[Pathophysiological studies in moyamoya disease by rCBF and cortical artery pressure measurements in comparison to those in ICA or MCA occlusion].

The authors measured preoperative rCBF and intraoperative cortical artery pressure (CAP) during STA-MCA anastomosis to investigate cerebral hemodynamics in moyamoya disease. Six of 13 patients including 3 children showed ischemic attack and the remaining presented hemorrhagic attack. rCBF was measured by single photon ECT with Xe-133 inhalation technique. CAP's and rCBF's in moyamoya disease were compared to those in the 22 internal carotid artery (ICA) and 8 middle cerebral artery (MCA) occlusion. Systemic arterial blood pressure (SABP) was obtained at the radial artery. Mean rCBF in the MCA

Transient neurological events in childhood moyamoya disease.

OBJECTIVE: Patients sometimes experience transient neurological events (TNEs) during the early postoperative period after revascularization surgery for childhood moyamoya disease. The clinical background and pathophysiology of TNEs remain unclear. This study aimed to evaluate the incidence rate of TNEs and discuss the factors associated with pediatric moyamoya disease. METHODS: The authors retrospectively reviewed 110 cerebral hemispheres of 61 pediatric patients younger than 15 years who were treated from 2011 to 2020. All children underwent either simple double-direct or combined revascular

Encephaloduroarteriosynangiosis effectively reduces rebleeding risk in adults with hemorrhagic moyamoya disease: A long-term ambidirectional cohort study.

This cohort study was conducted using prospective and retrospective data to investigate whether indirect encephaloduroarteriosynangiosis (EDAS) revascularization can reduce the risk of rebleeding in adult patients with hemorrhagic moyamoya disease (hMMD). We enrolled adult patients diagnosed with hMMD prospectively and retrospectively. All patients voluntarily chose to undergo EDAS or conservative treatment. The clinical features, angiographic findings, and follow-up outcomes were investigated and analyzed using the Kaplan-Meier survival analysis and Cox proportional hazards regression models

Perioperative care of the pediatric patient for pial synangiosis surgery.

Moyamoya disease (MMD) is a cerebrovascular occlusive disorder which causes recurrent strokes and transient ischemic attacks in children. The arteriopathy of MMD targets the internal carotid arteries (ICA) and their branches resulting in ischemia of the ICA circulation. Small perforator branches of the ICA dilate to provide collateral perfusion to ischemic areas. This small vessel dilatation creates the characteristic angiographic appearance that gives the disease its name ("moyamoya" after the Japanese expression for "something hazy just like a puff of cigarette smoke drifting in the air"). Preoperative collateralization depending on posterior components in the prediction of transient neurological events in moyamoya disease.

Changes in local cerebral blood flow (CBF) are a major cause of transient neurological events (TNEs) after revascularization for moyamoya disease (MMD); however, the influence of preoperative collateral pathway development on TNEs has not yet been investigated. This study included 28 hemispheres from 28 consecutive patients with MMD who underwent surgical revascularization, including a superficial temporal artery (STA) to middle cerebral artery (MCA) bypass, between January 2014 and March 2022. The collateralization pathways included the anterior communicating artery (AcomA) collaterals, poste

Therapeutic treatment with vitamin C reduces focal cerebral ischemia-induced brain infarction in rats by attenuating disruptions of blood brain barrier and cerebral neuronal apoptosis.

Stroke is a major public health problem and ranks third most common cause of death in adults worldwide. Thrombolysis with recombinant tissue plasminogen activator and endovascular thrombectomy are the main revascularization therapies for acute ischemic stroke. However, ischemia-reperfusion injury, mainly caused by oxidative/nitrosative stress injury, after revascularization therapy can result in worsening outcomes. For better clinical prognosis, more and more studies have focused on the pharmaceutical neuroprotective therapies against free radical damage. The impact of vitamin C (ascorbic acid

Efficacy of revascularization surgery for moyamoya syndrome associated with Graves' disease.

Appropriate management of moyamoya syndrome associated with Graves' disease is undetermined because of the rarity of this combination. Patients tend to present with cerebrovascular events such as transient ischemic attack (TIA) in a thyrotoxic state, which is relieved by proper antithyroid therapy. Four patients with moyamoya syndrome associated with Graves' disease were successfully treated with revascularization surgery on 5 hemispheres among 58 consecutive patients (2-62 years old, mean 34.4 years) with moyamoya disease in 80 hemispheres treated from March 2004 to May 2007. Three patients p

Revascularization of the posterior circulation.

The primary objective of revascularization procedures in the posterior circulation is the prevention of vertebrobasilar ischemic stroke. Specific anatomical and neurophysiologic characteristics such as posterior communicating artery size affect the susceptibility to ischemia. Current indications for revascularization include symptomatic vertebrobasilar ischemia refractory to medical therapy and ischemia caused by parent vessel occlusion as treatment for complex aneurysms. Treatment options include endovascular angioplasty and stenting, surgical endarterectomy, arterial reimplantation, extracra

Impact of intraoperative cortical indocyanine green extravasation on local vasogenic edema immediately after direct revascularization in an adult with moyamoya disease: illustrative case.

BACKGROUND: Local vasogenic edema induced after direct revascularization in moyamoya disease (MMD) is associated with blood-brain barrier dysfunction, potentially leading to postoperative cerebral hyperperfusion (CHP) or delayed intracerebral hemorrhage. This phenomenon allows the leakage of fluids, proteins, and other substances from the blood vessels into the extracellular compartment. Typically, such edema is observed postoperatively rather than intraoperatively. OBSERVATIONS: A 48-year-old female with ischemic-onset MMD underwent revascularization on her left hemisphere with Suzuki's angi

Intrinsic functional brain connectivity is resilient to chronic hypoperfusion caused by unilateral carotid artery stenosis.

INTRODUCTION: Chronic cerebral hypoperfusion caused by asymptomatic high-grade stenosis of the internal carotid artery (ICA) has been associated with impaired cognitive function. Only few studies exist on underlying changes of functional connectivity (FC). METHODS: 20 patients with unilateral high-grade ICA stenosis without MRI lesions and 25 aged-match controls underwent resting-state functional MRI (rsfMRI) and neuropsychological assessment. Patients were examined within ten days before and 6-10 weeks after surgical or interventional revascularization of carotid stenosis. We examined mean r

Normalization of reduced functional connectivity after revascularization of asymptomatic carotid stenosis.

Internal carotid artery stenosis is a risk factor for ischemic stroke. Even in the absence of visible structural brain changes, patients with asymptomatic stenosis are prone to cognitive impairment. On a neuronal level, it was suggested that stenosis may lead to disturbed functional brain connectivity. If so, carotid revascularization should have an effect on hypothesized brain network disturbances. We studied functional connectivity in a motor network by resting-state electroencephalography in 12 patients with high grade asymptomatic carotid stenosis before and after interventional or surgica

Fluid Dynamics Simulation Analysis for Identifying Optimal Anastomosis Sites in Superficial Temporal Artery to Middle Cerebral Artery Bypass Surgery in Cerebrovascular Occlusive Disease.

This study investigated the hemodynamic effects of anastomosis site selection in superficial temporal artery to middle cerebral artery bypass surgery using computational fluid dynamics based on three-dimensional cerebral vascular models. By noninvasively reproducing blood flow changes that are difficult to evaluate in clinical settings, we visualized and quantified the effects of different bypass locations under both normal and stenotic conditions. To our knowledge, this is the first study to noninvasively evaluate the validity of anastomosis site selection in superficial temporal artery to mi

Superficial Temporal Artery-to-M2 Bypass and Clip Trapping of a Ruptured Fusiform MCA Aneurysm.

Fusiform intracranial aneurysms (IAs) can be challenging to treat, especially when ruptured.1-3 Clip trapping and bypass is an important treatment modality for complex IAs not amenable to endovascular approaches (i.e., stent-assisted coiling, flow diversion), which can be limited by low occlusion rates and anatomical constraints (e.g., tortuosity, proximity to branching vessels).4,5 Here we highlight the treatment considerations and technical steps for clip trapping and intracranial bypass of a previously ruptured middle cerebral artery fusiform aneurysm. Challenges in Using the Posterior Inferior Cerebellar Artery for Revascularization of the Anterior Inferior Cerebellar Artery: A Microsurgical Anatomic Study.

BACKGROUND: A bypass is usually required to prevent ischemia during the treatment of anterior inferior cerebellar artery (AICA) aneurysms. The intracranial (IC)-to-IC bypass provides several advantages over the extracranial-to-IC bypass in the posterior fossa. However, there are only 2 case reports about AICA revascularization with the posterior inferior cerebellar artery (PICA). We aimed to investigate the microsurgical anatomical challenges for PICA to AICA anastomosis. METHODS: Ten cadaveric heads injected with colored silicone were inspected on both sides using a lateral transcondylar app
