Mechanism of atrioventricular nodal facilitation in rabbit heart: role of proximal AV node.

The phenomenon of atrioventricular (AV) nodal "facilitation," described in traditional "black box"-functional studies, implies enhanced AV nodal dromotropic function. We investigated the role of atrial prematurities in the modulation of the nodal cellular responses in the mechanism of AV nodal facilitation. Atrial and His (H) bundle electrograms and microelectrode recordings from proximal AV nodal cells were analyzed in 15 superfused rabbit AV node preparations. The pacing protocol consisted of 30 basic beats (S1; coupling interval S1-S1 = 300 ms) followed by a facilitating prematurity (S2; co

Transcatheter modification of the atrioventricular node using radiofrequency energy.

120 consecutive patients with symptomatic atrioventricular nodal reentrant tachycardia (AVNRT) underwent catheter ablation using radiofrequency energy. Fast pathway ablation was attempted in the first 16 consecutive patients by application of radiofrequency current in the anterior and superior aspect of the tricuspid annulus. Successful results were accomplished in 13 patients, complete AV block occurred in three. The other 104 patients initially underwent ablation of the slow pathway in the posterior and inferior aspects of the tricuspid annulus which was successful in 98 patients. The remain

Direct current cardioversion of established atrial fibrillation.

Restoration and maintenance of sinus rhythm is a desirable end point in patients with chronic atrial fibrillation. If clinically indicated, external cardioversion using direct current is the method of choice in most centers. The results range from 20 to 90% and are highly influenced by the underlying etiology. A number of determinants including age, duration of atrial fibrillation, and size of left atrium are usually taken into account in the indications but have been recently questioned. The use of prophylactic antiarrhythmic therapy has been shown to reduce the recurrence of atrial fibrillat

[Transvenous high frequency current ablation of atrioventricular conduction in therapy refractory tachycardic atrial fibrillation].

We present a case of radiofrequency ablation of the atrioventricular conduction system by means of intravenous catheter technique in a patient with drug-refractory paroxysmal atrial fibrillation. Total AV block was produced. Since the ablation and implantation of a pacemaker this female has had no recurrences. The advantages of this method compared with electrical fulguration are better control of delivered energy and reduction of complications (catheter perforation, thromboembolism). The disadvantage of radiofrequency ablation is the difficulty in positioning the catheter so as to give the mo

Catheter-mediated electrical ablation of the posterior septum via the coronary sinus: electrophysiologic and histologic observations in dogs.

In a series of 12 dogs, the electrophysiologic and histologic effects of a single damped sine-wave shock delivered via standard electrocatheters to the region of the coronary sinus orifice were investigated. Six dogs received 200 J and six received 360 J of stored energy. The shock was delivered to two consecutive proximal poles of a standard quadripolar catheter positioned at the coronary sinus orifice and connected to the positive output (anode) of a defibrillator. A disc electrode positioned on the anterior chest wall served as the cathode (negative pole). During the shock, voltage and curr

Maturational changes in ventriculoatrial conduction in the intact canine heart.

This study reports on the changes in ventriculoatrial (VA) conduction that occur with maturation. Programmed atrial and ventricular premature extra-stimulation (coupled to a fixed paced cycle length) and rapid atrial pacing were performed in three groups of dogs: Group I = 8 neonates aged 5 to 14 days, Group II = 9 young dogs aged 6 to 9 weeks and Group III = 10 adult dogs. High right atrial, His bundle and right ventricular electrograms were recorded. There were no differences in the AH intervals at rest. In all but five animals, atrioventricular conduction was limited by the atrial functiona

[Role of accessory atrioventricular pathways in the genesis of permanent junctional tachycardia. Apropos of 2 cases].

Electrophysiological studies were performed in two patients with permanent junctional tachycardias. In both cases, the heart rate was 160/mn and standard ECG showed negative P waves in leads D2, D3 and AVF, isobiphasic or negative P waves in the left precordial leads and a PR interval of 0.12 and 0.13 secs. respectively. The tachycardia started without lengthening of the PR interval or preceding extrasystoles. Shortening of the sinus cycle before the tachycardia was inconstant. The reciprocating mechanism was shown by stimulation studies. Premature ventricular depolarisations not conducted to

Atrial reentry in chronic repetitive supraventricular tachycardia.

Atrial reentrance as a mechanism of the tachycardia was demonstrated in a 28-year-old patient suffering from chronic repetitive supraventricular tachycardia. Criteria for diagnosis included the following: (1) Repetitive supraventricular tachycardia was induced and terminated by properly timed atrial extrastimuli. (2) Return cycles of all atrial extrastimuli not abolishing the tachycardia were fully compensatory. (3) A-H prolongation was not a prerequisite to induce the tachycardia. (4) The contours of P and A waves during tachycardia differed from those in sinus rhythm, but atrial activation r

Evidence suggesting dual A-V nodal pathways in patients without supraventricular tachycardias.

Electrophysiologic evidence for dual pathways of conduction through the A-V node is presented in three patients without history of supraventricular tachycardia. In case 1, abrupt spontaneous changes in the PR interval from 0.17 to 0.42 second were seen. His bundle electrographic studies showed two sets of A-H intervals during sinus rhythm and at several atrial pacing rates, although at rates over 100 per minute only the slow pathway conducted. Using the extrastimulus method, different refractory periods for the fast and slow pathways were documented. Cases 2 and 3 underwent His bundle electrog

Atrioventricular Delay Optimization in His-Optimized Cardiac Resynchronization Therapy: A Case Report.

His bundle pacing (HBP) has recently emerged as a physiologic pacing strategy that preserves ventricular synchrony and may improve outcomes in patients with heart failure and conduction abnormalities. Optimization of the atrioventricular (AV) delay plays a pivotal role in ensuring hemodynamic efficiency in patients with conduction system pacing. We report a 41-year-old male with dilated cardiomyopathy and severe heart failure (NYHA class IV, EF 16%), who was initially implanted with a cardiac resynchronization therapy (CRT) device. Despite multiple attempts to reposition the right and left ven

Electrophysiologic and histologic observations of chronic atrioventricular block induced by closed-chest catheter desiccation with radiofrequency energy.

Direct-current or laser energy has been used to induce atrioventricular (AV) block, but certain complications associated with this type of energy have been reported. We have previously documented that radiofrequency (RF) energy can effectively and safely induce acute AV block in closed-chest dogs during the 4-7 days of follow-up. This study was undertaken to determine if the ablation was permanent and to define the chronic pathology and site of AV block. Complete AV block was successfully achieved in four dogs immediately after ablation with a bipolar "standard" RF output (750 kHz) delivered b

[The effect of ouabain on sinus node function and intracardiac conduction (author's transl)].

The effect of 0.5 mg Ouabain (O) i.v. on sinus node function and on intracardiac conduction was studied by His bundle electrography and atrial stimulation. 7 patients with normal sinus node function and av conduction (group I), 5 patients with sick sinus syndrome (group II), and 5 patients with av block of changing degree (group III) were examined. 2 additional patients had simultaneous sinus node dysfunction and av block (group II/III). The sinus node recovery time was prolonged in 6 patients, the av block was located proximal to the His bundle in 4 patients and distal to the His bundle in 3

Physiologic Variants of Cardiac Conduction (Aberration, Gap, Supernormal Conduction).

Wide QRS complexes during supraventricular rhythms can be caused by fixed bundle branch block, functional (intermittent) bundle branch block, preexcitation, or toxic/metabolic causes. Functional bundle branch block can be caused by long-short aberrancy (usually physiologic), or acceleration/deceleration dependent aberrancy (usually pathologic). Electrocardiogram criteria have been proposed to differentiate aberration from ventricular tachycardia; they are not always accurate. The gap phenomenon "paradox" is that with increasingly premature extrastimuli, progressive proximal conduction delay al

Outcomes of Left Bundle Branch Area Pacing in Heart Failure Patients: A Systematic Review.

BACKGROUND: Left bundle branch area pacing (LBBAP) is an emerging physiological pacing technique that restores ventricular electrical synchrony by directly engaging the left conduction system. It has been proposed as an alternative to conventional pacing strategies, particularly in heart failure patients with reduced left ventricular ejection fraction (LVEF ≤ 50%). METHODS: A systematic literature search of PubMed, MEDLINE, and Scopus was conducted up to December 2024 in accordance with PRISMA guidelines. Sixteen studies involving 5680 patients were included. Reported outcomes included change

Left bundle area pacing in hypertrophied hearts: An ex vivo ovine model to study deployment of pacing leads in thick septum.

BACKGROUND: Knowledge of lead biomechanics in a hypertrophied interventricular septum (IVS) may improve the success rate of left bundle branch area pacing (LBBAP) in these patients. OBJECTIVES: We sought to investigate the biomechanics of lead penetration in an ex vivo ovine model of a hypertrophied IVS and compare the behavior of lumenless leads (LLL) vs stylet-driven leads (SDL) in terms of torque transfer and penetrating capacity. METHODS: Twenty fresh ex vivo ovine interventricular septae were excised to create a virtual hypertrophied IVS model by having 2 septae sandwiched together and

Distal His Pacing Promotes Reverse Remodeling in a Child With Conventional Pacing-Induced Ventricular Dysfunction.

Children with indications of pacing often require epicardial pacemakers with generators implanted in the abdominal wall due to small body size or challenging anatomy because of congenital heart diseases. However, left ventricle (LV) systolic dyssynchrony induced by epicardial pacing may result in adverse remodeling and LV dysfunction. Recently, distal His bundle pacing (dHBP) has been shown to restore normal ventricular function in adult patients with pacemaker induced ventricular dysfunction. Nevertheless, the effects and relevance of HBP in pediatric patients are largely unresolved, with onl
