The Double-Deck Cervical Paraspinal Interfascial Block: Combined Cervical Cervicis Plane and Lower Cervical Erector Spinae Plane Blocks for C1-C2 Fusion Under SSEP/MEP Monitoring-A Case Report.

Analgesia for craniovertebral junction (CVJ) and high cervical posterior surgery is challenging because surgical exposure disrupts multilevel posterior cervical musculature and irritates the upper cervical dorsal rami, particularly C2-C3. Procedures performed with somatosensory evoked potential (SSEP) and motor evoked potential (MEP) monitoring also favor posterior, motor-sparing regional techniques. We describe a "double-deck" strategy combining an upper cervical cervicis plane (CCeP) block with a lower cervical erector spinae plane (ESP) block for posterior C1-C2 fusion performed under multi

Rare early-onset radiation-induced lumbosacral plexopathy in cervical cancer: a case report.

INTRODUCTION: Radiation-induced lumbosacral plexopathy (RILSP) is a rare and debilitating complication of pelvic radiotherapy, classically presenting months to decades after treatment. Very early onset within weeks is exceedingly rare and may be misattributed to tumor recurrence or other etiologies. We report a case of rare early-onset RILSP that showed a remarkable response to corticosteroid therapy, highlighting crucial diagnostic and therapeutic considerations. CASE PRESENTATION: A 46-year-old cervical cancer patient, who developed symptoms nine weeks after radiotherapy (a total dose of 45

Communication of the transverse cervical nerve with the marginal mandibular nerve: a previously unreported anatomical variant.

Communications between the transverse cervical nerve (C2 and 3) and the cervical branch of the facial nerve have been reported. We describe a case in which the transverse cervical nerve joined the marginal mandibular branch of the facial nerve. It was found during a selective neck dissection and to our knowledge is the first report of such a communication. We discuss the clinical relevance particularly in relation to confusion with the marginal mandibular branch of the facial nerve when operating in the submandibular region.

A novel modification of the ansa to recurrent laryngeal nerve reinnervation procedure for young children.

Vocal ford paralysis is a condition often seen in otolaryngologic adult and pediatric clinics. We report a case we believe to be the youngest child to undergo ansa cervicalis (ansa) to recurrent laryngeal nerve (RLN) reinnervation for unilateral vocal fold paralysis. We have included the preoperative and postoperative videostroboscopic and acoustic findings. The acoustic data shows improvement and is consistent with the patient's improved voice quality. Most notably the patient's voice quality is less raspy and his volume has improved while subjectively requiring less effort.

Anatomic identification and functional outcomes of the nerve sparing Okabayashi radical hysterectomy.

OBJECTIVES: To clarify the anatomy necessary for the nerve sparing Okabayashi's radical hysterectomy, we meticulously separated the blood vessels and connective tissues to preserve the pelvic splanchnic nerve, the hypogastric nerve, and the bladder branch of the inferior hypogastric plexus under magnification (x2.5) during the Okabayashi radical hysterectomy. METHODS: Twenty-four patients (FIGO stage IB, n=22, and stage IIA, n=2) underwent meticulous nerve sparing radical hysterectomy during 2004 to 2006. Postoperative assessment of bladder function consisted of the time to (a) achieve a post

Brachioradial pruritus--a peculiar neuropathic disorder.

The case is presented of a middle-aged woman who suffered from lancinating itch on the dorsolateral aspect of the upper arm after a loco-regional injury, first on the right and later on the left side. Brachioradial pruritus (BRP) was diagnosed. Neurophysiologic examination was compatible with a neuropathy at the C5-C6 level, while a negative nerve root block supported an additional central impact. The presumed pathophysiology of BRP is discussed in terms of a neuropathic disorder. We suggest that damage from whatever cause from either the cutaneous nerves or from the more proximal sensory path

Man-in-the-barrel syndrome caused by cervical spinal cord infarction.

Acute brachial diplegia with normal findings of the legs, "man-in-the barrel" (MIB) syndrome, is generally thought to be caused by bilateral supratentorial brain lesions of the prerolandic cortical and subcortical area. We report 1 patient with a sudden onset of MIB syndrome with no supratentorial lesion but a hemodynamically induced atypical anterior spinal cord infarction after unilateral vertebral artery dissection. Thus, in MIB syndrome an infratentorial lesion site, including the cervical spinal cord, should also be considered.

Orthopedic conditions of the cervical spine and shoulder.

Orthopedic conditions of the cervical spine and shoulder can range from benign to life threatening. A careful history and examination are crucial to evaluate these children, and additional investigations may be necessary. The signs and symptoms in these conditions and the indications for referral are discussed.

[Ansa cervicalis to the adductor division of the recurrent laryngeal nerve anastomosis for unilateral vocal cord paralysis].

The main trunk of ansa cervicalis was anastomosed to the adductor division of the recurrent laryngeal nerve in seven patients to reinnvervate the vocal cord for unilateral vocal cord paralysis. Although the reinnervated vocal cord neither abducted nor adducted, it presented itself in midline for precise apposition with the nonparalyzed cord. Normal phonatory quality was achieved after the procedure. Electroacoustic analysis indicated that this technique might produce normal phonatory function in paralyzed larynx. Laryngeal electromyography demonstrated that the procedure seemed to induce satis

The involvement of the peripheral nervous system in biopsy proven active giant cell arteritis.

BACKGROUND: Peripheral nervous system (PNS) affection is an uncommon, sometimes life-threatening manifestation of giant cell arteritis (GCA). OBJECTIVE: To describe characteristics of neurological abnormalities of the PNS in GCA patients. METHODS: Eighty consecutive cases of biopsy proven GCA were studied. RESULTS: Three patients presented with subacute sensorimotor deficits abnormalities in the distribution of the arm plexus. In all cases PNS affection was the leading clinical symptom in addition to a typical clinical syndrome of cranial arteriitis. In one case MRI demonstrated diffuse sig
