Challenges in Utilizing Telehealth for Chronic Pain.

PURPOSE OF REVIEW: Chronic pain in the USA has presented with higher prevalence rates among women, older adults, those unemployed, living in poverty, living in rural environments, and adults with public health insurance. The COVID-19 pandemic has heavily played into the biopsychosocial model of pain. Consequently, greater impacts have affected patients with mood disorders, opioid abuse, and chronic pain. Concurrently, telemedicine has become a popular vehicle during the COVID-19 pandemic in continuing to provide quality patient care. The purpose of this article is to review the benefits and ch

The frequency and complexity of pediatric hospitalizations to pediatric and adult departments in Germany.

BACKGROUND: Germany’s upcoming hospital reform aims to restructure inpatient services by assigning hospitals to defined “care groups” that determine which services they may provide in the future. While pediatric care will continue to receive infrastructural readiness funding, the reform eliminates a dedicated care group for specialized pediatric medicine. As a result, subspecialized pediatric services will no longer be distinctly designated or planned for. This has raised concerns about future access to specialized inpatient care for children and adolescents, given the complexity and diversity

Design and validation of a predictive model for determining the risk of developing fibromyalgia.

OBJECTIVES: Fibromyalgia is a prevalent disease of unknown aetiology and is difficult to diagnose. Despite the availability of the American College of Rheumatology criteria for diagnosis, it continues to be a challenge in the field of primary health care in terms of identifying individuals with susceptibility to developing the disease. The aim of this study is to design and validate a predictive model of fibromyalgia in subjects with a history of chronic pain. METHODS: This multicentre observational retrospective cohort study was performed on patients aged >18 years, who visited four primary

Delivery of health care for cardiovascular and metabolic diseases among people living with HIV/AIDS in African countries: a systematic review protocol.

BACKGROUND: People living with HIV (PLHIV) in African countries are living longer due to the rollout of antiretroviral drug therapy programs, but they are at increasing risk of non-communicable diseases (NCDs). However, there remain many gaps in detecting and treating NCDs in African health systems, and little is known about how NCDs are being managed among PLHIV. Developing integrated chronic care models that effectively prevent and treat NCDs among PLHIV requires an understanding of the current patterns of care delivery and the major barriers and facilitators to health care. We present a sys…

Chronic fatigue syndrome: 3 cases and a discussion of the natural history of attention-deficit/hyperactivity disorder.

Fatigue is commonly reported in the primary care setting; however, its cause is often unclear. This article presents 3 cases involving patients with chronic fatigue syndrome who responded poorly to treatment. After clinical evaluation, all patients were found to meet criteria for attention-deficit/hyperactivity disorder (ADHD) and underwent a standard regimen of a psychostimulant medication. After treatment with psychostimulants, the 3 patients reported improved symptoms of fatigue and pain, and cognitive and core ADHD symptoms. These cases suggest that ADHD and chronic fatigue syndrome (and p

More Than a Perioperative Surgical Home: An Opportunity for Anesthesiologists to Advance Public Health.

Public health and the medical specialty of anesthesiology have been closely intertwined throughout history, dating back to the 1800s when Dr. John Snow used contact tracing methods to identify the Broad Street Pump as the source of a cholera outbreak in London. During the COVID-19 pandemic, leaders in anesthesiology and anesthesia patient safety came forward to develop swift recommendations in the face of rapidly changing evidence to help protect patients and healthcare workers. While these high-profile examples may seem like uncommon events, there are many common modern-day public health issu

Routine use of clinical management guidelines in Australian general practice.

Significant gaps remain between recommendations of evidence-based guidelines and primary health care practice in Australia. This paper aims to evaluate factors associated with the use of guidelines reported by Australian GPs. Secondary analysis was performed on a survey of primary care practitioners which was conducted by the Commonwealth Fund in 2009: 1016 general practitioners responded in Australia (response rate 52%). Two-thirds of Australian GPs reported that they routinely used evidence-based treatment guidelines for the management of four conditions: diabetes, depression, asthma or chro

A model for interprofessional health disparities education: student-led curriculum on chronic hepatitis B infection.

BACKGROUND: Although health disparities are commonly addressed in preclinical didactic curricula, direct patient care activities with affected communities are more limited. PURPOSE: To address this problem, health professional students designed a preclinical service-learning curriculum on hepatitis B viral (HBV) infection, a major health disparity affecting the Asian/Pacific Islander (API) population, integrating lectures, skills training, and direct patient care at student-run clinics. SETTING: An urban health professions campus. METHODS: Medical and other health professional students at U

The impact of the global budget payment on healthcare fund expenditure in integrated delivery systems: evidence from China's county-level medical alliance reform.

BACKGROUND: The global budget payment is a key policy tool for incentivizing integrated healthcare delivery systems to pursue financial sustainability. However, there is little empirical evidence showing its effects. This study evaluates the impact of the health insurance global budget payment reform implemented under China's county-level integrated healthcare consortium model on the efficiency of health insurance fund expenditure. The study also examines the heterogeneous effects across different service types. METHODS: The study used panel data from L County, and constructed a quasi-natural

A machine learning model for predicting short-term in-hospital mortality in acute myocardial infarction with coexisting chronic obstructive pulmonary disease.

BACKGROUND: Patients with acute myocardial infarction(AMI)complicated by chronic obstructive pulmonary disease (COPD) in the intensive care unit (ICU) face a significantly elevated risk of mortality. Therefore, timely and accurate risk stratification is critical for guiding clinical decision-making. However, validated interpretable prediction models for short-term mortality in patients with both AMI and COPD remain limited. METHODS: We retrospectively extracted data from the MIMIC-IV database (version 3.1) and identified ICU patients with acute myocardial infarction complicated by chronic obs

Improving Emergency Care for Children With Medical Complexity: A Pragmatic Review.

Children with medical complexity (CMC) are defined by the presence of significant chronic health problems that affect multiple organ systems, often requiring medical technology, and result in functional limitations and high health care needs, including frequent emergency department (ED) visits. CMC are an estimated 1.5% of the pediatric population in the United States, yet account for 20% of pediatric ED encounters and 33% of total pediatric health care costs. Beyond high utilization, significant challenges in providing high-quality emergency care for CMC have been identified. Communication wi

The association of blood pressure variability with adverse outcomes in a primary care chronic kidney disease cohort.

BACKGROUND: Hypertension is common in individuals with chronic kidney disease and both conditions are associated with adverse outcomes including cardiovascular morbidity. Therefore, it is clinically important to identify methods of risk prediction in individuals with chronic kidney disease. Blood pressure variability has recently emerged as a predictor of cardiovascular events and mortality in the general population, with growing evidence indicating that it may play a similar role in individuals with chronic kidney disease. However, there have been no large studies assessing blood pressure var

Introducing Nurse Navigators to Improve Pain Management in the Community.

PURPOSE: Chronic pain among adults is a highly prevalent condition characterized by persistent gaps in care delivery, inequitable access to evidence-based interventions, and variable care continuity across clinical settings. Nurse Navigators are strategically positioned to help eliminate barriers by assisting in care coordination and person-centered resource maximization. A community-based nurse-led telehealth pilot program was implemented in the Northwestern United States as an extension of a state-funded evidence-based, online platform designed to promote nonpharmacologic pain management opt

Determinants of Self-Care Practices Among Family Caregivers of Older Adults with Chronic Illness: A Qualitative Descriptive Study.

BACKGROUND AND OBJECTIVES: Caregiver burden significantly impedes self-care, often resulting in poor health and increased vulnerability to chronic illness. Despite these risks, the underlying factors influencing caregivers' self-care remain understudied. This qualitative descriptive study explored the context of caregiver self-care and identified perceived barriers and facilitators to practicing self-care. RESEARCH DESIGN AND METHODS: We conducted four focus groups with 14 caregivers primarily responsible for providing in-home care to older adults (≥ 65 years) with chronic illnesses. Focus gr

Headache psychiatry: A structured narrative review and integrative framework.

Headache disorders involve complex interactions between neurobiological, psychiatric, behavioral, and social factors. Despite robust evidence of psychiatric comorbidity and psychosocial influences, these dimensions remain inadequately integrated into clinical practice and conceptual models. This review synthesizes evidence linking psychiatric factors with headache disorders and proposes Headache Psychiatry as a unified, integrative framework, operationalized through the Psychocephalology model. We conducted a structured narrative review across PubMed, PsycINFO, and Web of Science (1990-2025),

WHF Roadmap for Integrated Care in People Living with - or at Risk of - Cardiovascular Disease and Multiple Long-Term Conditions.

Cardiovascular disease (CVD) commonly coexists with multiple long-term conditions (MLTC), including diabetes, chronic kidney disease, obesity, and mental health disorders. This clustering creates a syndemic burden associated with poorer outcomes, polypharmacy, high treatment burden, and rising healthcare costs. Fragmented, single-disease care models are ill-suited to address this complexity. The WHF roadmap for integrated care in people living with - or at risk of - CVD and MLTC provides a structured framework to support the design, implementation, and scale-up of person-centred, coordinated c

Intraoperative pain management shared decision making with a patient with a complex pain and mental health history: A case report.

Shared decision making is where the patient and clinician come together to decide on the appropriate course of action in a patient's medical care based on their goals, preferences, and values. Typically, decision making in anesthesia remains more paternalistic, with clinicians making decisions for patients. However, there is a move towards embracing a shared decision making model. We present here a case report of using shared decision making principles to develop a pain and anxiety management plan in a patient with chronic pain and a complex mental health history. A 40-year-old female with a h
