ischemic changes in white and gray matter in comparison to contemporaneous histopathology has not been adequately ascertained. The aim of this study is to assess whether postmortem MRI predicts the nature and extent of brain injury in a preterm fetal sheep model of cerebral hypoperfusion. MRI examinations were performed on fetal sheep brai

The significance of beta-APP immunoreactivity in forensic practice.

The neuropathologist involved in forensic work is not uncommonly confronted with a case in which there is no or only a limited history or, if available, the information is uncertain or is often conflicting. In recent years the immunohistochemical stain beta-amyloid precursor protein (beta-APP) has been used to assess the extent of axonal injury in a variety of pathological processes but in forensic practice is of greatest utility in the assessment of traumatic brain injury. Diffuse traumatic axonal injury (TAI) in humans has been demonstrated by beta-APP immunoreactivity in patients surviving

The relationship among H2S, neuroinflammation and MMP-9 in BBB injury following ischemic stroke.

Blood-brain barrier (BBB) is located at the interface between the central nervous system (CNS) and the circulatory system, which maintains the microenvironmental homeostasis of the CNS. BBB damage is a result of CNS diseases, including ischemic stroke, and is a cause of CNS deterioration. Cerebral ischemia unleashes a profound inflammatory response to remove the damaged tissue in the CNS and prepare the brain for repair. However, the excessive neuroinflammation following stroke onset is associated with BBB breakdown, resulting in neuronal injury and worse neurological outcomes. Additionally, m

Neuroinflammation Following Traumatic Brain Injury: Take It Seriously or Not.

Traumatic brain injury (TBI) is associated with high mortality and disability, with a substantial socioeconomic burden. With the standardization of the treatment process, there is increasing interest in the role that the secondary insult of TBI plays in outcome heterogeneity. The secondary insult is neither detrimental nor beneficial in an absolute sense, among which the inflammatory response was a complex cascade of events and can thus be regarded as a double-edged sword. Therefore, clinicians should take the generation and balance of neuroinflammation following TBI seriously. In this review,

Diffuse axonal injury in children: clinical correlation with hemorrhagic lesions.

An inception cohort of 40 children and adolescents with traumatic brain injury and suspected diffuse axonal injury were studied using a new high-resolution magnetic resonance imaging susceptibility-weighted technique that is very sensitive for hemorrhage. A blinded comparison was performed between the extent of parenchymal hemorrhage and initial clinical variables as well as outcomes measured at 6 to 12 months after injury. Children with lower Glasgow Coma Scale scores (< or =8, n = 30) or prolonged coma (>4 days, n = 20) had a greater average number (p = 0.007) and volume (p = 0.008) of hemor

MR imaging and outcome in neonatal HIBD models are correlated with sex: the value of diffusion tensor MR imaging and diffusion kurtosis MR imaging.

OBJECTIVE: Hypoxic-ischemic encephalopathy can lead to lifelong morbidity and premature death in full-term newborns. Here, we aimed to determine the efficacy of diffusion kurtosis (DK) [mean kurtosis (MK)] and diffusion tensor (DT) [fractional anisotropy (FA), mean diffusion (MD), axial diffusion (AD), and radial diffusion (RD)] parameters for the early diagnosis of early brain histopathological changes and the prediction of neurodegenerative events in a full-term neonatal hypoxic-ischemic brain injury (HIBD) rat model. METHODS: The HIBD model was generated in postnatal day 7 Sprague-Dawley r

Value of electroencephalographic monitoring in newborns with hypoxic-ischemic encephalopathy treated with hypothermia.

BACKGROUND: The values of electroencephalography (EEG) in neonatal hypoxic-ischemic encephalopathy (HIE) during therapeutic hypothermia (TH) are still uncertain. AIMS: The aim of this study is to detect EEG background, the prevalence of seizures during cooling, and to determine different EEG patterns that can predict brain injury in magnetic resonance imaging (MRI). PATIENTS AND METHODS: Thirty-nine newborns with HIE were subjected to TH. Continuous monitoring by video-EEG was carried out throughout cooling and during rewarming. MRI was done for all newborns after rewarming. The predictive v

Multi-site, in vivo MRI dataset of brain diffusivity measures before and after harmonization, and atrophy measures following controlled cortical impact in male and female adult rats.

Traumatic brain injury (TBI) research faces persistent challenges in data comparability and reproducibility, particularly in multi-center preclinical studies. Structured, interoperable datasets are essential to identify robust imaging biomarkers and validate cross-site findings. This dataset comprises 343 diffusion-weighted MRI scans from 186 male and female Sprague Dawley rats subjected to controlled cortical impact (CCI) or sham procedures at four independent research sites. Imaging was performed at 3 and 30 days post-injury using harmonized acquisition protocols and field strengths ranging

The neural basis of impaired self-awareness after traumatic brain injury.

Self-awareness is commonly impaired after traumatic brain injury. This is an important clinical issue as awareness affects long-term outcome and limits attempts at rehabilitation. It can be investigated by studying how patients respond to their errors and monitor their performance on tasks. As awareness is thought to be an emergent property of network activity, we tested the hypothesis that impaired self-awareness is associated with abnormal brain network function. We investigated a group of subjects with traumatic brain injury (n = 63) split into low and high performance-monitoring groups bas

Susceptibility of Cervical Spinal Stenosis to Hypoxic-Ischemic Cord Injury.

BACKGROUND: The adult spinal cord is typically resistant to hypoxic-ischemic injury because of collateral blood supply; however, congenital or acquired stenosis may result in baseline maximal vasodilation, such as superimposed hemodynamic stresses, that cannot be accommodated, leaving the spinal cord vulnerable to ischemic injury. We present a rare case of spinal cord hypoxic-ischemic injury in an adult with underlying cervical spinal stenosis. CASE DESCRIPTION: A 37-year-old man with a history of morbid obesity, diabetes mellitus, hypertension, and obstructive sleep apnea presented after dev
