Postprandial glycaemic response and pain sensitivity in breast cancer survivors suffering from chronic pain: a double-blind, randomised controlled cross-over pilot experiment.

INTRODUCTION: The study's primary goal is to investigate differences in postprandial glycaemic response (PPGR) to beverages with varying glycaemic index (i.e. low and medium) between breast cancer survivors (BCS) with chronic pain and healthy pain-free controls (HC). The secondary goal of the study is to investigate the potential link between PPGR and pain-related outcomes in BCS with chronic pain. METHODS: In this study, 15 BCS and 15 HC were included. After 12 h of fasting, subjects were randomised between drinking a beverage made with 50 g of sucrose (medium) or isomaltulose (low) within 2

Effect of pain neuroscience education after breast cancer surgery on pain, physical, and emotional functioning: a double-blinded randomized controlled trial (EduCan trial).

Pain is one of the most common and long-lasting side effects reported by women surgically treated for breast cancer. Educational interventions may optimize the current physical therapy modalities for pain prevention or relief in this population. Pain neuroscience education (PNE) is an educational intervention that explains the pain experience not only from a biomedical perspective but also the psychological and social factors that contribute to it. Through a double-blinded randomized controlled trial (EduCan trial) it was investigated if PNE, in addition to the standard physiotherapy program i

Pulsed radiofrequency in clinical practice - A retrospective analysis of 238 patients with chronic non-cancer pain treated at an academic tertiary pain centre.

BACKGROUND AND AIMS: Pulsed radiofrequency is a non-neurodestructive invasive pain treatment which, in contrast to conventional continuous radiofrequency treatment, does not entail nerve tissue destruction. The aim of this study was to retrospectively analyse the short-term benefits of a broad use of pulsed radiofrequency in clinical practice. METHODS: The medical records of all patients treated with pulsed radiofrequency, or who received a diagnostic test block with a local anaesthetic in view of such a treatment, were retrospectively analysed. The patients had been referred to a tertiary pa

Association of sexual pain and psychological factors among gynecologic and breast cancer patients: application of components of the fear-avoidance model of chronic pain.

A significant number of gynecologic and breast cancer survivors report chronic issues with pain during sexual activity. The fear-avoidance (FA) model of chronic pain provides a potential framework for addressing chronic sexual pain. The purpose of this study is to investigate the relationships among components of the FA model (acute pain, anxiety, avoidance, and distress) among gynecologic and breast cancer survivors to help identify those who may be at risk for chronic sexual pain. Gynecologic and breast cancer patients (n = 97) completed baseline questionnaires as part of a psychosexual inte

A meta-ethnography of how children and young people with chronic non-cancer pain and their families experience and understand their condition, pain services, and treatments.

BACKGROUND: Chronic non-cancer pain in childhood is widespread, affecting 20% to 35% of children and young people worldwide. For a sizeable number of children, chronic non-cancer pain has considerable negative impacts on their lives and quality of life, and leads to increased use of healthcare services and medication. In many countries, there are few services for managing children's chronic non-cancer pain, with many services being inadequate. Fourteen Cochrane Reviews assessing the effects of pharmacological, psychological, psychosocial, dietary or physical activity interventions for managing…

Pain, numbness, or both? Distinguishing the longitudinal course and predictors of positive, painful neuropathic features vs numbness after breast cancer surgery.

INTRODUCTION: Both positive (burning, stabbing, and allodynia) and negative (numbness) neuropathic symptoms may arise after surgery but likely contribute differently to patients' postoperative pain experience. Numbness has been identified as divergent from positive neuropathic symptoms and therefore excluded from some neuropathic assessment tools (Neuropathic Pain Scale for PostSurgical patients [NeuPPS]). OBJECTIVES: In this prospective longitudinal study of patients undergoing breast surgery, we aimed to delineate the time course of numbness and its coincidence with NeuPPS and to contrast t

Methadone dose escalation in patients with opioid use disorder and cancer as a strategy for controlling cancer-related pain: A case series.

OBJECTIVES: Opioid use disorder (OUD) and cancer gained attention as co-occurring diseases in the last 2 decades due to the possible relationship between opioid prescriptions for cancer pain and the risk of developing substance use disorder in cancer patients. However, little is known about patients previously diagnosed with OUD who develop cancer and how to manage both OUD symptoms and control pain. METHODS: The present case series deals with this subpopulation and proposes a dose escalation of methadone to control both the cancer-related pain and drug addiction symptoms. RESULTS: This appr…

Tumor-Macrophage-Nerve interactions drive neuroinflammation and neuropathic pain in prostate cancer perineural invasion.

OBJECTIVE: Perineural invasion - a hallmark of cancers such as prostate and pancreatic - is strongly associated with severe, treatment-resistant pain. To dissect the neuron-specific mechanisms underlying this pain, we established a rat model of prostate cancer-associated perineural invasion to get insights into neuroinflammatory processes arising from direct tumor-nerve interactions and to provide a platform for evaluating targeted therapeutic strategies. METHODS: GFP-expressing AT-1 prostate cancer cells were directly microinjected into the perineurium of the sciatic nerve in syngeneic Copen

Opioid-specific risk of respiratory depression in non-cancer pain: a retrospective cohort study.

BACKGROUND: Opioids are associated with serious adverse outcomes, including premature death. Respiratory depression is among the most severe opioid-related events, yet data on its incidence in non-cancer pain remain limited. Pharmacological differences suggest varying respiratory risks across opioid drugs. This study evaluated the comparative risk of respiratory depression by opioid drug and dose, and the impact of concomitant gabapentinoids and benzodiazepines in hospitalised patients with non-cancer pain. METHODS: A retrospective cohort study was conducted using electronic health records fr…

Pain-Related Emergency Department Visits and Hospitalizations Following Hydrocodone Rescheduling in Metastatic Lung Cancer.

Background: Emergency department (ED) use for cancer-related pain is common, particularly among patients with metastatic disease who frequently require opioid analgesia. In October 2014, the U.S. Drug Enforcement Administration rescheduled hydrocodone combination products from Schedule III to Schedule II, introducing stricter prescribing and dispensing requirements. We evaluated whether hydrocodone rescheduling was associated with pain-related ED visits and hospitalizations among older adults with metastatic lung cancer. Methods: We conducted a retrospective SEER-Medicare cohort study of benef

Expert Consensus Develops Multidisciplinary Pathway for Cancer Pain Management in Italy: A Delphi Study.

BACKGROUND: Cancer pain remains highly prevalent and undertreated despite established guidelines. In Italy, Law 38/2010 mandates systematic pain assessment, yet only 26% of clinicians routinely evaluate pain at each clinical visit, and fewer than one-quarter have received formal training in pain medicine or palliative care. A national multidisciplinary roundtable, convened in Rome in March 2025, formally identified four systemic gaps-insufficient education, fragmented care pathways, unclear professional roles, and challenges in implementing shared diagnostic and therapeutic pathways-and planne…

Perineural invasion in pancreatic cancer: Current biological function in R status, prognosis, and pain.

Pancreatic ductal adenocarcinoma (PDAC) is predicted to become the second leading cause of death in 2030 and it is characterized by poor prognosis, recurrence and resistance to therapies. Several factors contribute to the complexity of this disease, among those the invasion of nerves by PDAC cells. This condition, defined as perineural invasion (PNI), is responsible of PDAC progression and pain generation. To date, PNI emerges as a hallmark feature of PDAC, showing the same oncological weight of lymph node metastasis in terms of prognosis. Targeting PNI could help improve prognosis and pain re

Spinal TREM2/PI3K/Akt signal axis elicits microglial activation and aggravates bone cancer pain.

BACKGROUND: The TREM2 signaling pathway is activated in multiple pain conditions; nevertheless, its role in the regulation of bone cancer pain remains largely unexplored. The persistent pathological progression of bone cancer pain induces microglial overstimulation, leading to the exacerbation of spinal central inflammation. These results confound the neurobehavioral symptoms and obscure the underlying mechanisms. Thus, the precise mechanism by which TREM2 induces bone cancer pain (BCP) has yet to be clarified. METHODS: Lewis lung carcinoma cells (LLC) were administered into the femoral bone …
