"When the obvious brother is not there": political and cultural contexts of the orphan challenge in northern Uganda.

It is estimated that two million of Uganda's children today are orphaned primarily due to AIDS. While recognising the immense impact of HIV/AIDS on the present orphan problem, this article calls for a broader historic and cultural contextualisation to reach an understanding of the vastness of the orphan challenge. The study on which the article is based was carried out among the Langi in Lira District, northern Uganda, with a prime focus on the situation of orphans within the extended family system. The data were collected through ethnographic fieldwork (8 months); in-depth interviews with com

School Readiness and Early Childhood Education and Care Services Among Dual Language Learners.

IMPORTANCE: Dual language learners (DLL) (ie, children learning 2 or more languages) present lower school readiness than non-DLL children, putting DLL children at risk of later school difficulties and adverse outcomes. However, it is unclear whether participation in early childhood education and care (ECEC) services may reduce this gap. OBJECTIVE: To assess whether ECEC exposure may reduce the school readiness gap between DLL and non-DLL children in a population-based sample. DESIGN, SETTING, AND PARTICIPANTS: This census survey study was performed from February to May 2022 in the Canadian p

Effect of a Short, Animated Storytelling Video on Transphobia Among US Parents: Randomized Controlled Trial.

BACKGROUND: Parents play a pivotal role in supporting transgender and gender diverse (TGD) youth. Yet only 35% of TGD youth describe their home as a gender-affirming place. Lack of parental support contributes to recent findings that TGD youth are approximately three times more likely to attempt suicide than their cisgender peers. In contrast, parents' affirmation of their children's gender identity significantly improves their mental health outcomes, by reducing anxiety, depression, and suicidality. OBJECTIVE: Addressing the urgent need for effective, scalable interventions, this study evalu

Puerperal psychosis: associated clinical features in a psychiatric hospital mother-baby unit.

OBJECTIVE: Current psychiatric diagnostic systems do not regard puerperal psychosis as a separate entity. However, there is continuing debate about the validity and clinical utility of this concept. This paper aims to investigate the prognostic importance of a number of clinical features in a sample of patients with puerperal psychosis. METHOD: A retrospective case note study was conducted on 42 consecutive admissions to a mother-baby unit in a psychiatric hospital. Data were collected on a range of variables, and diagnoses made according to DSM-III-R and RDC criteria for puerperal psychosis.

Listening to the community: Using formative research to strengthen maternity waiting homes in Zambia.

BACKGROUND: The WHO recommends maternity waiting homes (MWH) as one intervention to improve maternal and newborn health. However, persistent structural, cultural and financial barriers in their design and implementation have resulted in mixed success in both their uptake and utilization. Guidance is needed on how to design a MWH intervention that is acceptable and sustainable. Using formative research and guided by a sustainability framework for health programs, we systematically collected data from key stakeholders and potential users in order to design a MWH intervention in Zambia that could

AI driven interactive extraction of tomographic ultrasound imaging sequences for anal sphincter defects.

Anal incontinence is common after childbirth, often due to structural anal sphincter injury, affecting up to 42% of postpartum women, with 6% experiencing stool loss. Exo-anal ultrasound aids in detecting anal sphincter injury through tomographic ultrasound imaging (TUI), but manual TUI sequence extraction is expert-dependent and time-intensive. In this international multi-centre study, we developed and independently evaluated the performance of an interactive AI-assisted pipeline for automated TUI positioning and alignment with real-time adjustability, from exo-anal ultrasound images acquired

The predictive relationship between parents' perceptions of physical activity and children's physical literacy.

Parents play an important role in children's physical literacy development (across cognitive, physical, affective, and behavioral domains) and physical activity participation. The purpose of this study was mainly to ascertain the predictive effects of parents' perceptions of physical activity (PPPA) on children' physical literacy and its four domains. Children (N = 195; Mage = 9.09 ± 1.08) from five classes at one primary school in Central China completed the simplified Chinese version of Canadian Assessment of Physical Literacy version two (CAPL-2). Their parents completed the PPPA questionna

Literature Review: Support for caregivers of children on the ketogenic diet for epilepsy and other neurologic disorders.

Epilepsy affects millions of people and when medications are insufficient to maintain seizure control, individuals are diagnosed with refractory epilepsy (RE). Medical ketogenic diet therapy (KDT), a diet high in fat and low in carbohydrates and sufficient in protein, is a well-established treatment for RE. However, compliance is one of the main reasons for discontinuation of KDT and, with pediatric RE patients, the ability of informal caregivers, typically family members, to maintain diet adherence is vital for successful KDT treatment. The central role that informal caregivers play for effec

Introduction of the pneumococcal conjugate vaccine in humanitarian and fragile contexts: Perspectives from stakeholders in four African countries.

Childhood pneumonia causes a significant burden of preventable child morbidity and mortality in Chad, Guinea, Somalia/Somaliland, and South Sudan. Leaders from these countries have committed to reducing this burden and are preparing to introduce the pneumococcal conjugate vaccine (PCV) into their immunization programs. To support long-term sustainability for expected PCV introductions in settings afflicted by prolonged humanitarian crises this research explores national stakeholders' perspectives on contextual factors that may influence optimal vaccine implementation. This qualitative study us

Child health and the implementation of Community and District-management Empowerment for Scale-up (CODES) in Uganda: a randomised controlled trial.

INTRODUCTION: Uganda's district-level administrative units buttress the public healthcare system. In many districts, however, local capacity is incommensurate with that required to plan and implement quality health interventions. This study investigates how a district management strategy informed by local data and community dialogue influences health services. METHODS: A 3-year randomised controlled trial (RCT) comprised of 16 Ugandan districts tested a management approach, Community and District-management Empowerment for Scale-up (CODES). Eight districts were randomly selected for each of t
