Glioblastoma multiforme (GB) is the most common and most malignant primary intracranial tumor. Of the 577 patients who underwent surgery for newly diagnosed GB (World Health Organization grade IV) between January 1991 and March 2008 at our department, seven had infratentorial GB (iGB) (incidence 1.2%). Patients younger than 21years of age, as well as patients with gliomatosis cerebri, were excluded from the analysis. We concluded that iGB is rare in adults. Because of its rarity and the non-specific radiological features of iGB, it can easily be misdiagnosed as a brain metastasis, ependymoma o

Surgery of posterior fossa tumors is afflicted with some important problems. One of these is the postoperative communicating hydrocephalus. According to the literature this form of hydrocephalus is found in 10-30% of all children operated on; but a permanent shunt is necessary in only 8-18%. Aim of the present investigation was to find out disturbances of cerebrospinal fluid (CSF)--circulation and--reabsorption after posterior fossa surgery. Therefore 25 children ranging in the age from 3 to 12 years were examined on the basis of isotope cisternographic studies 8 to 21 after the operation. In

Fifteen patients with internuclear ophthalmoplegia (INO) were examined by simultaneous electro-oculography of each eye. In 11 patients the INO was bilateral and in 4 patients unilateral. In 6 of the bilateral cases, the INO was asymmetric. A classic presentation of the INO was shown by only 2 patients. The rest had additional disorders of eye mobility (INO plus). The high percentage of additional affections, which were not often clinically apparent, consisted of slowing of abduction (45%), impairment of vertical movements (80%) and convergence (30%), and dysmetric overshoot of the abducing eye

OBJECTIVE: Brainstem cavernous malformations (BCMs) are low-flow vascular lesions of the central nervous system that pose significant surgical challenges due to their deep location and proximity to eloquent structures. Resection is associated with significant morbidity, and published literature is sparse. This single-institutional, single-surgeon study aimed to delineate factors affecting outcomes in the resection of BCMs and to assess the efficacy of the omnidirectional CO2 laser for lesion resection.
METHODS: A retrospective review of all BCMs treated between 1990 and 2022 at Stanford Hospi

OBJECTIVE: The aim of this study was to describe the role and long-term outcomes of high-definition fiber tractography (HDFT) in the surgical management of brainstem cavernomas.
METHODS: The authors performed a retrospective evaluation of their database at the HDFT laboratory in a single academic institution.
RESULTS: The authors identified 11 patients with brainstem cavernomas who had HDFT for preoperative workup and underwent microsurgical resection. The mean patient age was 39 years (range 20-76 years), and the mean follow-up was 75.2 months (range 37-149 months). Four cavernomas were loc

OBJECT: The authors present their experience with the 17th pediatric intramedullary cavernoma reported in English literature.
METHODS: The patient firstly underwent surgery for a left frontal cavernoma when he was 2 years old. Also the child's mother was operated for a C2-C3 intramedullary cavernoma. He grew up normally and the radiological follow-up was negative for other brainstem cavernous malformations. When he was 11 years old he complained a worsening tetraparesis. A cerebral and spinal magnetic resonance (MR) imaging revealed the presence of a C1 intramedullary cavernoma and a pontine

OBJECTIVE: Tectal plate gliomas are rare, slow-growing tumors of the midbrain that are discovered predominantly in the pediatric population. Because of their indolent nature, treatment mainly consists of observation and management of hydrocephalus. Unfortunately, a subset of tectal gliomas may exhibit tumor enlargement and disease progression. Currently, there are no established guidelines for predicting future progression of tectal gliomas or the need for tumor-directed treatment. In this paper, the authors present a large case series of tectal plate gliomas with the aim of determining early

In this work a prioritized optimization algorithm is adapted and applied to treatment planning for intensity modulated proton therapy (IMPT). Originally, this algorithm was developed for intensity modulated radiation therapy (IMRT) with photons. Prioritized optimization converts the clinical hierarchy of treatment goals into an effective optimization scheme for treatment planning. It presents an alternative to conventional methods that combine all optimization goals into a single optimization run with a weighted sum of all planning aims in the objective function. The highest order goal in the

The leading cause of brain cancer-related death in children is diffuse midline glioma (DMG). A particularly aggressive DMG subtype is pontine DMG (formerly diffuse intrinsic pontine glioma, DIPG), which is caused by the histone mutation H3.3K27M. Because of its diffuse growth and location in a critical brainstem structure, therapeutic options are limited, and pontine DMG is considered universally fatal. The lack of appropriate animal models has hindered our understanding of the developmental origins and progression of pontine DMG, which in turn has limited the development of effective therapeu

Glioblastoma remains a highly lethal primary brain tumor with limited therapeutic options. Zika virus (ZIKV) has emerged as a candidate oncolytic platform because of its tropism for glioblastoma stem-like cells and its immunomodulatory potential. We conducted a systematic review and multilevel meta-analysis of preclinical in vivo studies evaluating ZIKV-based therapy in glioblastoma. Six studies met the eligibility criteria, yielding 27 survival-relevant experimental clusters, of which 24 entered the primary quantitative synthesis. The primary analysis included 38 survival contrasts nested wit

Cavernous malformations (CM) in the ventral pons pose significant surgical challenges due to their deep anatomical location and complex neurovascular structures [1]. This report details the successful surgical management of a giant ventral pontine CM in a 38-year-old female exhibiting left-sided limb weakness (muscle strength Grade IV), and was approved by the ethics committee. Magnetic resonance imaging (MRI) findings indicated the CM extended from the pial surface predominantly towards the right side. Utilizing diffusion tensor imaging (DTI), we determined that the corticospinal tract was la

BACKGROUND: Tectal plate gliomas (TPGs) are a heterogeneous group of uncommon brain tumors. TPGs are considered indolent and are usually managed conservatively but they have the potential to transform into higher-grade tumors. The aims of this study were to investigate the natural history of adult TPG, treatment outcomes, and overall survival.
METHODS: A retrospective cohort analysis was performed of adult patients with TPG between 1993 and 2021. Baseline clinical, radiologic, and management characteristics were collected. The primary outcome was tumor progression, defined as increasing size

Immune checkpoint inhibitors (ICIs) have revolutionized cancer treatment, but they have been known to cause immune-related adverse events (irAEs) by promoting T-cell activation. Neurological irAEs are rare (1%) but have a high fatality rate (11.5%). Here we report the first case of Bickerstaff brainstem encephalitis (BBE) induced by an ICI. A woman in her 60s with metastatic breast cancer was treated with atezolizumab plus nab-paclitaxel once intravenously. Eighteen days later, she lost consciousness with ophthalmoplegia and was diagnosed with a neurological irAE. She recovered consciousness i

Hemangioblastoma is a rare tumor of vascular origin, most commonly located in the posterior fossa, which presents with severe symptoms and usually very hard to resect without remarkable operative blood loss.1-2 Pre-operative embolization may decrease the amount of intra-operative bleeding, but the endovascular treatment of such tumor may be very challenging due to the high risk of infarction of the surrounding tissues. Direct puncture embolization has been developed to overcome many of the limitations of endovascular techniques for many hypervascular lesions, also hemangioblastomas.3-5 We pres

Diffuse intrinsic pontine glioma (DIPG) is an aggressive incurable brainstem tumor that targets young children. Complete resection is not possible, and chemotherapy and radiotherapy are currently only palliative. This study aimed to identify potential therapeutic agents using a computational pipeline to perform an in silico screen for novel drugs. We then tested the identified drugs against a panel of patient-derived DIPG cell lines. Using a systematic computational approach with publicly available databases of gene signature in DIPG patients and cancer cell lines treated with a library of cli

Glioblastoma (GBM) is the most aggressive type of brain tumor that originates from glioblastoma stem cells (GSCs). In the brain, GSCs are supported by a tumor microenvironment (TME) residing in the perivascular niche and the hypoxic niche. The GBM TME is highly heterogenous and exhibits complex cell-to-cell interactions. Three-dimensional tumorspheres cultured in stem cell-enriching media is often used as an in vitro model. The GBM tumorspheres retain some of the transcriptional and translational GSC features but often fails to recapitulate intertumor heterogeneity. Here, we developed a simple
